1.Current Status and Optimization Strategies for Setting Objective Performance Criteria in Clinical Trials of Traditional Chinese Medicine

Current formulation of objective performance criteria (OPC) in traditional Chinese medicine (TCM) faces methodological challenges, including poor reporting quality, insufficient standardization of methods, limited comparability of study populations, and inadequate consideration of the genetic and environment particularities of ethnic minority populations. In response to these issues, this paper proposes optimization strategies for setting OPC in TCM, with particular attention to ethnic minority medicine. These include improving methodological techniques and statistical descriptions, ensuring transparency in meta-analyses and data sources; standardizing the estimation of point estimates and confidence intervals, and incorporating multidisciplinary expert consensus for value determination; introducing local real-world data from the same period, especially cohort study evidence, to compensate for the lack of high-quality randomized controlled trial data. Through process transparency and dynamic updating mechanisms, the credibility of OPC method can be enhanced, thereby providing methodological support for strengthening evidence-based research capacity in this field.
2.Construction of a value evaluation index system for health management and logistics positions in public hospitals

Objective To develop a value evaluation index system for health management and logistics positions in pub-lic hospitals,providing a research tool for future performance evaluations and salary reform.Methods Literature review,focus group interviews,and the point-factor method were employed to establish an initial index pool.A public hospital in Guangzhou was selected as a case study.Delphi expert consultation was then utilized to refine and finalize the indicators for the evaluation system.Results For the two rounds of Delphi expert consultation,the response rates were 87.5%and 100.0%,with the expert authority coefficient of 0.812.The Kendall's W coordination coefficients were 0.796 and 0.624 for the first round,and 0.747 and 0.918 for the second,(all<0.001).The coefficients of variation ranged from 0.157 to 0.265 for the first round and from 0.108 to 0.230 for the second round.Experts provided four suggestions,leading to the removal of one secondary indicator and modifications to the definitions of three others.Finally,the evaluation system consists of 4 primary indicators and 12 secondary indicators.Conclusion This system can provide a research tool and reference for the follow-up performance evaluation,improve the salary reform of public hospitals,and promote the high-quality development of public hospitals.
3.Association between the related gene polymorphisms and clinical features of Han population with type 2 diabetes mellitus in Qinhai region

Objective To investigate the association between polymorphic susceptibility loci for diabetes mellitus and clinical characteristics of type 2 diabetes mellitus(T2DM)in the Han Chinese population in the Qinghai region.Methods A total of 286 T2DM patients treated in Qinghai Provincial People's Hospital from December 2021 to July 2023 were selected as the study subjects.DNA samples were sequenced by Illumina sequencing.LASSO regression model and Kolmogorov-Smirnov(K-S)test were used to analyze the association between SNP and age of onset in T2DM patients.Logistic regression model was used to analyze the association between SNP and DM family history,FPG,HbA1c,HOMA-IR and HOMA-β.Results The age of onset,smoking history,drinking history and WC of male patients were higher than those of female patients(P<0.05),and the HOMA-β,HDL-C,LDL-C and TC were lower than those of female patients(P<0.05).LASSO regression model and K-S test analysis showed that 7 SNPs were associated with the age of onset of T2DM(P<0.05).Logistic regression analysis showed that 13 SNPs were correlated with family history of DM(P<0.05),13 SNPs were correlated with FPG(P<0.05),11 SNPs were correlated with HbA1c(P<0.05),20 SNPs were correlated with HOMA-IR(P<0.05),16 SNPs were associated with HOMA-β(P<0.05).Conclusions In Chinese Han patients with T2DM in Qinghai province,multiple SNPs variants may affect the disease phenotype through different biological pathways,including age of onset,blood glucose index and islet function.

This paper systematically summarizes the practical experience of the 2025 Dingri earthquake emergency medical rescue in Tibet. It analyzes the requirements for earthquake medical rescue under conditions of high-altitude hypoxia, low temperature, and low air pressure. The paper provides a detailed discussion on the strategic layout of earthquake medical rescue at the national level, local government level, and through social participation. It covers the construction of rescue organizational systems, technical systems, material support systems, and information systems. The importance of building rescue teams is emphasized. In high-altitude and cold conditions, rapid response, scientific decision-making, and multi-party collaboration are identified as key elements to enhance rescue efficiency. By optimizing rescue organizational structures, strengthening the development of new equipment, and promoting telemedicine technologies, the precision and effectiveness of medical rescue can be significantly improved, providing important references for future similar disaster rescues.

7.Effect of echinacoside intervention on liver and pancreas injury in rats with acute pancreatitis and its mechanism

ObjectiveTo investigate the effect and mechanism of echinacoside (ECH) in improving liver injury in rats with acute pancreatitis by establishing a rat model of acute pancreatitis and liver injury. MethodsA total of 24 Sprague-Dawley rats were randomly divided into blank group (Con group), control group (Con+ECH group), acute pancreatitis group (AP group), and acute pancreatitis+ECH intervention (AP+ECH group). The rats were given intraperitoneal injection of 10 mg/kg ECH on day 7 before the establishment of the model of acute pancreatitis; at 24 hours after the last administration of cerulein, blood samples were collected via the abdominal aorta, and serum was separated for biochemical analysis including alanine aminotransferase (ALT), aspartate aminotransferase (AST), lactate dehydrogenase (LDH), gamma-glutamyl transpeptidase (GGT), alkaline phosphatase (ALP), albumin (Alb), total bilirubin (TBil), cholinesterase, blood amylase (Amy), and lipase (LPS). HE staining was used to observe the histopathological changes of the pancreas and the liver; transmission electron microscopy (TEM) was used to observe the microstructural changes of pancreas and liver tissue; ELISA was used to measure the levels of interleukin-1β (IL-1β), interleukin-16 (IL-6), tumor necrosis factor-α (TNF-α), and interleukin-10 (IL-10) in liver tissue homogenate; immunohistochemistry was used to measure the levels of TNF-α and p-p65 NF-κB in pancreas and liver tissue; Western blot was used to measure the expression levels of NF-κB pathway proteins in liver tissue. A one-way analysis of variance was used for comparison of continuous data between multiple groups, and the SNK test or the Dunnett’s T3 method was used for further comparison between two groups. ResultsCompared with the Con group, the AP group had significant increases in ALT, AST, GGT, LDH, ALP, TBil, Amy, and LPS (all P<0.01), as well as significant increases in the levels of IL-1β, IL-6, IL-10, and TNF-α in liver tissue homogenate (all P<0.01). ECH intervention reduced the levels of ALT, AST, GGT, LDH, ALP, TBil, AMY, and LPS and inhibited the secretion of IL-1β, IL-6, and TNF-α in rats with acute pancreatitis. HE staining showed that ECH intervention alleviated the vacuolar degeneration of acinar cells, inflammatory cell infiltration in pancreatic tissue, and the necrosis of hepatocytes compared with the AP group. TEM showed that compared with the AP group, there was a reduction in the degree of mitochondrial swelling in liver and pancreatic cells after ECH intervention. ECH intervention partially reversed the elevated expression levels of p-p65 NF-κB and TNF-α in liver and pancreatic tissue. In addition, the expression levels of MyD88, p-IκBα, p-IKKα, and p-p65 were upregulated in liver tissue of rats with acute pancreatitis, which could be partially reversed after ECH intervention. ConclusionEchinacoside can alleviate liver and pancreatic injury induced by acute pancreatitis by inhibiting the TLR4/MyD88/NF-κB pathway.
8.Echinococcus multilocularis metacestode vesicle-derived EVs containing emu-miR-10-5p promote mouse hepatic stellate cell activation

This study investigated the role of Echinococcus multilocularis vesicle-derived extracellular vesicles(EVs)con-taining emu-miR-10-5p in regulating HSC activation in vitro.Mice were injected intraperitoneally with protoscoleces to con-struct a secondary alveolar echinococcosis model,and E.multilocularis vesicles were cultured in mice and identified by PCR.The EV structure in cultured vesicle tissue was observed by transmission electron microscopy.EVs were isolated from vesicle culture supernatants through differential-ultracentrifugation,and identified by transmission electron microscopy,nanoparticle size analysis,and western blotting.The activity of HSCs treated with different EV concentrations was detected with CCK-8 assays.The expression levels of α-SMA,and collagen Ⅰ and Ⅲ were detected by RT-qPCR and western blotting after EV treatment of HSCs.Bioinformatics analysis identified the specific miRNA emu-miR-10-5p.The proliferation of HSCs was detected with EdU after overexpression of emu-miR-10-5p,and the expression levels of α-SMA,collagen Ⅰ,and collagen Ⅲ in cells were detected by RT-qPCR and western blotting.Detection of α-SMA,collagen Ⅰ,and collagen Ⅲ expression levels in HSCs was performed after EVs were extracted from the culture supernatant of vesicular tissues with emu-miR-10-5p knockdown.E.multilocularis vesicles were successfully cultured from focal material in mice,and the E.multilocularis origin was confirmed through PCR amplification of specific genes.TEM observation of culture vesicles in-dicated abundant surface vesicular structures.E.multilocularis vesicle-derived EVs were isolated by differential-ultracentrifu-gation,and confirmed by TEM to be spherical and membrane like-structures.Vesicle-derived EVs were internalized by HSCs,and promoted the proliferation and activation of HSCs.Comprehensive analysis of miRNA sequencing results revealed emu-miR-10-5p expression in E.multilocularis-infected mouse serum,metacestode tissue,E.multilocularis-derived EVs,germi-nal layer cells,and protoscoleces.Moreover emu-miR-10-5p was confirmed to be present in higher amounts in E.multilocu-laris-derived EVs,according to RT-qPCR.Further studies confirmed that overexpression of emu-miR-10-5p promoted HSC proliferation and increased α-SMA,collagen Ⅰ,and collagen Ⅲ mRNA and protein expression levels in HSCs.In contrast,EVs extracted from culture supernatants after emu-miR-10-5p knockdown in vesicles did not increase a-SMA,collagen Ⅰ,and colla-gen Ⅲ mRNA and protein expression levels.E.multilocularis vesicle-derived EVs and emu-miR-10-5p thus promote the acti-vation of HSCs.
9.Relationship between polymorphism of resistin gene and metabolic associated fatty liver disease in type 2 diabetes mellitus patients in middle and high altitude areas

Objective To investigate the relationship between polymorphism of resistin(RETN)gene and metabolic associated fatty liver disease(MAFLD)in type 2 diabetes mellitus(T2DM)patients in middle and high altitude areas.Methods A total of 400 patients with T2DM in Qinghai area were recruited and divided into simple T2DM group(T2DM,n=200)and T2DM combined with MAFLD group(T2DM+ MAFLD,n=200)according to liver ultrasonography.Healthy individuals confirmed by physical examination were selected as the normal control group(NC,n=180).Plasma resistin levels were measured by ELISA.The polymorphism of RETN-420C/G and +299G/A genes were detected by PCR sequencing.Results By comparing the polymorphism of RETN-420C/G gene in each group,it was found that the frequencies of G/G genotype and G allele frequency in T2DM+MAFLD group were higher than those in NC group and T2DM group(P<0.05),while the frequencies of C/C genotype and C allele frequency were lower than those in NC group and T2DM group(P<0.05).The risk of MAFLD increased by 1.571,2.126 and 1.537 times respectively in T2DM patients with C/G,G/G genotype and G allele.Logistic regression analysis showed that G/G genotype was a risk factor for MAFLD in T2DM patients.By comparing the polymorphism of RETN+299G/A gene in each group,it was found that A allele frequency in T2DM+MAFLD group was higher than that in NC group and T2DM group,while G allele frequency was lower than that in NC group and T2DM group(P<0.05).The allele A increased the risk of MAFLD in T2DM patients by 1.432 times compared to allele G.Conclusion RETN gene-420C/G locus G/G genotype increases the risk of T2DM combined with MAFLD in middle and high altitudeareas.
10.Radiological imaging evaluation of children's intracranial central nervous system malformations in Xigaze city

Objective To summarize the imaging findings of intracranial central nervous system(CNS)malformations children in the high-altitude plateau(Xigaze city).Methods We retrospectively reviewed the imaging data of the suspected patients having CNS malformations,which enrolled either by the clinic or by the ultrasound in the Shigatse People's hospital from June,2019 to June,2023.All imaging data were interpreted by two experienced radiologists with consensus reading.Imaging characteristics were recorded roughly by primary/secondary,supra-/subtentorial malformation type and the corresponding clinical symptoms.Results A total of 36 children were included in this study,including 19 patients≤1 year old(including 2 newborns),7 patients between 1 and 3 years old,and 10 patients>3 years old.Seven cases underwent MRI examination,others having CT scan.Polygyria and pachygyria malformation were the most common type of congenital neurological malformations(7 cases,31.8%),followed by cystic changes of the cerebral parenchyma(3 cases,13.6%).Cerebral atrophy was the most common type of secondary CNS abnormalities group(8 cases,57.1%),followed by communicative hydrocephalus(3 cases,21.4%).Five patients in the congenital group and 4 patients in the secondary CNS group had the complex malformations.There were 8 dead cases(all in the≤1-year-old group),12 cases having neurological sequelae,1 case with normal development and 15 cases with loss of follow-up.There were no significant differences between primary and secondary CNS group regarding the final outcome.Conclusion In this study,gyrus developmental malformation and encephalatrophy were the most common etiologies in infants'CNS malformations.Reasonable use of imaging modalities will help depict the complicated CNS malformations and design individual treatment.
