Objective:To assess the current clinical application status of provocation/stress echocardiography in hypertrophic cardiomyopathy(HCM)among echocardiography physicians from large grade A tertiary hospitals, located in 24 provinces or directly administered municipalities, and to achieve a relatively clear and comprehensive overview of the current clinical application status of provocation/stress echocardiography in HCM. This study was conducted by the Chinese Society of Ultrasound in Medicine(CSUM)and Chinese Society of Echocardiography(CSE).Methods:An online survey was anonymously conducted using Question Star application from 20 March to 30 September 2023. The survey covered the following topics including the echocardiographic diagnostic overview, the selection of views and measurement parameter of echocardiography, and the cognition and application, awareness and management of risk, and clinical demand of provocation/stress echocardiography. Results:A total of 337 valid responses were included in the final statistical analysis. The study revealed that the number of HCM patients seen by echocardiography physicians was very few(0-10%),with a low proportion of diagnosed obstructive HCM patients. There was incomplete mastery of the left ventricular outflow tract(LVOT)obstruction criteria(68.25%),insufficient awareness of the importance of LVOT pressure gradient measurement(7.12% echocardiographic doctors routinely performed LVOT gradient measurement for suspect HCM patients),non-standardized selection of echocardiographic views and measurement parameters for HCM,and significant deficiencies in knowledge and application of Valsalva provocation/stress echocardiography(17.21% and 79.23% doctors were quite aware of the principles of Valsalva provocation/stress echocardiography, respectively). The risk awareness of provocation/stress echocardiography(13.65% physicians were fully aware of the potential risk)and the ability to manage associated risks(19.29% physicians were able to handle all emergencies)were lacking. Existing guidelines did not sufficiently standardize the clinical practice of provocation/stress echocardiography(23.44%),and there was diversity in learning formats requirements and unmet clinical needs. Conclusions:This survey revealed insufficient fundamental theoretical knowledge and a need for further standardization and training in the application of provocation/stress echocardiography for HCM among echocardiography physicians. Additionally, risk awareness and adequate response skills need improvement. There is a need for more practical and guiding guidelines for clinical practice,as well as numerous unmet clinical needs.

Objective:To explore the effect of physical activity trajectories on cognitive function in middle-aged and older adults. Methods:Based on data from the China Health and Retirement Longitudinal Study (CHARLS) from 2011 to 2020, 809 participants aged ≥45 were selected. General data, physical activity levels, and cognitive function were collected. The group-based trajectory model (GBTM) was used to establish the physical activity trajectory model, and multiple linear regression was applied to investigate factors affecting cognitive function in middle-aged and older adults and to analyze the relationship between physical activity trajectories and cognitive function. Results:The cognitive function score for the 809 middle-aged and older adults was (11.71±4.60), and the logarithmic metabolic equivalent of physical activity was (8.38±1.06). Based on GBTM, four physical activity trajectories were identified: stable group, slow-increase group, rapid-decrease group, and stable-decrease group. Multiple linear regression analysis showed that the stable and slow-increase activity trajectories, gender, age, marital status, and retirement status were significant factors influencing cognitive function in middle-aged and older adults ( P<0.01) . Conclusions:Physical activity trajectories are associated with cognitive function in middle-aged and older adults. Maintaining stable or slowly increasing physical activity is beneficial for cognitive health. Nurses should design targeted interventions based on factors influencing cognitive function.

Objective:To investigate the similarities and differences of Doppler echocardiographic parameters in healthy adults among Chinese (EMINCA study), Japanese (JAMP study) and Europeans (NORRE study). Methods:Based on the published Doppler echocardiographic data of JAMP and NORRE studies, the corresponding Doppler parameters were selected from the original database of EMINCA study and stratified by sex and age groups accordingly. Independent-samples t-test, summary t-test, and one-way analysis of variance (ANOVA) were used to test the differences between groups. Results:①Common Doppler parameters in three studies included: peak velocities of early diastolic (E) and late diastolic waves (A) of mitral valve inflow, E/A ratio, deceleration time (DT) of E wave, tissue velocities of early diastole (e′ ) and late diastole (a′ ) at both septal and lateral sites of the mitral annulus and the average value of e′ at both sites (average-e′), ratios of E to septal e′ (septal-E/e′), E to lateral e′ (lateral-E/e′), and E to average e′ (average-E/e′). ②Differences between genders: There existed significant gender differences for 5/10 of parameters in EMINCA study, 8/10 in JAMP study and 3/12 in NORRE study (all P<0.05). ③Stratification by sex and age groups: Except for DT for men in EMINCA study, significant differences were found in all 12 Doppler parameters both for men and women in all three studies (all P<0.05). ④Comparison between EMINCA and JAMP studies: There were statistically significant differences in 8/10 and 9/10 of parameters for men and women, respectively (all P<0.05). ⑤Comparison between EMINCA and NORRE studies: 7/12 of parameters both in men and women were significantly different between the two studies (all P<0.05). ⑥For the three studies, 6/10 of parameters both in men and women correlated positively with age ( r=0.12-0.66, all P<0.001), while 4/10 correlated negatively with age ( r=-0.27--0.72, all P<0.001). Conclusions:Doppler echocardiographic measurements in healthy adults are significantly different with genders, ages and races/nationalities. Therefore, gender, age and race/nationality-specific reference values of Doppler echocardiography parameters should be encouraged in clinical practice.
