1.Noonan syndrome with entropion and epicanthic folds: a case report

An 8-year-old female patient was admitted to the hospital because of foreign body sensation and lacrimation in both eyes for more than 5 years, and was diagnosed with " Noonan syndrome: (1) entropion and trichiasis; (2) congenital epicanthic folds; (3) ptosis; (4) corneal epithelial damage; (5) nystagmus; (6) ametropia in both eyes" based on her medical history and physical signs. After consultations with ophthalmology, pediatrics and anesthesiology departments, correction of entropion + blepharoplasty + medial canthoplasty in both eyes were performed under general anesthesia. Postoperatively, the patient’s lower eyelid entropion, trichiasis and epicanthic folds were completely corrected. No recurrence was observed during the six-month follow-up. Noonan syndrome with ocular abnormalities is rarely reported. This article summarized the diagnosis and treatment of this case to remind clinicians to pay attention to the facial and systemic manifestations of patients with eyelids abnormity, so as to reduce the misdiagnosis of Noonan syndrome and related medical safety risks.
2.Functional study of M1/M2 macrophages in the progression of oral squamous cell carcinoma

Objective:This study aimed to identify potential prognostic and biotherapeutic targets for oral squamous cell carcinoma(OSCC)by investigating the regulatory effects of M1/M2 macrophages on OSCC progression and their underlying mechanisms.Methods:The ef-fects of M1/M2 macrophages on the proliferation,migration,invasion,and apoptosis of SCC-15 cells were examined.Immunohistochemical staining was used to detect the expression of CD68 and M2 macrophage marker proteins in human OSCC tissue microarrays,and their cor-relations with clinicopathological features were analyzed.Results:M1 macrophages promoted apoptosis of SCC-15 cells while inhibiting their proliferation,migration,and invasion.In contrast,M2 macrophages exhibited the opposite effects.The infiltration density of tumor-associ-ated macrophages(TAMs)in OSCC tissues was significantly positively correlated(P<0.05)with lymph node metastasis.Conclusions:M1 mac-rophages suppress OSCC progression,whereas M2 macrophages promote it.
3.The modern pharmacological effects of Yujin and its research progress in neurodegenerative diseases

The active ingredients of Curcuma aromatica have multi-target effects and could intervene in the process of neurodegenerative diseases from multiple perspectives, such as promoting the clearance of Aβ, anti-inflammatory, anti-oxidant, neuroprotective, anti-depressant, and anti-tumor effects. However, the low bioavailability of Curcuma extracts limits their therapeutic effects in the body. In addition, the side effects of long-term use and the dose-effect relationship are still unclear, and more new pharmacodynamic observations are needed. This article reviews the research on single-ingredient and compound traditional Chinese medicine preparations of Curcuma to provide references for clinical medication.
4.Exploration and reflection on the mutual recognition of medical ethical review results in the Beijing-Tianjin-Hebei region:based on the promotion of tertiary medical and health institutions in Tianjin

The construction of medical ethical review is the basic guarantee for strengthening the ethical governance of medical science and technology,and mutual recognition of ethical review results serves as a pivotal approach to accelerate the construction of ethical review.This paper conducted a questionnaire survey on 30 tertiary medical and health institutions with clinical trial qualifications in Tianjin,based on the exploration of the implementation of the Notice on the Implementation of Mutual Recognition of Medical Ethics Review Results in the Beijing-Tianjin-Hebei Region jointly issued by the Health Committees of the three regions in Tianjin's tertiary medical and health institutions.By investigating and analyzing the situation of clinical trials,ethics committees,and mutual recognition from 2021 to 2023,the regular reasons affecting the promotion of mutual recognition of ethical review results in the Beijing-Tianjin-Hebei region were identified,and reasonable recommendations were proposed.These aimed to offer references for the Tianjin region in deepening the integration of Beijing-Tianjin-Hebei development and accelerating the promotion of regional ethical mutual recognition work,thereby advancing the construction and development of medical ethical review.
5.Pediatric tumor predisposition syndrome:Management strategy and whole-body MRI screening

Pediatric tumor predisposition syndrome(TPS)is a heterogeneous group of diseases caused by inherited gene mutations or germline mutations,which increasing risk of tumors.Screening pediatric TPS remains a multidisciplinary,multimodal and longitudinal management process.Imaging techniques are beneficial for early detection of tumors reducing mortality rates.Whole-body MRI,combined with multidisciplinary follow-up,including pediatric oncology,pediatric surgery,endocrinology,radiology,hematology and genetics,is helpful to management of pediatric TPS.The research progresses in management strategy and whole-body MRI screening of pediatric TPS were reviewed in this article.
6.Correlation between psoas major index of diffuse large B-cell lymphoma patients and prognosis after R-CHOP chemotherapy

Objective To explore the correlation between psoas major index(PMI)in patients with diffuse large B-cell lymphoma(DLBCL)and prognosis after rituximab,cyclophosphamide,doxorubicin,vincristine,prednisone(R-CHOP)chemotherapy.Methods Totally 148 DLBCL patients who received R-CHOP chemotherapy were retrospectively enrolled.Based on axis CT image at L3 vertebral transverse process,bilateral psoas muscles were delineated,and PMI was calculated.Then the patients were divided into muscle reduction group(≤lower quartile,n=37)and control group(>lower quartile,n=111)according to the lower quartile of PMI.Clinical and imaging data were compared between groups,and the correlations of PMI with the above indexes were analyzed.The impact factors of prognosis of DLBCL were observed.Results Significant differences of body mass index(BMI),Eastern Cooperative Oncology Group(ECOG)score,international prognostic index(IPI),the proportion of B symptoms,mortality rate,3-year survival rate,β2-microglobulin,hemoglobin,albumin and PMI were found between groups(all P<0.05).PMI of DLBCL patient was negatively correlated with age,ECOG score,IPI,β2-microglobulin and lactate dehydrogenase(LDH)(r=—0.126,-0.225,—0.262,—0.232,—0.229,all P<0.05),but positively correlated with BMI and hemoglobin(r=0.282,0.376,both P<0.05).The age,ECOG score,IPI and PMI were all independent impact factors of overall survival in DLBCL patients(all P<0.05).Conclusion PMI was an independent impact factor of prognosis of DLBCL after R-CHOP chemotherapy.
7.Pro-tumorigenic mechanisms of tumor-associated macrophages in oral cancer

Oral cancer is a common inflammation-associated malignant tumor of the head and neck.In recent years,the role of the tumor microenvironment in the progression of oral cancer has received increasing attention.In the microenvironment of solid tumors,including or-al cancer,tumor-associated macrophages(TAMs)are among the most common type of immune cell and are closely associated with the poor prognosis of more than 80%of human malignant tumors.An increasing body of evidence indicates that TAMs play important roles in the oc-currence and progression of oral cancer.On the basis of the most recent research progress,both domestic and international,we systematic-ally reviewed the phenotypic characteristics,clinicopathological correlations,and molecular regulatory mechanisms of TAMs in the microen-vironment of oral squamous cell carcinoma(OSCC).In particular,we focus on the key signaling pathways promoting tumorigenesis and de-velopment and immune escape mechanisms,with the aim of constructing a systematic theoretical framework for developing therapeutic strategies targeting TAMs and providing a scientific basis for clinical translational research.
8.Practice of medical insurance management for ambulatory surgery from the perspective of DRG/DIP

Objective With the comprehensive implementation of Diagnosis-Related Groups(DRG)and Diagnosis-In-tervention Packet(DIP)payment reforms,ambulatory surgery has been widely adopted in clinical practice due to its efficiency and cost-effectiveness.However,medical insurance compliance issues have become increasingly prominent during management processes.Standardizing ambulatory surgery management and ensuring the security of medical insurance funds have grown in im-portance.Methods Based on the current status of ambulatory surgery medical insurance policies in Tianjin,this study analyzes common compliance issues under DRG/DIP payment systems,explores their causes,and proposes targeted management strate-gies.Results Tianjin currently has 786 ambulatory surgery procedures,with orthopedics being the most common specialty,fol-lowed by ophthalmology,urology,and general surgery.From 2020 to 2024,the sample hospital completed 12,705 ambulatory surgery cases,with a significant increase in 2024.In 2020,the most frequent procedure was circumcision,while in 2024,it was colonoscopic polypectomy.Under DRG/DIP payment systems,common medical insurance compliance issues include upeoding,split hospitalization,excessive treatment,cost shifting,and undertreatment.Conclusion Healthcare and medical insurance sys-tems should collaborate to implement targeted management strategies.These include strengthening hospital medical record and coding management,improving medical quality control,refining medical insurance supervision mechanisms and payment stand-ards,and establishing a multi-stakeholder collaborative oversight system.These measures aim to standardize ambulatory surgery management,safeguard medical insurance funds,and promote the healthy development of the healthcare industry.
9.Percutaneous vertebroplasty via Kambin's triangle for treatment of osteoporotic compression fractures:evaluation of safety and effectiveness

BACKGROUND:Currently,the main purpose of surgical treatment for lumbar compression fractures is to improve symptoms,reduce pain,improve quality of life,minimize surgical risks,and reduce surgical exposure time.OBJECTIVE:To explore the safety and clinical efficacy of percutaneous vertebroplasty via Kambin's triangle in the treatment of senile spinal deformity accompanied by osteoporotic vertebral compression fractures.METHODS:A retrospective analysis was conducted on 31 patients with spinal deformity and osteoporotic vertebral compression fractures who were admitted to Tianjin Union Medical Center from January 2019 to December 2022.There were 21 males and 10 females,aged between 60 and 84(70.0±10.3)years old.The duration of illness ranged from 1 to 6(3.5±1.7)weeks.A total of 35 vertebral segments were affected.The distribution of vertebral compression fractures included:10 cases of L1 fracture,12 cases of L2 fracture,2 cases of L1+L2 fracture,5 cases of L3 fracture,and 2 cases of L1+L3 fracture.Based on the surgical approach,patients were divided into two groups:group A(via Kambin's triangle approach)with 15 cases,and group B(via traditional pedicle puncture)with 16 cases.Unilateral approach vertebroplasty was performed on both groups.The surgical duration,number of radiation exposures,presence of bone cement leakage,occurrence of complications such as nerve and vascular injuries were recorded in both groups.Postoperative CT scans were used to observe the dispersion and hardening of bone cement.RESULTS AND CONCLUSION:(1)In the group A,the surgical duration was 21-30 minutes per vertebra,with an average of(25.0±5.7)minutes.In the group B,the surgical duration was 25-43 minutes per vertebra,with an average of(33.0±7.2)minutes.The surgical duration for puncturing a single vertebra was significantly longer in the group B compared to the group A,with a statistically significant difference(P<0.05).(2)The average number of radiation exposures per vertebra during surgery was(6.2±1.6)in the group A and(9.3±1.8)in the group B,with a statistically significant difference between the two groups(P<0.05).(3)In the group A,no bone cement leakage was found.In group B,2 cases had bone cement leakage,and 1 case had bone cement entering the spinal canal,but no obvious neurological symptoms were observed.The leakage rate was 13%.Both groups had 1 case of subcutaneous hematoma.(4)Postoperative CT scans for observing the distribution of bone cement in the vertebral body showed that the rates of uniform dispersion of bone cement within the vertebrae were 71%and 33%in the groups A and B,respectively,with a statistically significant difference between the two groups(P<0.05).(5)It is suggested that percutaneous vertebroplasty via Kambin's triangle approach offers advantages such as shorter surgical duration,better dispersion and filling of bone cement within the vertebrae,lower risk of complications,reduced radiation exposure,and satisfactory results with unilateral puncture.
10.Efficacy of selective expansive opendoor laminoplasty in the treatment of multisegmental cervical spondylotic myelopathy

Objective To evaluate the clinical efficacy of selective expansive open-door laminoplasty(SEOLP)with preservation of C7 spinous process in the treatment of multisegmental cervical spondylotic myelopathy and its impact on changes in sagittal parameters of cervical spine.Methods A retrospective analysis was conducted on the clinical data and radiological information of 73 patients who underwent expansive open-door laminoplasty(EOLP)for cervical spondylotic myelopathy in our department between March 2018 and June 2022.Patients were divided into the SEOLP group(n=35)and the EOLP group(n=38)based on the surgical method.Follow-up was conducted for one year.The operation time,blood loss,axial symptom scores,JOA scores,VAS scores and neck disability index(NDI)were recorded in two groups of patients.Radiological data were also recorded for both groups during the perioperative period,and the C2-7 Cobb angle,C2-7 SVA and T1 slope were measured.The cervical curvature index(CCI)and cervical range of motion(ROM)were calculated.The perioperative clinical outcomes and changes in cervical sagittal parameters were observed,and their correlations were analyzed.Results There were no significant differences in blood loss,operation time,JOA scores at various follow-up time points between the two groups(P>0.05).During postoperative follow-up,axial symptoms were observed in 5 patients in SEOLP group and 14 patients in EOLP group.There were statistically significant differences in axial symptom scores,incidence and severity of axial symptoms between the two groups(P<0.05).The NDI indices at one year after operation were 21.1±2.3 for SEOLP group and 24.8±3.5 for EOLP group respectively(P<0.01).There were no statistically significant differences in T1 slope and C2-7 Cobb angle at various follow-up time points after surgery between the two groups(P>0.05).One year after operation,CCI indices for two groups were(13.4±2.7)and(12.1±2.4),respectively,with a statistically significant difference(t=2.178,P<0.05).The C2-C7 SVA values for two groups at one year after operation were(22.4+3.8)mm and(26.7±5.9)mm,respectively(t=3.667,P<0.01).The results of the correlation analysis showed that there was a significant negative correlation between clinical functional improvement(NDI)and changes of the radiological parameter C2-C7 SVA in both groups of patients.Conclusion After SEOLP,the recovery of C2-C7 SVA is faster and has less impact on cervical spine function,and the occurrence degree and incidence of axial symptoms are lower.
