1.Network Meta-analysis of traditional Chinese medicine compound prescription combined with western medications for treating coronary heart disease with depression

Objective To assess the effectiveness of various traditional Chinese medicine(TCM)compound prescription combined with conventional western medications for treating coronary heart disease with depression through network Meta-analysis.Methods Search CNKI,Wanfang data,VIP,SinoMed,PubMed,Web of Science,Embase,Cochrane Library by computer.Randomized controlled trials of TCM compound prescription combined with conventional Western medications in the treatment of coronary heart disease with depression were collected from established to March 17,2024.The quality of the literatures was assessed using RoB 2.0 software,and network Meta-analysis was conducted using Stata 16.0 software.Results A total of 41 iteratures were included,involving 7 efficacy combinations.The results of the analysis showed that the highest clinical efficacy was found in the heat-clearing and tranquilizing category of TCM compound prescription+conventional western medicine[surface under the cumulative ranking curve(SUCRA)=79.4%],which significantly improved Hamilton depression scale scores(SUCRA=84.4%).The regulating qi type of TCM compound prescription+conventional western medicine significantly improved TCM symptoms(SUCRA=68.4%)and depression-related symptoms(SUCRA=94.8%).Conclusion Compared to using conventional Western medicines alone,the combined application of TCM compound prescription proves to be more effective,with regulating qi being crucial for treating this condition.
2.Analysis on Clinical Trial Registration of TCM in the Treatment of Coronary Artery Microvascular Diseases

Objective To review clinical trial registration status of TCM treatment for coronary microvascular disease;To analyze the effectiveness and safety of TCM in treating coronary microvascular disease.Methods The clinical trials of TCM in the treatment of coronary artery microvascular disease included in the Chinese Clinical Trials Registry and the US Clinical Trials Registry from the establishment of the database to January 31,2024 were retrieved,and the general characteristics(time,region,funding source),design type,intervention regimen and outcome indicators of the included clinical trials were extracted and analyzed using Excel 2019 software.Results A total of 17 clinical trials were included,including 16 pre-registrations.The registered units were distributed in 5 provinces across the country,involving 17 registration institutions.The two regions with the most distribution were Shanghai(6 studies,35.29%)and Beijing(5 studies,29.41%).The types of studies were mainly interventional studies,and most of the study designs were randomized parallel controlled studies(16 studies,94.12%).A total of 8 interventions were reported,including Chinese patent medicine,TCM decoction,TCM intravenous preparation,and acupuncture and moxibustion.A total of 143 outcome indicators were designed,including 10 first-level indicators,including coronary artery microcirculation,clinical efficacy,symptoms and signs,TCM syndromes,quality of life,exercise tolerance,cardiac function,physical and chemical testing,safety,and pharmacoeconomic evaluation.Conclusion The registration of clinical trials by TCM in the treatment of coronary microvascular diseases has been gradually receiving attention from researchers,but the overall number is still small.At present,the study needs to be optimized in terms of study design and index selection.
3.Exploration on Shengmai Decoction Targeting Bcl-2/Bax/Caspase-3 Pathway to Mediate Mitochondrial Apoptosis in Treatment of Heart Failure Based on Network Pharmacology

Objective To explore the mechanism of Shengmai Decoction in improving heart failure by regulating mitochondrial apoptosis based on network pharmacology and experimental verification;To provide a basis for its clinical application.Methods The active components and targets of Shengmai Decoction were obtained through TCMSP and TCMID databases.Heart failure and mitochondrial apoptosis related targets were retrieved from GeneCards,OMIM,DrugBank,PharmGKB and TTD databases.A protein-protein interaction(PPI)network at the intersection of drugs and diseases was constructed using the STRING database,and a component-target network was constructed using Cytoscape 3.8.2 software.GO and KEGG pathway enrichment analysis was performed on intersecting targets using the DAVID database.Molecular docking was conducted to assess the binding affinity of key components to core target proteins.A rat model of heart failure was established,and the cardiac function of rats were detected by echocardiogram,ELISA was used to detect serum contents of BNP and NT-proBNP,mitochondrial ultrastructure was observed by transmission electron microscope,and Western blot was used to detect protein expressions of Bcl-2/Bax/Caspase-3 signaling pathway.Results A total of 66 active components and 146 targets of Shengmai Decoction were identified,and 22 intersecting targets with heart failure and mitochondrial apoptosis,including key proteins such as TNF,Bcl-2,Bax and Caspase-3.GO enrichment analysis revealed that the intersecting targets were closely associated with inflammation and cell signaling,while KEGG pathway analysis showed that the targets were primarily involved in the TNF signaling pathway,IL-17 signaling pathway,and mitochondrial function regulation.Molecular docking results indicated that Ginsenoside rh2,Beta-sitosterol and other components of Shengmai Decoction exhibited strong binding affinities with Bcl-2,Bax and Caspase-3.Animal experiments demonstrated that Shengmai Decoction significantly improved cardiac function(P<0.05,P<0.01),reduced serum BNP and NT-proBNP contents(P<0.01),alleviated mitochondrial damage,and inhibited mitochondrial apoptosis by upregulating Bcl-2 and downregulating Bax and Caspase-3 protein expression(P<0.01),thereby protecting cardiomyocytes.Conclusion Shengmai Decoction can target and regulate the Bcl-2/Bax/Caspase-3 signaling pathway,improving pathological processes associated with heart failure,which can provide a foundation for clinical optimization of Shengmai Decoction.
4.High-frequency transcranial magnetic stimulation is more effective than lower frequencies in improving the upper limb function of stroke survivors

Objective:To compare the effect of transcranial magnetic stimulation (rTMS) of the contralesional hemisphere at different frequencies on the recovery of upper limb motor function after a moderate-to-severe ischemic stroke.Methods:The inter-hemisphere compensation model was applied along with electroencephalogram (EEG) power spectrum density measurements. Thirty stroke survivors were randomly assigned to a sham stimulation group ( n=9), a high-frequency stimulation group ( n=11) or a low-frequency stimulation group ( n=10). In addition to physical and pharmacological therapy, the low-frequency and high-frequency groups received 1Hz or 5Hz rTMS, while the sham group received sham stimulation. The rTMS was delivered over the contralesional (unaffected) hemisphere once daily for 20 minutes over 15 consecutive days. Before, as well as 7 and 15 days after the treatment, all of the subjects′ motor functioning was assessed using the Fugl-Meyer Assessment for the upper extremity (FMA-UE) and their ability in the activities of daily living was assessed using the modified Barthel Index (MBI). Resting-state EEGs with the eyes closed were also recorded, and absolute alpha power across the whole brain was calculated. Changes from baseline FMA-UE and MBI scores and absolute alpha power were analyzed using one-way and repeated-measures analysis of variance. Results:After the treatment, significant within-group improvements from baseline were observed in the FMA-UE scores, MBIs and absolute alpha power, except for absolute alpha power in the low-frequency and sham groups. The repeated-measures analysis of variance revealed significant time × group interactions for FMA-UE ( F=9.926, P≤0.001), MBI ( F=8.789, P≤0.001) and absolute alpha power ( F=4.511, P≤0.05). So the treatment effects varied among the groups. Post hoc Bonferroni-corrected comparisons showed that the high-frequency group exhibited significantly greater improvements from baseline in terms of all three indicators compared with the other two groups. Conclusions:High-frequency (5Hz) rTMS applied to the contralesional hemisphere produced greater improvement than low-frequency (1Hz) stimulation in the upper limb motor function of patients with moderate-to-severe stroke. These findings support the use of the interhemispheric compensation model to guide rTMS therapy, particularly for patients with FMA-UE scores below 43.
5.Research Progress in the Clinical Efficacy and Mechanism of TCM for the Treatment of Functional Dyspepsia with Liver Depression and Spleen Deficiency Syndrome

Functional dyspepsia(FD)is a common functional gastrointestinal disease in clinical practice,which has the characteristics of high incidence,difficult to cure,and recurrence.FD belongs to the categories of"ruffian"and"stomach pain"in TCM,and the disease is located in the stomach,which is closely related to the liver and spleen,and the syndrome of liver depression and spleen deficiency is the most common.This article summarized the literature related to the TCM treatment for FD with liver depression and spleen deficiency syndrome,and concluded the clinical application,efficacy characteristics and mechanism,so as to provide reference for clinical treatment and basic research.The analysis found that the clinical efficacy of TCM in the treatment of FD is significant,which can not only improve the digestive symptoms of patients,but also improve their anxiety and depression state and daily life quality,and has the characteristics of overall regulation,syndrome differentiation and treatment,and improvement of physical fitness.Its mechanism may involve multiple pathways and levels such as abnormal gastric motility,abnormal brain-intestinal interaction and immune inflammatory response.
6.Analysis on the Current Situation of Outcome Indicators in Randomized Controlled Trials of Acupuncture Treatment for Chronic Fatigue Syndrome

Objective To study the current status of the randomized controlled trial(RCT)outcome indicators of acupuncture and moxibustion in the treatment of chronic fatigue syndrome(CFS);To provide guidance and references for clinical research and protocol design.Methods RCT of acupuncture and moxibustion for CFS were retrieved from CNKI,Wanfang Data,VIP,SinoMed,PubMed,Embase,Web of Science databases from January 1,2015 to December 9,2024.Through literature screening and data extraction,Excel 2019 was used to make a descriptive analysis of the basic characteristics,diagnostic criteria,intervention measures,classification of outcome indicators,use frequency and evaluation time points of the included studies.Origin2021 software was used to draw bubble charts for visual display.Results Totally 119 RCT were included,involving 8 445 subjects,5 diagnostic criteria of Western medicine,5 diagnostic criteria of TCM,and 6 outcome indicators,namely clinical symptoms/signs,symptoms/syndrome of TCM,physical and chemical examination,quality of life,safety evaluation,and patient satisfaction.A total of 58 outcome indicators were reported,with a cumulative frequency of 430 times.Conclusion The RCT of acupuncture and moxibustion treatment of CFS have some problems,such as incomplete baseline assessment,lack of sample size calculation basis,high risk of bias,ignorance of primary and secondary indicators in the selection of outcome indicators,numerous indicators selection and measurement points,lack of long-term follow-up,inconsistent evaluation criteria for TCM syndrome efficacy,and insufficient attention to health economics evaluation and safety outcome indicators.
7.Study on Objective Characteristics of Tongue in TCM Diagnosis of Primary Immune Thrombocytopenia

Objective To explore the objective characteristics of tongue in primary immune thrombocytopenia(ITP);To provide references for accurate syndrome differentiation.Methods Totally 112 patients with ITP were divided into groups according to their platelet count,TCM symptoms and drug treatment lines.Infrared thermal imager,hygrometer,colorimetry and image analysis software were used to detect the temperature,humidity,colorimetry and morphology of tongue body(TB),tongue coating(TC)and sublingual vessel(SV),and analyze their characteristics.The tongue was divided into five areas:tip of tongue(T),middle of tongue(M),behind of tongue(B),middle left of tongue(ML)and middle right of tongue(MR),and its temperature was measured.Humility(H)of tongue and tongue coating were measured by hygrometer.The images of tongue body,tongue coating and sublingual collaterals were taken,and the chromaticity indexes of R,G,B,L,a,b were analyzed by Adobe Photoshop 2023.The width,length and tortuosity of sublingual collaterals were under morphological analysis.Results ① Comparison of platelet count groups:compared with 30-50 platelet count group and more than 100 platelet count group,the TB-R of 51-100 platelet count group decreased(P<0.05,P<0.01),and compared with 30-50 platelet count group,the TB-a of 51-100 platelet count group decreased(P<0.01);compared with platelet<30 group and platelet 30-50 group,the TC-H of platelet>100 group increased(P<0.05,P<0.01);compared with platelet>100 group,the length of sublingual collaterals in platelet<30 group was longer(P<0.05).② Comparison of different TCM symptom groups:compared with the bleeding group and asymptomatic group,the M and SV-B in the fatigue group decreased(P<0.05,P<0.01);compared with the asymptomatic group,the TC-b in the anxious group was significantly higher(P<0.05).③ Comparison of administration groups:compared with the Danazol group and Chinese materia medica group,the TC-H in the glucocorticoid group was significantly higher(P<0.05,P<0.01).Conclusion There are differences in tongue characteristic indexes among ITP patients with different platelet counts,TCM symptoms and drug treatment lines,which can measure of tongue indexes to provide references for accurate syndrome differentiation of ITP.
8.Analysis of the mechanism of acute hypotension:Bioinformatics-driven target screening and pathway mining

Objective Based on multi-dimensional bioinformatics technology,the molecular pathological mechanism of acute hypotension is systematically analyzed.Methods Integrate the gene expression profile of the Gene Expression Omnibus database(GSE2401),use limma package(R software)to screen differential genes,and optimize targets through data dimensionality reduction(log2(FC)>1,P<0.05);Further combine Gene Ontology,Kyoto Encyclopedia of Genes and Genomes and gene set enrichment analysis pathway enrichment and STRING-Cytoscape(MCODE,CytoHubba plug-in)to build a protein-protein interaction(PPI)network to mine core genes;realize the full-chain analysis of"differential gene-functional pathway-PPI network".Results 676 differential genes(304 upregulated,372 downregulated),ribosome structural components(Rps8,Rps27,Rpl35,etc.)and multiple pathways such as forkhead box protein and cyclic adenosine monophosphate were found to coordinate the regulation of acute hypotension.Conclusion This study uses a low-cost and efficient bioinformatics analysis framework to reveal the association between ribosomal dysfunction,changes in insulin signal efficacy and blood pressure regulation,providing new ideas for targeted therapy.
9.Development and Validation of a Predictive Model for Heart Failure in the People with Hypertension

Objective To establish and validate a risk model for predicting heart failure(HF)in patients with hypertension.Methods The clinical data of 1500 patients with hypertension in Department of Cardiology of Tianjin Union Medical Center from January 2019 to December 2021 were retrospectively collected.The dataset was randomly partitioned into a training set(n=1050)and a valida-tion set(n=450)at a ratio of 7∶3.Then,the patients of training set were segmented into heart failure group(n=144)or non-heart failure group(n=906).The univariate Logistic regression,least absolute shrinkage and selection operator(LASSO)regression model and multivariate Logistic regression analysis were used for screening the risk factors and developing model.The R software was used to con-struct a nomogram.The discrimination ability of the model was determined by the area under the receiver operating characteristic curve(AUC),the calibration degree was evaluated by the calibration plot and Hosmer-Lemeshow goodness-of-fit test.Decision curve analy-sis(DCA)was performed to assess the clinical utility.Results This study showed that age,coronary heart disease,uric acid(UA),red blood cell distribution width(RDW),blood urea nitrogen/albumin ratio(BAR)and neutrophil to lymphocyte ratio(NLR)are inde-pendent predictors of the development of HF in patients with hypertension.A predictive model was constructed using the above six predic-tive factors.In the training set,the model had an AUC value of 0.864(95%CI;0.831-0.896),Hosmer-Lemeshow goodness of fit test showedx2=10.29,P=0.25.In the validation set,the AUC value was 0.842(95%CI:0.794-0.891),Hosmer-Lemeshow goodness of fit test showedx2=10.48,P=0.23.The clinical decision analysis also showed that the nomogram model had a better clinical perform-ance and could bring net clinical benefits to the patients.Conclusion This study successfully constructs a predictive model for the risk of HF in adults with hypertension and it can effectively identify patients at high risk of HF and formulate targeted intervention measures,thereby providing a basis for performing early prevention and treatment and improving prognosis.
10.Reconstruction of Lumbar Vertebrae Images from Abdominal CT Examinations Using Deep Learning Image Reconstruction Algorithms

Purpose To evaluate the effectiveness of deep learning image reconstruction(DLIR)algorithms in reconstructing lumbar vertebrae images from abdominal CT scans,aiming to reduce radiation dose and eliminate the need for repeat lumbar CT examinations.Materials and Methods A retrospective collection was conducted from March to May 2024 in the First Affiliated Hospital of Tianjin University of Traditional Chinese Medicine.Thirty-two patients who underwent both abdominal and lumbar CT scans in a supine head-first position were enrolled.The abdominal CT(DLIR group)utilized a tube voltage of 120 kVp and a current of 200 mA with high-intensity DLIR for lumbar reconstruction.The standard lumbar CT(lumbar group)used the same voltage with a tube current of 260 mA and was reconstructed using 60%weighted adaptive statistical iterative reconstruction.Objective assessments was used to measure the CT values,noise(standard deviation,SD value),signal-to-noise ratio and contrast-to-noise ratio(excluding adipose tissue)at the third lumbar vertebral pedicle level and the L2/L3 intervertebral disc level for muscle,adipose tissue,cancellous bone,intervertebral discs,dura mater and cortical bone.Subjective assessments employed a five-point scale to evaluate image contrast,noise and sharpness.Results The volume CT dose index in lumbar group and DLIR group were 15.25 mGy and 11.74 mGy,respectively.There was no statistical difference in CT values between the structures of both groups(all P>0.05).Compared with the lumbar group,the DLIR group showed significant reductions in SD values across the measured tissues by 31.09%,35.66%,13.48%,27.82%,24.93%and 15.09%(t=5.09-7.21,all P<0.05).The signal-to-noise ratio improved by 36.40%,52.31%,16.56%,34.13%,38.39%and 18.81%,and the contrast-to-noise ratio improved by 51.70%,51.32%,36.24%,34.47%and 53.56%(t=-9.58--4.23,all P<0.001).The DLIR group significantly outperformed the lumbar group in image contrast[4.45(4.00,5.00)points vs.4.75(4.00,5.00)points],image noise[4.06(4.00,4.00)points vs.4.39(4.00,5.00)points],and spatial resolution of fine structures[4.00(4.00,4.00)points vs.4.27(4.00,5.00)points](Z=-3.80,-4.38,-3.55,all P<0.001).Conclusion Using high-intensity DLIR for abdominal examinations can achieve high-quality lumbar CT images with a 25%reduction in radiation dose,enabling simultaneous abdominal and lumbar scanning in a single session.
