Objective We sought to investigate the clinical characteristics and echocardiographic changes of cryptogenic perforator infarction（PI） patient with patent foramen ovale（PFO）. Methods From Jan 2015 to Dec 2023， patients diagnosed with cryptogenic stroke were retrospectively included in our study. All patients received transthoracic echocardiography（TTE） and transcranial Doppler（TCD）.PFO was diagnosed by the bubble test of TCD. Comparisons of clinical features and TTE findings were performed firstly in PI patients， between PFO and non-PFO， and then in patients with PFO， between PI and cortical infarction（CI）. Results A total of 251 patients were retrospectively analyzed. PFO was detected in 104 patients，of those，40.4%（42/104） had pure PI（PFO-PI） and 59.6%（62/104） had CI（PFO-CI）. In patients without PFO，30.6%（45/147） had pure PI（non-PFO-PI）.Compared with non-PFO-PI， PFO-PI associated with a higher proportion of vertebrobasilar circulation infarctions（VCI）（47.6% vs 17.8%， P=0.003）， lower mean value of left ventricle end-diastolic diameters（LVEDd）（47.3 mm vs 49.8 mm， P=0.037） and lower peak E-wave velocity（62.6 cm/s vs 70.8 cm/s，P=0.015）.In PFO patients， PFO-PI was detected with a lower mean value of aorta root diameters（32.5 mm，P=0.011） and lower mean value of LVEDd（47.3 mm，P=0.045） than PFO-CI（34.5 mm and 49.4 mm， respectively）. Conclusion Higher proportion of VCI was found in PFO-PI. Lower LVEDd， lower Em and without aortic root dilatation were three echocardiographic features associated with PFO-PI in cryogenic PI patients.

Objective To explore the effect of multi-sensory artificial intelligence feedback gait training on the recovery of walking function in stroke patients based on enriched environment theory. Methods From July,2021 to June,2023,a total of 80 stroke patients in Huashan Hospital Affiliated to Fudan University were randomly divided into control group(n=40)and experimental group(n=40).Both groups received rou-tine rehabilitation in the lying and seated positions,for 40 minutes.The control group received ground walking training,for 20 minutes,while the experimental group received multi-sensory feedback gait training in enriched environment,for 20 minutes.Before and after four weeks intervention,the digital motion monitoring treadmill was used to mearsure step speed,step length,hip and knee swing angle and weight symmetry.They were as-sessed with Berg Balance scale(BBS),Fugl-Meyer Assessment-Lower Extremities(FMA-LE)and Barthel Index(BI). Results After intervention,the hip swing angle,step length of both sides and step speed significantly improved in both groups(|t|＞3.162,P＜0.05),and they were better in the experimental group than in the control group(|t|＞2.568,P＜0.05);the average knee joint swing angle and bilateral weight-bearing symmetry significantly im-proved in the experimental group(|t|＞3.249,P＜0.01);the scores of BBS,FMA-LE and BI improved in both groups(|t|＞3.569,P＜0.01),and they were better in the experimental group than in the control group(|t|＞2.922,P＜0.05). Conclusion Multi-sensory feedback gait training based on enriched environment theory could effectively improve the walking and balance of stroke patients,and increase the ability of independence.

Objective To observe the effect of wrist-hand orthosis combined with modified constraint-induced movement therapy(mCIMT)on upper limb and hand function in patients with stroke. Methods From February,2022 to December,2023,32 patients after stroke in Huashan Hospital,Fudan University were randomly assigned to control group(n=16)and experimental group(n=16).Both groups underwent routine re-habilitation,and wore constraint glove almost four hours a day.The experimenal group wore dynamic wrist-hand orthosis four hours everyday,additionally;five days every week,for three weeks.They were evaluated with Wolf Motor Function Test(WMFT),Action Reach Arm Test(ARAT),the strength of gripping,Amount of Use(AOU)and Quality of Movement(QOM)of Motor Activity Log(MAL),Hamilton Anxiety Scale(HAMA)and Hamil-ton Depression Scale(HAMD)before and after treatment,while root mean square ratio of affected/healthy exten-sor muscle of wrist was measured with surface electromyography Results After treatment,the scores of WMFT,ARAT,MAL-QOM,HAMA and the root mean square ratio of affected/healthy extensor muscle of wrist improved in both groups(|t|＞2.179,P＜0.05),and the improvement of WMFT score and the strength of gripping was greater in the experimental group than in the control group(|t|＞2.343,P＜0.05);the strength of gripping,the scores of MAL-AOU and HAMD improved in the experimental group(|t|＞2.819,P＜0.05). Conclusion mCIMT assisted with dynamic wrist-hand orthosis could improve upper limb and hand function in stroke pa-tients.

Objective To explore the application value and related factors of serum carbonic anhydraseⅢ(CAⅢ)in the clinical diagnosis of mild to moderate Alzheimer disease(AD).Methods A total of 106 elderly patients initially diagnosed with mild to moderate AD at Huashan Hospital,Fudan University from October 2020 to November 2022 were enrolled as the AD group,and 89 healthy elderly people during the same period were enrolled as the control group.The serum biochemical indicators including liver and kidney function,blood lipids,blood glucose,folic acid and homocysteine were detected in both groups.Cognitive function was assessed by Mini-mental State Examination(MMSE).Patient Health Questionnaire-9(PHQ-9)and Generalized Anxiety Disorder-7(GAD-7)were used to assess psychological status.The activities of daily living(ADL)were assessed by modified Barthel Index(BI).Serum CAⅢlevels were measured by enzyme-linked immunosorbent assay(ELISA).Correlation analysis and multivariate linear regression analysis were used to identify factors influencing serum CAⅢlevels,and receiver operating characteristic(ROC)curve analysis was performed to evaluate the diagnostic value of serum CAⅢlevels in elderly patients with mild to moderate AD.Results The MMSE score of the AD group was significantly lower than that of the control group(P＜0.001),and the PHQ-9 and GAD-7 scores were significantly higher than those of the control group(P＜0.001).The serum CAⅢlevel in the AD group was significantly lower than that in the control group(P＜0.000 1).In patients with AD,serum CAⅢlevels in patients with a disease course＞3 years,accompanied by depression or anxiety,moderate AD,and serum creatinine≤111 μmol/L were significantly lower than those in patients with a disease course≤3 years,normal emotions,mild AD,and serum creatinine＞111 μmol/L(P＜0.05).Correlation analysis and multivariate linear regression analysis showed that serum CAⅢlevels were negatively correlated with disease duration,PHQ-9 score,GAD-7 scores and severity degree,positively correlated with serum creatinine level(P＜0.05).The PHQ-9 score,severity degree,and serum creatinine level were independent related factors for serum CAⅢlevel in mild to moderate AD elderly patients(P＜0.05).ROC curve result showed that the area under the curve(AUC)of serum CAⅢin diagnosing mild to moderate AD in elderly patients was 0.946,with sensitivity and specificity of 88.79% and 96.74%,respectively.Conclusions Serum CAⅢlevels in elderly patients with mild to moderate AD are higher than those in healthy individuals.Mild AD,without depressive mood,and elevated serum creatinine levels are related factors for elevated serum CAⅢlevels in elderly AD patients.Serum CAⅢmay serve as a novel biological marker for the diagnosis of mild to moderate AD in the elderly.

Due to the unique characteristics and complexity of administration of orally inhaled drug products,bioequivalence trial for orally inhaled generic drugs presents greater challenges in clinical implementation compared to conventional oral administration.This difficulty is particularly evident inmainly attributed to the variability during the self-administration inhalation of inhaled drugss by subjects.Therefore,effective management of subjects is crucial in clinical trials involving orally inhaled products.This paper,based on the experience in conducting bioequivalence trials for orally inhaled drug products,discusses key factors and measures for successful subject management in clinical trials.The aim is to enhance the clinical implementation capabilities of researchers in the evaluation of generic consistency for inhaled drug products and to ensure the quality of clinical trials.

Sphingosine 1-phosphate receptor 1(S1PR1)is currently a popular research target for the treatment of immune-related diseases,but it also has potential cardiac safety risks.This article summarizes the experience of the completed phase Ⅰ clinical trial of S1PR1 innovative drug and discusses the necessity and implementation points of cardiac safety risk management and control in the phase Ⅰ clinical trial of S1PR1 innovative drug by summarizing the experience of the completed phase Ⅰclinical trial of S1PR1 innovative drug,combined with the guiding principles of risk identification and control strategy for early clinical trial and clinical evaluation of drug QT at home and abroad,for the reference of the industry.

Objective To evaluate the pharmacokinetics characteristics of single-dose of Etripamil nasal spray 70 mg in healthy adult Chinese subjects.Methods This was a single-center,randomized,double-blind,placebo-controlled study.Twelve healthy adult Chinese subjects were randomized to receive single-dose of Etripamil nasal spray 70 mg(n=10)or placebo nasal spray(n=2).Blood and urine samples were collected prior and post dose.Etripamil in plasma and urine were analyzed by liquid chromatography-tandem mass spectrometry.The pharmacokinetic parameters were calculated by WinNonlin non-compartmental model.Results Following the single-dose of Etripamil nasal spray 70 mg in healthy adult Chinese subjects,the peak concentration of Etripamil in plasma was quickly attained,with a Cmax of(66.76±56.61)ng·mL-1 and a median(range)tmax of 4.00(3.00-5.00)min.The plasma concentrations of Etripamil had fallen approximately 65％from peak value at 25 min after dosing,and close to 80％within 50 min.The AUC0-last and AUC0-∞ were(3 104.16±2 654.46)and(4 048.77±2 682.38)ng·min·mL-1,respectively.The urine excretion percentage of Etripamil during 24 h was(0.01±0.01)％.Among the 12 subjects who were treated with Etripamil or placebo,10 subjects reported a total of 29 treatment-emergent adverse events(TEAEs).All of the TEAEs were mild in severity.The most common TEAEs were rhinorrhoea and lacrimation increased.Conclusion Etripamil was quickly absorbed after intranasal administration,followed by rapid distribution and elimination(not primarily excreted by renal);Etripamil 70 mg was safe and well tolerated by the healthy Chinese adult subjects.

Objective To evaluate tolerability,safety and pharmacokinetics of JS026 and JS026-JS016 single dose intravenous infusion in healthy adults.Methods This phase 1,randomized,double-blind,placebo-controlled,dose-escalation study totally included 48 participants:32 healthy subjects were enrolled in JS026 single intravenous infusion groups and 16 healthy subjects were enrolled in JS026-JS016 groups.JS026 was sequentially administered from low dose to high dose(30-1 000 mg),with intravenous infusion of JS026 or placebo in JS026 single-dose groups,and intravenous infusion of JS026-JS016 or placebo in the combination drug groups.Blood was collected according to the time point designed for trial.Serum concentrations of JS026 and JS016 were determined by enzyme linked immunosorbnent assay(ELISA),and pharmacokinetics parameters were calculated by WinNonlin 8.2.The power model method was used to evaluate the linear analysis of dose and drug exposure.Results 47 subjects completed trial and 1 subject lost to follow-up.After a single intravenous injection of JS026 of 30 mg,100 mg,300 mg,600 mg,and 1 000 mg,mean Cmax were(9.47±1.53),(33.20±4.95),(96.10±13.70),(177.00±22.20)and(353.00±56.70)μg·mL-1,respectively;mean AUC0-∞ were(4 225.00±607.00),(1.78 × 104±3 268.00),(5.83 × 104±1 038.00),(1.07 × 105±152.00),(1.66 × 105±327.00)μg·h·mL-1,respectively;mean t1/2 of JS026 were 563-709 h.The Cmax and AUC0-∞ of JS026 were basically similar alone or in combination with JS016.The results of Power model showed that Cmax and AUC0-∞ increased approximately linearly with the increasing dose of JS026.Treatment emergent adverse event was not increasing when dose increased and most of adverse event associated with drugs were abnormal on laboratory tests and haematuria,thus JS026 and JS016 was well tolerated in all groups.Conclusion The single intravenous infusion of JS026 can almost be thought to be a linear relationship between the doses and drug serum exposure.JS016 had no significant effect on serum concentration of JS026 and JS026 was well tolerated and safe in healthy subjects within 30-1 000 mg.

The consensus has been reached on the benefits of surgical drainage. However, catheter-related blockage and retrograde infection remain bottleneck problems in the treatment process. To this end, with Huashan Hospital, Fudan University, as the main inventors, a drainage tube dredging umbrella and anti-retrograde infection kit have been designed and applied for the national utility model patent (patent number: ZL 2023 2 1300036.2). The main body of the kit consists of a catheter dredging umbrella, drainage tube, and drainage bag. Several isolation layers are installed in the drainage bag to form a maze structure and a reflux valve is added, thereby increasing the distance and resistance of liquid reflux, greatly reducing the possibility of liquid reflux entering the drainage tube, so as to reduce the risk of retrograde infection through physical means. When the drainage tube is blocked, the drainage tube and joint tube of the drainage bag can be separated, the unblocking umbrella can be inserted into the blockage through the guide wire, the cannula can be inserted along the guide wire, the guide wire is pulled to release the dredging umbrella in the contraction state, and the dredging umbrella can be pulled back in the expansion state until the blockage is removed from the drainage tube. The operating procedure is standardized and simple. While preventing retrograde infection (anti-retrograde infection kit), the catheter dredging umbrella could effectively address the issue of catheter blockage. It has certain clinical promotion and application value.

AIM:To assess the efficacy of Quyin Koufuye in different types of psoriasis vulgaris and analyzes the relationship between efficacy and vari-ous disease-related factors,as well as its comple-mentary role in biologic therapy.METHODS:This study included a total of 396 patients with psoria-sis.Based on the patient history,participants were categorized into the biologics group(n=98),Quyin Koufuye-assisted biologics group(n=62),and Quyin Koufuye monotherapy group(n=236).Patient histo-ry data were collected,including gender,duration of illness,disease type,initial site of onset,degree of itching,recurrence status and time,smoking habits,joint pain,family history of psoriasis,nail damage,treatment plan,and PASI/BSA scores.A retrospective analysis was conducted to identify factors influencing the efficacy of Quyin Koufuye and to analyze its combined effects with biologics.RESULTS:Combining Quyin Koufuye with biologics significantly boosted the PASI90 response rate to 72.6％(P=0.014).Responders to PASI50 with Quyin Koufuye experienced longer recurrence intervals(＞6 months)than non-responders(50％vs.33.6％,A=0.045).Influencing factors included psoriasis-affect-ed body surface area(OR=0.960,P=0.000),pro-longed smoking history(OR=2.10,P=0.046),and psoriasis type(OR=2.47,P=0.015).CONCLUSION:This study underscores the synergy of Quyin Kou-fuye and biologics in treating psoriasis,particularly for longer recurrence intervals-factors like smoking history,psoriasis type,and affected body surface area impact Quyin Koufuye's efficacy.
