Research of MSC-seeded biomaterials promoting ischemic diabetic ulcers repair

Objective To observe the effect of mesenchymal stem cells ( MSC) in treating ischemic diabetic ulcers, and to explore its clinical perspectives. Methods Prepared electro-spinning biomaterials and cultured MSC to study effect of MSC composite biomaterials in vitro by scanning electron microscope,MTT array and influence of MSC conditioned medium on endothelial cells. The use of 5 to 8 week old male C57BL/6J mice were prepared into diabetic mice,femoral artery ligation in the proximal thigh,in dorsal skin full-thickness wounds caused by the diameter 5 mm. Then the effect of MSC composite biomaterials on ischemic diabetic ulcers was determined. Results This study found that the MSC grow well on electro-spinning biomaterials. Cells foot extension and connections between cells were observed by scanning electron microscope. Stimulated by high glucose,growth and proliferation of MSC has a stronger ability on biomaterials. MSC condi-tioned medium on biomaterials increased human umbilical vein endothelial cells proliferation ability. Conclusion MSC composite biomateri-als can effectively improve the treatment effect of MSC on ischemic diabetic ulcers. The study indicated stem cells composite biomaterials have great potential and application prospect in the treatment of ischemic diabetic ulcers healing.

Identification of peroneal nerve’ s safety zone and its clinical value in treatment of MIPPO for proximal tibia fracture

Objective To define the peroneal nerve’ s safety zone and study its clinical value in treatment of MIPPO for proximal tibia fracture. Methods The peroneal nerve’ s safety zone was defined according to dead body anatomy. Patients with proximal tibia fracture were divided into traditional therapy group ( control group) and peroneal nerve’ s safety zone therapy ( observation group) . The curative effects and complications were compared. Results The peroneal nerve’s safety zone of R1 was (45. 30 ± 1. 55) mm,R2 was (45. 61 ± 1. 40) mm,R3 was(45. 42 ± 1. 62) mm,angle A was(33 ± 2. 1)°,angle B was(97 ± 2. 3)°. The operating time,bleeding volume,hospitalization time and fracture time in observation group were significantly lower than those in the control group,but there was no siginificant difference in post-ther-apy infection,delayed healing and other complications. Conclusion The curative effects according to peroneal nerve’ s safety zone are better than traditional MIPPO in proximal tibia fracture.

Clinical analysis on combination therapy of ERCP and EST for elderly patients with choledocholithiasis in 256 cases

Objective Objective To analysis the clinical curative effect on elderly patients with choledocholithiasis by endoscopic retro-grade cholangiopancreatography ( ERCP) and endoscopic sphincterotomy ( EST) and discuss its clinical significance. Methods 256 patients with choledocholithiasis were collected for diagnosis and treatment by ERCP. The stone characteristics was mastered by ERCP and the pa-tients were treated with EST. Results Patients with choledocholithiasis were successful in 249 cases (97. 27%) for ERCP intubation and 239 cases (93. 36%) for stone remove. After EST,the stones were removed successfully at one time in 199 cases (83. 26%),2 times in 40 cases (16. 74%). 7 patients (2. 73%) with multiple diverticulum and papillary position poor by intubation failure were changed over to op-eration treatment. After ERCP,patients were performed endoscopic nasobiliary drainage (ENBD) 64. 44% (154/239). Pull out the drain-age tube of 90. 91% (140/154) of the patients after stones disappeared which was proved by angiography for 3~5 days. 14 cases (9. 09%) were performed second times to remove the stones due to the presence of residual stones. Postoperative complications occured in 15 cases (6. 02%) including 9 cases of acute pancreatitis and 6 cases of infection of biliary tract, and they were cured after 1 week of corresponding treatment. Transient increase of serum amylase occurred in 39 cases, and all of them recoveried after 3 days without special treatment. Con-clusion The results showed that ERCP ( or EST) were well tolerated by elderly patients with choledocholithiasis,and they were of obvious curative effect and quick recovery,which is worthy of promotion and application.

Role of Rho-kinase and PKC in cyclosporine A regulating vascular reactivity and it relationship to MPTP after traumatic hemorrhagic shock

Objective To investigate the relationship of the beneficial effect of CsA on vascular reactivity to Rho-kinase,protein kinase C ( PKC) and mitochondrial permeability transition pore ( MPTP) in traumatic hemorrhagic shock rats. Methods With traumatic hemor-rhagic shock rats and hypoxia-treated vascular smooth muscle cells ( VSMCs) ,the role of Rho-kinase and PKC in CsA-regulating vascular re-activity following shock and their relationship to MPTP was observed. Meanwhile,the effects of CsA on inflammatory mediators including TNF-α, IL-1β and IL-6 were also studied. Results CsA significantly improved the vascular reactivity of superior mesenteric artery following hemor-rhagic shock. Rho-kinase inhibitor Y27632 significantly antagonized CsA-induced increase of vascular reactivity,while PKC inhibitor stauros-porine had no significant influences on the effects of CsA. Further studies showed that CsA and Rho-kinase agonist U46619 inhibited the o-pening of MPTP in hypoxia-treated VSMCs. In addition,shock induced a significant increase of TNF-αand IL-1β,but CsA did not show a sig-nificant inhibitory effect on their level. Conclusion CsA-induced restoration of vascular reactivity following traumatic hemorrhagic shock via inhibiting MPTP opening,and Rho-kinase participate in this process.

Clinical application of Mammotome biopsy system in breast lesions

Objective To explore the application value of Mammotome biopsy system in diagnosis and treatment of breast lesions. Methods Summed up the experience of 318 breast lesions in 192 patients who were treated with mammotome. Results 316 lesions were re-sected safely and completely. Skin hematoma was found in 17 patients and ecchymosis was found in 8 patients after operation. All patients had no recurrence and residual. Conclusion The Mammotome biopsy system has many advantages for the diagnosis and treatment of breast lesions, including accurate localization, minimal invasion, safety and good cosmetology.

Dynamic MRI for assessment of the anatomical and functional significance of the bladder and urethra in female stress urinary incontinence

Astract:Objective To assess the anatomical and functional significance of the bladder and urethra in female SUI by dynamic MRI and urodynamic. Methods The clinical data were retrospectively analysed on 60 cases of female stress urinary incontinence from Feb 2011 to Nov 2013. All the women received pelvic floor dynamic MRI and Urodynamic study. Measured the urethral angle,the distance of the H line, M line and the descent of the bladder,cervical,anorectal junction from the PCL in the mid-sagittal dynamic MR image of the pelvic. Pearson 's correlation coefficient was used to examine the association among the distance of the H line,M line and the urethral angle,the descent of the bladder,cervical,anorectal junction from the PCL and the Urodynamic data. Results The distance of the H line,M line were significantly and positively associated with urethral angle and the descent of the bladder,cervical,anorectal junction from the PCL. The distance of the H line,M line were significantly and negatively associated with functional profile length, maximum urethral closure pressure,VLPP and detrusor opening pressure. Conclusion Pelvic floor dynamic MRI combined with Urodynamic study is useful for assessing the anatomical and func-tional significance of the bladder and urethra in female SUI.

Clinical research of small doses penehyclidine hydrochloride in painless colonoscopy

Objective To discuss the application value of small doses penehyclidine hydrochloride in painless colonoscopy examination. Methods From July 2012 to January 2013, 90 patients who planed to have painless colonoscopy were randomly divided into 3 groups with each group 30 cases. Before anesthesia, giving small doses of penehyclidine hydrochloride to small-dose group, giving conventional dose of penehyclidine hydrochloride to routine-dose group, and giving atropine intravenous administration to atropine group. Observe the smoothly de-gree, and check the heart rate, mean arterial pressure and postoperative adverse reactions of three groups. Results There was no obvious difference among the three groups in trems of smooth degree and oxyhemoglobin saturation. In small-dose group, heart rate and mean arterial pressure before and after the examination were better than that of routine-dose group and atropine group, and there were obviously lower inci-dence of postoperative adverse reaction compared with the routine-dose group and atropine group. Conclusion Giving small-dose of penehy-clidine hydrochloride before anesthesia of painless colonoscopy examination can significantly improve the effect of examination, and it can re-duce the incidence of postoperative adverse reactions.

Irritation of a new specimen preservation solution on the skin,eye of rabbits and its effect for specimens preser-vation

Objective To conduct the preliminary toxicology tests and evaluate the preservation effect on a new specimen preservation solution in order to provide scientific basis for its application security. Methods Twenty-four New Zealand white rabbits were randomly di-vided into the normal saline group, the formalin preservation group,the mixture group,and the new preservation solution group. Recorded the irritation of different solution on the skin,eye of rabbits,the effect of liver sample preservation in different solution,and the evaporation rate of these solution. And to find a set of preservative solution which is more suitable for preservation of specimens and can reduce the cost. Results The new preservation solution is stimulus on rabbit skin and eye,and it is stronger than the formaldehyde group. Compared with the other three groups,the mixture group has the best preservation effect, and the new preservation solution group has a better preservation effect than the formalin preservation solution group. Conclusion The 1∶1 mixture solution of the new preservation solution and formaldehyde is more suitable preservation solution and it can reduce the costs.

Ulinastain with ghrelin improves small intestine dysfunction in endotoxemia rats

Objective To evaluate the antiinflammatory effect of ulinastain( UTI) with ghrelin( GHL) on amelioration of small intestine dysfunction and its possible mechanisms in endotoxemia rats. Methods Animals were received intraperitoneal injection with lipopolysaccha-ride(LPS,15 mg/kg)as a endotoxemia model. 60 male SD rats were randomly divided into control group(CON group),LPS group,UTI group,GHL group,and UTI+GHL group. Microstructure of small intestinal submucosa was observed with HE staining. Dextran blue-2000 (BD-2000)was drenched for calculation of propulsion rate of the small intestine. The level of tumor necrosis factorα(TNF-α),IL-6 and HMGB1 in serum and small Intestinal mucosal tissue were determined by enzyme linked immunosorbent assay( ELISA) . RealTime-PCR was administrated for detection of rat defensin-5 mRNA(RD-5)and trefoil factor family-3(TFF-3)mRNA. All above measurement were taken re-spectively at 12 hours and 24 hours after LPS injection. Results HE staining shows that UTI+GHL group significantly alleviate the damage of intestinal microtructure caused by LPS when compared with UTI group and GHL group. The UTI+GHL group markedly increased expres-sion of RD-5 and TFF3 mRNA than those of UTI and GHL group in small Intestinal mucosal tissue (P<0. 05). Both the GHL group and the UTI+GHL group significantly enhanced the function of intestine motility,but the propulsion rate of UTI+GHL group was significant higher than that of GHL group(P<0. 05). In LPS group,the level of TNF-α、IL-6 and HMGB1 both in serum and intestinal mucosa tissue were markedly increased(P<0. 05),but those of UTI group,GHL group and UTI+GHL group were significantly decreased when compare to LPS group after the drugs administration at 12 and 24 hours. Conclusion UTI combined with GHL can significantly improve the intestinal func-tion of mucosal barrier and the motor through the inhibition of both systemic and intestinal mucosal inflammatory reaction in the process of en-dotoxemia.

Curative effect observation on anterolateral calf flap with extra-territorial blood supply in repairing large area of foot skin tissue injury

Objective To study the effect of anterolateral calf flap with extra-territorial blood supply in repairing large area of foot skin tissue injury. Methods 40 cases of arge area of foot skin tissue injury were divided into the observation group which were given anterolateral crural flap transplantation and the control group which were given anterolateral thigh flap transplantation. The wound healing condition and walking ability were observed. Results The level of wound healing, Holden walking ability and FAC walking ability were better than control group (P<0. 05). In the observation group, wound healing time was (15. 2 ± 6. 2) d and donor skin crusting time was (9. 4 ± 1. 5) d, which were less than the control group(P<0. 05). Conclusion Anterolateral calf flap with extra-territorial blood supply has positive clinical value as it can improve wound healing and walking function.
