THE HEART STRUCTURE AND FUNCTION CHANGES AND RELATED RISK FACTORS OF TYPE 2 DIABETIC PATIENTS

Objective To investigate the changes of cardiac structure and function in type 2 diabetes mellitus (T2DM), analyze the related risk factors of heart failure, and improve the awareness of the effect of diabetes on the heart. Methods Randomly selected 170 cases of T2DM patients As the object of study, 170 cases of non diabetic patients as con-trol group, according to whether complicated with heart failure were divided into heart failure patients and non heart failure patients.Comparison of each group left ventricular posterior wall thickness, left ventricular end diastolic diameter, left ven-tricular end systolic diameter, left ventricular ejection fraction, E/A ratio Differences, analysis of the related risk factors of heart failure by logistic regressive.Results T2DM group left ventricular posterior wall thickness, left ventricular end diastolic diameter and left ventricular end systolic diameter increased , Left ventricular ejection fraction decreased, E/A ratio de-creased, were statistically significant difference compared with the control group (p <0.05).These differences are mainly come from non heart failure patients.Logistic regression showed that factors in patients with diabetes duration, HbA1c, com-pliance and complications of hypertension, coronary heart disease, is a risk factor for heart failure complicated with T2DM (all p<0.01).These may be the risk factor for diabetic patients with heart failure.Conclusion Diabetes can cause cardiac remodeling, systolic and diastolic dysfunction maybe some T2DM patients.They did not show symptoms of heart failure in the clinical, but their Cardiac structure and function are abnormal.We should pay attention to the cardiac function examination and early evaluation, prevention of risk factors, to avoid the occurrence of heart failure.

THE PREVENTIVE EFFECT OF HEALTH EDUCATION ON RECURRENCE OF VAGINITIS A-MONG GUANGZHOU'S CAREER WOMEN

Objective To investigate the preventive effect of health education on prevention of vaginitis among Guangzhou ’ s career women.Methods 479 career women with confirmed diagnosis of vaginitis in 2013 were cured and followed up for 1 year.Pa-tient were divided into Control Group (n=202) and Observation Group (n=277) according to whether accompanied the health edu-cation on prevention of vagnnitis.The recurrence rate of vaginitis during the 1 -year follow -up was compared between the two groups.Results In Control Group, 192 patients (95.05%) finished the follow -up, and in Observation Group, 260 patients (93.86%) finished the follow-up, there was not statistical difference in accomplishment ratio of follow -up (p>0.05).The recur-rence of rate of vaginitis was statistical higher in Control Group (72/192, 37.50%) than that in Observation Group (64/260, 24.62%) (P<0.05).Conclusion Health education could reduce the recurrence rate of vaginitis significantly.

DISTRIBUTION AND DRUG RESISTANCE OF 689 STRAINS OF ACINETOBACTER BAU-MANNII

Objective To explore the distribution of Acinetobacter baumannii and its drug resistance to 17 antimicro-bial agents to provide basis for clinical treatment.Methods The data of 689 strains of A.baumannii from Jan.2010 to Dec. 2012 were collected and reviewed for bacterial identification and drug sensitive test and then analyzed using WHONET 5.4. Results 689 strains of A.baumannii isolates were broadly distributed in sputum (75.5%), most primarily isolated from the ICU (36.9%).The antimicrobial susceptibility testing showed that the drug resistance rate of Ab was the highest to Bactrim (36.3%), followed by gentamicin (29.0%), and the lowest to cefperazone-sulbactam (8.0%).The drug resistance rate of Ab to cefperazone-sulbactam was the lowest (8.5%) in 2012.In the three years, the drug resistance rate of Ab to imipenem was the biggest rise (14.5%);while to gatifloxacin and levofloxacin declined.The drug resistance rate of Ab to gatifloxacin dropped by 13.1%, which was bigger than that to levofloxacin (9.5%).The drug resistance rates of Ab to the rest antimi-crobial agents fluctuated insignificantly, but were all in an increasing trend.Conclusion Acinetobacter baummannii mainly distribute in sputum of patients in the ICU and department of respiratory diseases, and its detection rate and drug resistance rate are increasing year by year.

THE APPLICATION OF PBL -SEMINAR TEACHING METHOD IN NURSING TEACHING FOR UNDERGRADUATE MAJORING NURSING

Objective To explore the application of “PBL -Seminar” teaching method in nursing teaching for undergraduates majoring in nursing.Methods 54 nursing undergraduates from July 2012 to April 2013 were set as control group receiving the traditional teaching method, and another 52 nursing undergraduates from July 2013 to April 2014 at internship were set as experimental group, receiving “PBL -Seminar” teaching method, and then the two groups were compared in terms of teaching effects.Results The patient satisfaction was obviously improved, and the qualification rate of the nursing students and the comprehensive evaluation score were significantly higher as com-pared with the control group (p <0.5).Conclusion “PBL -Seminar” teaching method can effectively improve the effect of the nursing internship of nursing undergraduates and the teaching quality for the nursing undergraduates.

THE STATUS AND OPTIMIZATION OF INFORMATIONALIZED OUTPATIENT SERVICE FLOWSHEET

The influences of medicine by the rapid development of economy are at least reflected on the following three as-pects: dramatically increasing awareness of health and medical demand of the majority and the serious “five long one short” phenome-non in outpatient clinic.This paper is based on the medical data of Nanfang Hospital.We analyzed the problems in the five links of appointed registration, visiting the doctors, taking examination and treatment, charging medical fees and medicine -claiming, putting forward four suggestions including updating concept for outpatients , emphasizing the application of information equipment and popular-ization of mobile device, such as WeChat guidance, in order to enhance the hospital networking mechanism , realize the synchronous transfer of information, introduce high-technology, and finally perfect the e-record of patients.

CLINICAL VALUE OF CA125, CA153, HE4 JOINT DETECTION IN DIAGNOSIS OF GYNE-COLOGICAL MALIGNANCIES

Objective To investigate the clinical value of CA125, CA153, HE4 joint detection in diagnosis of gynecological malignancies.Methods 70 cases of gynecological malignancies were selected from our hospital in 2013, 72 cases of benign gynecological diseases selected in the same period, and 70 healthy controls were selected. The expression levels of CA125, CA153 and HE4 were detected and statistically analyzed.Results The positive rate of serum CA125 in the gynecological malignancies group was higher than that in the benign gynecological disease group (p <0.05).The positive rate of serum HE4 in the gynecological malignancies group was higher than that in the benign gynecological disease group (p <0.01).The positive rate of CA125, CA153, HE4 joint detection in the gy-necological malignancies group (up to 72.9%) was also significantly higher than that in the benign gynecological dis-ease group (p <0.01).Conclusion is a single tumor marker with the highest sensitivity and specificity for diagno-sis of gynecological malignancies.CA125, CA153, HE4 joint detection can improve the positive rate of diagnosis of gynecological malignancies.

EFFECTS OF NURSING INTERVENTION ON THE REHABILITATION OF PATIENTS WITH BENIGN PAROXYSMAL POSITIONAL VERTIGO

Objective To investigate the effects of nursing intervention on the rehabilitation of patients with benign paroxysmal positional vertigo (BPPV).Methods 64 BPPV patients were randomly divided into control group and observation group, with 32 cases in each group.The control group received otorhinolaryngology routine nursing, while the observation group was managed with continuously systemic nursing interventions, including manipulative re-duction, psychological nursing intervention, health education, head nursing, follow -up and family nursing interven-tion.The Hamilton Anxiety Scale (HAMA) and Hamilton Depression Rating Scale (HDRS) were applied to evaluate the intervention effects.Results Cure rate of the observation group was significantly higher than that of the control group, and recurrence rate of the observation group was obviously lower than that of the control group(p <0.05).The anxiety and depression assessment of the observation group was significantly lower than that of the control group, with statistical difference (p <0.05).Conclusion Systemic nursing intervention can effectively improve the rehabilita-tion of patients with BPPV, and reduce recurrence rate.

CLINICAL EFFECTS OF DEPRESSING EDENTULOUS AREA WITH MICRO -SCREW IM-PLANT ANCHORAGE COMBINED WITH SEGMENTAL ARCH TECHINIQUE ON POSTERI-OR TEETH IN ELONGATED JAW

Objective To study the application of micro -screw implant anchorage combined with segmental arch technique on posterior teeth in elongated jaw to depress edentulous area.Methods 29 adult patients with 31 posterior teeth were depressed with micro -screw implant as skeletal anchorage combined with segmental arch tech-nique in order to help restoration.Results 31 posterior teeth in the experimental group were successfully depressed to the occlusal plane with micro -screw implant anchorage combined with segmental arch technique in order to create restoration space in edentulous area with an average course of treatment of 10 months.29 posterior teeth in the control group were grinded and given root canal therapy to create restoration space in edentulous area.Conclusion The ap-plication of micro -screw implant anchorage combined with segmental arch technique on posterior teeth in elongated jaw to depress edentulous area is an effective straightening method, whereas grinding is the last resort in creating res-toration space in edentulous area.Because patients in primary hospitals have relatively low oral health awareness, some hope for faster and easier solutions and find it difficult to accept traumatic micro -screw implant.
