Effect of CBP on the level of blood lactic acid and 6 h lactic clearance rate in the patients with severe sepsis

Objective To investigate the effect of continuous blood purification ( CBP) on the level of blood lactic acid and 6 h lactic clearance in the patients with severe sepsis. Methods 48 patients with severe sepsis were cho-sen in ICU, and they were randomly divided into two groups: control group ( n=20 ) and CBP group ( n=28 ) . Their arterial blood lactic acid level was tested on admission and was retested 6, 24, 48 and 72 h after treatment. The patients’6 h blood lactic clearance rate and the corresponding APACHEIIscore were calculated. The level of arterial blood lactic acid and the score of APACHE IIwere compared at different time. 6 h blood lactic clearance, the period of stay in ICU and the 28-days mortality were compared too. Results The level of lactic acid and the score of APACHE II after 24 h were lower in CBP group, and the period of stay in ICU was shorter in CBP group than that in control group at different time ( P<0.05 ) . 6 h blood lactic clearance was higher than that in control group (P<0.05). Moreover, the 28-days mortality had no significant difference in CBP group. Conclusion The CBP plays an important role in clearing the blood lactic acid and it can reduce the severity of the sepsis as well as shorten the period of stay in ICU.

Clinical and pathological characteristics of 491 gastric and colorectal polyps

To analyze the polyps in the aspects of distribution, helicobacter pylori ( HP) infection and the use of proton pump inhibitors ( PPI) in different histological types of gastric polyps;to observe the morphosis under endos-copy histopathology and the surveillance situation in colonic polyps. The infection rates of HP in gastric hyperplasic polyps, inflammatory polyps and adenomatous polyps were 31.11%, 45.31% and 58.33%;the using rate of PPI among inflammatory polyps patients was higher. The main histopathological type of colonic polyps was adenomatous polyps;20 cases of colorectal polypectomy surveillance of 2 years had recurrence. The occurrence of gastric adeno-matous polyps may be related to HP infection;PPI had no obvious correlation with the pathological type of the gas-tric polyps. Colorectal adenoma should strengthen the surveillance colonoscopy of postpolypectomy.

Relationship of fragmented QRS complexes with inflammation in patients with acute coronary syndromes

To analyze the correlation between fragmented QRS complexes( fQRS) and inflammation in 136 patients with acute coronary syndromes(ACS). Patients with fQRS had increased hs-CRP levels (P<0.01) in comparison to patients with non-fragmented QRS. The higher hs-CRP levels was,the more the number of leads in the fQRS on ECG was(P<0.01). When we performed multiple logistic regression analysis, fQRS was found to be related to in-creased hs-CRP levels [ OR:1.507 , 95% CI:1.172~1.936 , P<0.01 ] . fQRS was independently related to in-creased hs-CRP. fQRS that may result as an end effect of inflammation at cellular level can represent increased car-diac risk in patients with ACS.

Anti-hyperuricemic activity of bergenin and its mechanism

Objective To explore the anti-hyperuricemia activity of bergenin in the model of hyperuricemic mice induced by potassium oxonate. Methods 60 Kunming male mice were divided randomly into six groups, which were normal control group;hyperuricemic model group;and hyperuricemic groups with 20 , 40 , 60 mg/kg berge-nin, and 5 mg/kg allopurinol. Mice were orally administered once daily with 250 mg/kg potassium oxonate for 7 continuous days to create the model, and then three doses of bergenin and allopurinol were orally initiated on the day 1 h after potassium oxonate was given, separately. Serum uric acid, creatinine and urea nitrogon levels, as well as urinary uric acid and creatinine levels were measured. mRNA and protein expression levels of mouse kidney u-rate transporter 1(URAT1), and glucose transporter 9(GLUT9) were also determined. Results Compared with hyperuricemic model group, bergenin significantly reduced serum uric acid, creatinine and urea nitrogon levels, in-creased 24 h uric acid and creatinine excretion, and fractional excretion of uric acid in hyperuricemic mice;mRNA and protein levels of mURAT1 and mGLUT9 were also markedly down-regulated. Conclusion Anti-hyperuricemia effect of bergenin is attributed to the enhancement of uric acid excretion and reversion of mouse urate transporters o-ver-expression.

Space closure using micro-implant and sliding mechanics:the three-dimensional finite element modeling and numerical analysis

A volunteer was scanned by spiral CT,the Dicom format images were imported into Mimics software to reconstruct 3-D mandibular model,then optimized by Geomagic and meshed in Ansys software. A good geometric precise three-dimensional finite element model was constructed. The displacement of dentition was similar with clin-ical situation by computer.

Study on the relationship between T cell subsets and count and virus load in patients with hepatitis C

Objective To investigate the correlation between T cell subsets and HCV viralload in HCV infection. Methods Flow cytometry was used to detect peripheral T cell subsets count of 69 patients with hepatitis C and 20 cases of normal health human, and fluorescent quantitative PCR was used to detect viral load in patients with hepa-titis C. Results ① The study showed that the percentage ratio of CD4 +T cells (42.87 ±6.11) and of CD4 +/CD8 +(1.34±0.25) in patients patients was significantly lower than those of normal health group,it was 49.55±6.68 and 1.82±0.11 (P<0.01);but the percentage ratio of CD8 +T cells (32.78±5.48) was higher than that of the normal health human ( 27.35 ±4.32 ) ( P<0.01 ) . There were no significant differences between the two groups in the percentage ratio of CD3 + T cells. ② The viral load gradually increased as the percentage ratio of CD8 +T cells increased, while the percentage ratio of CD4 +T cells and the percentage ratio of CD4 +T cells and CD4 +/ CD8 + were decreased gradually. Conclusion It may be one of the important reasons as the chronic infec-tion of hepatitis C virus by change of T cell subsets, and may be an important cause leading to the decrease of T lymphocyte response ability as the expression level of HCV RNA increases.

Value of neutiophil gelatinase-associated lipocalin in early diagnosis of acute kidney injury following adult valvular heart surgery

Objective To explore the value of neutiophil gelatinase-associated lipocalin ( NGAL) taken from blood and urine samples in early diagnosis of acute kidney injury ( AKI) after heart valves replacement surgeries. Meth-ods A total of 56 patients received heart valves replacement surgeries were selected prospectively in this study. NGAL from blood and urine samples and serum creatinine ( Scr) were tested among them at different moments. AKI and non-AKI groups were divided based on Scr levels and the value of NGAL taken from blood and urine sam-ples was estimated in early diagnosis of AKI with receiver operating characteristic curve ( ROC) . Results 16 AKIs were observed among all of them. The peak value of Scr in AKI group was shown between 12 and 24 hours after surgeries, while blood-NGAL was seen high significantly (P<0.05) since 2 hours after surgeries, peak value at 4 hours, for urine-NGAL, peak value was seen at 2 hours. The area of ROC of blood-NGAL 4 hours and urine-NGAL 2 hours after surgeries for AKI diagnosis were 0.891 and 0.934, respectively. The better sensitivity and specificity were shown in both threshold set as 50 μg/L and 110 μg/L. Conclusion Blood-NGAL and urine-NGAL can be used as early diagnostic markers of AKI after heart valves replacement surgeries, whose change is significantly earli-er than that of Scr.

Rapid detection of Pseudomonas aeruginosaby loop-mediated isothermal amplification

A rapid and sensitive method of loop-mediated isothermal amplification(LAMP) was established to detect Pseudomonas aeruginosa(P.aeruginosa).Three pairs of LAMP primers(inner,outer and ring primers) were designed according to the gbca gene of P.aeruginosa.Since adding hydroxy naphthol blue(HNB) to the reaction system, a positive reaction was indicated by a colorchange before and after the reaction,and was verified by agarose gellectrophoresis.Both LAMP and PCR were applied to detect clinical specimens, the sensitivity and specificity of the detection method were evaluated,and were compared with those of conventional PCR.A LAMP method for detecting P.aeruginosa was successfully established.The LAMP method showed specificity for P.aeruginosa without other bacteria amplification.The established LAMP method in this study enables rapid,sensitive and specific detection of P.aeruginosa,and can be applied for grass roots and small scale laboratories as well as field surveillance.

Observation of curative effect of Wenxinkeli combined with buspironeon patients with cardiac neurosis associated with ST-T changes

Objective To observe the effects of Wenxinkeli combination use with buspirone on the treatment of cardiac neurosis with ST-T changes.Methods Ninety patients with cardiac neuropathy were randomly divided into placebo group, Wenxin Granule group and combination use group (Wenxinkeli combination use with buspirone),30 cases in each group.The scores of HAMA-14 and HAMD-24 of the three groups were observed at the end of 4 and 8 weeks after treatment when the score of SF-36 and the improvement of ST-T of combination use group were evaluated followed 12 weeks treatment.Results After 4 weeks of treatment, the two scale scores of combination use group and the Wenxinkeli group, including HAMD-24 and HAMA-14, were significantly lower than the placebo group (P<0.05).In comparison with placebo group, the scores of HAMD-24 and the HAMA-14 were significantly lower after 8 weeks treatment.After 12 months of follow-up treatment, the SF-36 scores of combination use group were significantly higher than the other two groups'(P<0.05).Meanwhile, our results showed that the ST-T changes of the combination use group was significantly improved than the other two groups'(P<0.05).Conclusion The treatment of Wenxinkeli combination use with buspirone can significantly decrease anxiety and improve depression in patients with cardiac neurosis, moreover, long-term combination use treatment can distinctly improve patients' quality of life and relieve their physical symptoms of ST-T changes.

Clinical application of MRD in acute myelogenous leukemia bywt1 quantitative combined with FCM

Objective To discuss the clinical application of minimal residual disease in acute myelogenous leukemia(AML) by wt1 mRNA quantitative combined with multi-parameter flow cytometry (FCM).Methods Real time quantitative polymerase chain reaction (qRT-PCR) method was established for detecting wt1 gene expression level in 35 AML patients.The indexes were detected by different subtypes;And 9 cases of ease and 4 cases of recurrence in patients was followed-up and detected the wt1 level.The multiparameter flow cytometry was used to analyze the minimal residual disease in AML.Results The expression of wt1 gene was significantly higher than that of the control group.Significant difference was found(P<0.05).In the newly diagnosed AMLs, wt1 was the highest in M2 and the lowest in M6.Follow-up of 4 AML patients showed that wt1 gene expression level was markedly decreased after CR, but obviously increased after relapse.The proportion of abnormal myeloid cells in different phases significantly changed by FCM.There was no difference of minimal residual disease in AML by qRT-PCR and multiparameter flow cytometry.The ROC curve was used to analyze the recurrent cases to get the threshold value(3.33%).Conclusion The quantitative analysis of wt1 combined with multi-parameter flow cytometry can be used to monitor minimal residual disease in leukemia patients, assess the treatment efficacy and prognosis, and predict the risk of recurrence.
