Clinicopathologic analysis of lung epithelial malignant tumors with epithelioid granuloma:re-port of 11 cases

Purpose To investigate the clinicopathologic changes of the lung epithelial tumor with epithelioid granuloma. Methods E-leven cases of lung malignant tumor with epithelial epithelioid granuloma reaction were studied according to the histological changes of lung epithelial tumor, immunophenotype, acid-fast staining, and tuberculosis PCR test results combined with follow-up data. Results The mean age was 62. 3 years, and male to female ratio was 10∶ 1. The main clinical manifestations displayed a cough, chest pain, fever and hemoptysis or bloody sputum. Nine cases were malignant tumors, tuberculosis or inflammatory lesions was 1 case each in pre-vious surgery. The tumor with epithelioid granuloma formation of the 11 cases displayed in the same site ( mixed) . The histopathologic changes of the lesions showed a epithelioid or typical tuberculous granulomas formation, including 4 cases of adenocarcinoma, 5 cases of squamous cell carcinoma and 2 cases of neuroendocrine tumors. The acid-fast bacilli staining was positive in 3 cases;6 cases were positive for PCR detection of mycobacterium tuberculosis. 5 patients died within 2 years due to various causes. Conclusions The lung epithelial tumors with epithelioid granuloma is more commonly found in the middle-aged, The signs and symptoms of the patients are similar to primary epithelial malignant tumors of the lung . The biological behavior was progressive with poor prognosis. The diagnosis of the lesions depends on pathological examination. It is very important to pay more attention to the pathological examination, especially in lung puncture samples, and the differential diagnosis for a definate diagnosis and treatment.

Effect of valsartan on Notch pathway and extracellular matrix in glomeruli of diabetic mice

Purpose To investigate effect of valsartan on expression of Notch pathway and production of extracellular matrix. Methods Urine protein and production of extracellular matrix were measured after diabetic mice were treated with valsartan. The levels of Jag-ged1, Notch1, Notch intracellular domain 1 (NICD1), transforming growth factor-β1 (TGF-β1), TypeⅣcollagen and Laminin were determinated by immunohistochemistry, Western blot and real-time PCR. Results Valsartan suppressed urine protein and production of extracellular matrix in diabetic mice (P<0. 05). Valsartan also inhibits overexpression of Jagged1, Notch1, NICD1, TGF-β1, Type Ⅳ collagen and Laminin in glomerular tissues of diabetic mice (P<0. 05 or P<0. 01). Conclusion Valsartan inhibits activa-tion of Notch pathway in glomerular tissues of diabetic mice, inhibits production of extracellular matrix in glomerulus and delays glomer-ulosclerosis.

Comparison of liver cancer models established by different injection routes of rat L2 hepato-cellular carcinoma cells

Purpose To compare and investigate the oncogenic and metastatic phenotypes in nude mice by injection of rat hepatocellu-lar carcinoma ( HCC) cells L2 via two different routes. Methods Twenty of 7-week-old female BALB/cA mice were randomly divided into 2 groups. After the injections of L2 from liver or spleen lobe, the survival rate, tumor formation rate, carcinogenic features, and metastasis were comparatively studied. Results All of the liver orthotopic nude mice developed liver cancer (100%) with 60% lung metastasis rate, and exhibited an expansive growing pattern with surrounding invasion. In the spleen orthotopic model, 78% mice de-veloped HCC in spleen, with 67% liver metastatic rate and 11% lung metastatic rate, lower than the liver orthotopic model ( P <0. 05). But the microscopically hilar lymph node metastasis rate was 33%. Conclusion The direct liver injection of L2 in female nude mice is a better modeling method for studying the mechanism of both carcinogenesis and metastasis, as well as the evaluation of therapeutic effect of liver cancer.

Clinicopathological and prognostic significance of NQO1 protein expression in patients with colorectal carcinoma

Purpose To investigate the prognostic significance of NQO1 protein expression in colorectal carcinoma ( CRC) patients. Methods 192 cases of primary CRC, 28 of colonic dysplasia, and 44 of adjacent non-tumor tissues were selected for immunohisto-chemical staining of NQO1 protein. Correlation between NQO1 overexpression and clinicopathologic characteristics, and the survival rates were calculated by the statistical methods. Results The strongly positive rate of NQO1 protein in CRC was significantly higher than that in gastric dysplasia and adjacent non-tumor tissues (P<0. 01, respectively). NQO1 high-expression rate was positively cor-related with differentiation, serosal invasion, lymph node metastasis, and clinical stage (P <0. 05, respectively). Survival curve showed that the disease-free survival and 5-year survival rates of the patients with high NQO1 expression were obviously shorter than those of patients with low NQO1 expression (P<0. 001, respectively). Further analysis showed that, high expression of NQO1 predic-ted the lower disease-free survival and 5-year survival rates in late-stage patients (P<0. 01, respectively). Importantly, NQO1 was an independent risk factor for the prognosis of CRC using Cox proportional hazards regression model ( HR: 1. 398,95%CI: 1. 011 ~1. 934, P=0. 043). Conclusions Detection of NQO1 protein expression in CRC has an important clinical significance, and it is ex-pected to become a new biomarker for CRC.

Pathological characteristics of loco-regional recurrent nasopharyngeal carcinoma

Purpose To investigate the pathological characteristics of loco-regional recurrent nasopharyngeal carcinoma ( rNPC ) . Methods Nasopharyngeal biopsy specimens of 46 rNPCs and 63 primary NPCs were collected. HE staining, immunohistochemistry and EBV small RNAs ( EBERs) in-situ hybridization were performed. Results The over-expression rates of both p63 and CK5/6 in rNPC were significantly higher than those of primary NPCs (P=0. 005, P=0. 026), while no statistical significance of Ki-67 over-ex-pression existed between the two groups ( P=0. 387 ) . More necrotic tissues, inflammatory exudates, giant bizarre carcinoma cells, desmoplastic stroma, giant bizarre tumor cells and higher degree of squamous differentiation were found in rNPCs. The carcinoma cells of 5 rNPCs were negative for both EBERs in-situ hybridization and LMP-1 immunohistochemical staining. Conclusion The loco-re-gional rNPC has two peaks of latency interval:2~5 and 9~11 years. The loco-regional rNPC cells have higher degree of squamous differentiation with higher expression of p63 and CK5/6, as well as more invasive ability. In addition, both EBERs in-situ hybridization and LMP-1 immunostaining are negative in 10. 87% (5/46) of loco-regional rNPC.

Clear cell sarcoma of gastrointestinal tract:clinicopathologic analyses and review of litera-tures

Purpose To study the clinicopathologic and genetic features of clear cell sarcoma of the gastrointestinal tract. Methods One case of clear cell sarcoma of the gastrointestinal tract was analyzed by histology, immunohistochemistry, fluorescence in situ hy-bridization ( FISH) , with review of relevant literatures. Results A female patient was admitted to the hospital with a history of 1-week abdominal pain. Computed tomography revealed a mass in right hemicolon. A grey-white ulcerated mass was observed in the colon, in-volving the entire thickness of the colon wall extending into the subserosa. Microscopically, medium-size oval or round cells were ar-ranged in sheets and scattered osteoclast-like multinucleated giant cells were also presented. Immunohistochemically, the tumor cells were strongly positive for S-100 protein, but negative for HMB-45, Melan-A, CD117, CD1a and PCK. FISH showed the rearrangement of EWSR1 in 74% of the neoplasm cells. Conclusions Clear cell sarcoma of the gastrointestinal tract is an extraordinarily rare gastro-intestinal neoplasm that shows peculiar histopathological, immunohistochemical, ultrastructural, and genetic features. Whether clear cell sarcoma-like gastrointestinal tumor represents a distinct entity awaits larger series, including cytogenetic or molecular biological in-vestigation.

Expression of Bmi-1, p14 ARF and Mdm2 and their clinical significance in colorectal carcinoma

Purpose To explore the expression of Bmi-1, p14ARF and Mdm2 in colorectal carcinoma and to find out their association with clinicopathological features. Methods The expression of Bmi-1, p14ARF and Mdm2 was detected by immunohistochemistry in 125 cases of colorectal carcinoma and 20 cases of normal colorectal tissues. Results ( 1 ) The positive rates of Bmi-1 and Mdm2 were 56. 8% and 62. 4% in colorectal carcinoma, respectively, which were much higher than that in normal colorectal tissues (20. 0% and 15. 0%). The positive rates of p14ARF was 47. 2% in colorectal carcinoma, which was much lower than that 75. 0% in normal colorec-tal tissues ( P<0. 05 ) . ( 2 ) The expression of Bmi-1 was significantly correlated with serosal invasion, lymph node metastasis and Dukes stage. The expression of p14ARF was significantly correlated with serosal invasion and Dukes stage. The expression of Mdm2 was significantly correlated with histologic grade, lymph node metastasis and Dukes stage (P<0. 05). (3) In colorectal carcinoma, the expression of Bmi-1 and Mdm2 was negatively correlated with p14ARF, respectively (P<0. 05). Conclusions Abnormal expression of Bmi-1, p14ARF and Mdm2 is involved in the occurrence, development and metastasis of colorectal carcinoma, which could be as the reference factors in judgement of biological behavior and provide a new target for clinical treatment.

Expression of CD24 in cardiac adenocarcinoma and its clinical significance

Purpose To investigate the expression of CD24 in cardiac adenocarcinoma, and to explore the relationship between the two molecules and other clinicopathological features. Methods The expression of CD24 in 140 cases of cardiac adenocarcinoma was detec-ted by using immunohistochemistry. And the relationship between CD24 and clinicopathological features, adenocarcinoma biological be-havior were analyzed, total RNAs were extracted from 26 cases of fresh carcinoma tissues and adjacent mucosa, each groups were divid-ed by different clinical staging, different degree of differentiation. The expression of CD24 was detected by using RT-PCR and the re-sults were shown as relative quantity. The difference of CD24 expression between two groups was further analyzed and the invasion of both groups was compared. Results Immunohistochemically, the expression of CD24 was significantly higher in carcinoma tissues than that in adjacent mucosa (P<0. 05), and those expression was associated with tumor differentiation (P<0. 05). CD24 was also closely related with the invasion, lymph node metastasis, clinical stages and nerves involvement (P<0. 05), while no relation with age, gender, vascular blots and residual situation (P>0. 05), the expression of CD24 in two pathological types showed that Lauren diffuse type was higher than intestinal type, the expression of CD24 was different among tubular adenocarcinoma, polypoid adenocarci-noma, mucinous adenocarcinoma and low adhesion adenocarcinoma (WHO classification) (P<0. 05). RT-PCR results demonstrated that the expression of CD24 was markedly higher in carcinoma tissues than those in adjacent mucosa ( P<0. 05 ) . The expression of CD24 in poorly differentiated adenocarcinoma was significantly higher than those in intermediate/highly differentiated adenocarcinoma (P<0. 05). CD24 expression was also associated with clinical stages. The relative quantity from stageⅠtoⅣwas progressively ris-ing (P<0. 05). Conclusions The expression of CD24 is closely related with the tumor differentiation, invasion, lymph node metas-tasis and clinical staging, suggesting that CD24 may be involved in the progression of cardiac adenocarcinoma, and the patients with higher CD24 expression may have poorer prognosis.

Construction and identification of RNAi lentiviral vectors targeting human midkine gene

Purpose To construct a lentiviral ventor-mediated RNA interference of human midkine ( MK) gene and to provide the ba-sis for further experiment in vivo and in vitro. Methods Four RNAi sequences targeting human MK gene were designed, and cloned into the lentiviral vector to construct lentiviral vectors:GV115-MK-1, GV115-MK-2, GV115-MK-3 and GV115-MK-4. After transfec-tion into competent E. Coli bacteria, the candidate clones were identified by PCR and DNA sequencing. The titer of lentiviruses was determined after 293 T cells were co-transfected with GV115-MK, pHelper 1. 0 and pHelper 2. 0. The four kinds of recombinant lenti-viruses were used to infect human breast cancer cell MDA-MB-231, and the expression levels of MK mRNA were detected by real-time PCR. Results PCR analysis and DNA sequencing confirmed that the four inserted MK RNAi sequences were corrected. Strong green fluorescence was observed in the 293 T cells under the fluorescent microscope after co-transfection. The titer of the concentrated virus was 6 × 108 , 5 × 108 , 5 × 108 and 6 × 108 TU/ml, respectively. The MK expression in MDA-MB-231 cells infected with lentiviral vec-tors GV115-MK-1 was decreased by 87. 2% (P<0. 05). Conclusion Lentiviral RNAi vector for MK gene is successfully construc-ted, and it could effectively inhibit the expression of MK gene in MDA-MB-231 cells in vitro.

Expression of Axin, β-catenin and p53 and their significance in pleural fluid of lung adeno-carinoma patients

Purpose To investigate the expression of Axin, β-catenin and p53 in the pleural fluid and to discuss their relationships and significances. Methods The expression of Axin, β-catenin and p53 was detected using immunocytochemistry in adenocarinoma cells of the pleural fluid from 40 patients with primary lung adenocarcinomas or reactive mesothelial cells of the pleural fluid from 40 pa-tients with benign lung diseases. Results Axin positive expression rate was 30% (12/40) in cases with primary lung adenocarcino-mas. The positive expression rate of β-catenin was 15% (6/40) on cell membrane and 60%(24/40) in cytoplasm. The expression rate of p53 was 57. 5% (23/40) in cases of primary lung adenocarcinomas. The expression of Axin was positively correlated with the membranous expression of β-catenin, and negatively correlated with p53 in cancer cells of the pleural fluids. The positive expression rate of Axin was 77. 5% (31/40), but the expression rate of p53 was 1. 25% (5/40). There was no expression ofβ-catenin in reac-tive mesothelial cells from benign lung diseases. Conclusions The expression of Axin is significantly reduced in adenocarcinoma cells. The expression rate ofβ-catenin in cytoplasm is higher than that in membrane of cancer cells. It has a certain value to detect the expression of Axin,β-catenin and p53 for diagnosis and differential diagnosis of lung adenocaicinomas from benign lung diseases.
