Factors Influencing Cancer Rescreening Intention of Korean Elderly.

PURPOSE: This study was conducted to survey the cancer screening status of Korean elderly and to analyze factors influencing cancer rescreening intention. METHODS: Participants were 262 elderly aged 65 or older who visited medical examination center at D University hospital located in D city. Data were collected from June 1 to July 31, 2012 using a structured questionnaire which included health behavior and cancer screening related characteristics, theory of planned behavior variables, EQ-5D, and cancer screening satisfaction. The data were analyzed by SPSS/WIN 18.0 program. RESULTS: Factors influencing elderly's cancer rescreening intention were 'satisfaction about the cancer screening', 'attitude toward cancer rescreening behavior', 'subjective norm toward cancer rescreening behavior', and 'previous experience with cancer screening in the past 2 years'. Cancer rescreening intention was higher in participants with satisfied experience of cancer screening, more positive in attitude and higher in subjective norm toward cancer rescreening. Cancer rescreening intention was lower when participants were not screened for cancer in the past 2 years. CONCLUSION: The study results indicate that customized interventions based on influential factors such as subjective norm, attitude toward cancer screening, and cancer screening satisfaction are necessary in order to promote the consistent cancer screening of Korean elderly.

Effect of Self-monitoring Rehabilitation Program after Stroke on Physical Function, Self-efficacy and Quality of Life.

PURPOSE: This study was conducted to identify the effects of a self-monitoring rehabilitation program based on the Bandura's self-efficacy theory on the activities of daily living (ADL), 6-minute walking distances, self-efficacy and quality of life (QoL) among stroke patients after three to six months. METHODS: The participants consisted of 29 patients in the experiment group and 28 patients in the control group who admitted at rehabilitation specific hospital. Self-monitoring program developed by the researcher lasted twice a week for 8 weeks from August to September, 2013. RESULTS: ANCOVA showed that all of dependent variables of this study, ADL and 6-minute walking distances as a physical function, self-efficacy and QoL for intervention group were higher than those for control group(p<.001). CONCLUSION: The self-monitoring rehabilitation program based on the self-efficacy theory was found to be effective in improving physical function, self-efficacy and QoL for early post-stroke patients. Early rehabilitation program for stroke patients was recommended to consider the self-monitoring of current physical and psychosocial status as a strategy of self-management.

Reliability and Validity of the Clinical Judgment Rubric on Simulation Practice with a Post-operative Rehabilitation Case.

PURPOSE: The purpose of this study was to evaluate the reliability and validity of the clinical judgement rubric on simulation practice with a post-operative rehabilitation case. METHODS: Methodological study design was used to evaluate the reliability and validity of the clinical judgement rubric on simulation practice. The participants were 35 students in a college of nursing. RESULTS: There were showed reliability and validity of the clinical judgement rubric on simulation practice with a post-operative rehabilitation case. In terms of internal consistency, the Cronbach's alpha for all columns was .821. Factor analysis showed that planning, intervention and evaluation domains with Eigen values of 69.87% were bound, unlike the original tool where four factors including noticing, interpretation, responding and reflecting domains were bound. Convergent validity was established by the correlation between the total clinical judgement score and critical thinking disposition before and after simulation practice. Discriminative validity was established by the correlation between the total clinical judgement score and simulation effectiveness score. CONCLUSION: The findings of this study suggested that clinical judgement rubric on simulation practice is reliable. Repeated studies are needed to objectively evaluate the clinical judgement rubric of simulation practice for applying various clinical situations.

Developing a Community Capacity Builded Exercise Maintenance Program for Frail Elderly Women.

PURPOSE: This study was conducted to develop a community capacity builded exercise maintenance program for frail elderly women. METHODS: As a guideline to develop the exercise maintenance program, the intervention mapping framework, including needs assessment, setting program goals, selecting theory-informed intervention methods, producing program components, planning program implementation and evaluation, was used. Focus group interviews with public health nurses and frail elderly women were conducted for needs assessment. Intervention strategies and components were formulated based on community capacity theory. RESULTS: The developed exercise maintenance program consisted of strategies focusing on leadership development, partnership construction, organization development, community systematization of dimension of community capacity. A exercise maintenance program using health leader, health contract, exercise pocketbook, rhythmic activity suiting song and self-help group was included. CONCLUSION: The intervention mapping method was found to be useful to develop theory-based valid and community capacity builded exercise maintenance strategies for frail elderly women.

Effects of an Individualized Cardiac Rehabilitation Education Program on Knowledge about Coronary Artery Disease, Compliance of Sick Role, and Vascular Health Status in Patients of Percutaneous Coronary Intervention.

PURPOSE: This study was conducted to examine the effects of an individualized cardiac rehabilitation education program on knowledge about coronary artery disease (CAD), compliance of sick role and vascular health status in patients with percutaneous coronary intervention (PCI). METHODS: Quasi-experimental design-based nonequivalent control group pretest-posttest design was employed. Using convenience sampling, data were collected from 60 patients who underwent PCI at S Hospital in S City from September 2014 to February 2015. For examining the effects of an individualized cardiac rehabilitation education program, knowledge about CAD, compliance of sick role and vascular health status were measured. RESULTS: The experimental group showed statistically significant differences in knowledge about CAD (t=24.21, p<.001), compliance with sick role (t=20.81, p<.001) and vascular health status (t=15.07, p<.001) compared to the control group. CONCLUSION: The individualized cardiac rehabilitation education program is effective in improving knowledge about CAD, compliance of sick role and vascular health status in patients who underwent PCI. Based on the findings of this study, nursing intervention programs focusing on individualized approach will be useful for patients undergoing PCI.

Comparison of the Effects of Hand Massage Provided with Different Intervals and Periods on Pain and Sleep Disturbance after Orthopedic Surgery.

PURPOSE: The purpose of this study was to compare the effects of hand massage provided with different intervals and periods on pain and sleep disturbance after orthopedic surgery. METHODS: A non-equivalent control group pretest-posttest design was used. The subjects were admitted in an orthopedic hospital to get a surgery. They were divided into three groups. Group I (n=30) had hand massage every day. Group II (n=30) had hand massage every other day. Control group (n=31) had usual care. Data of all three groups were collected on the day before operation, POD (postoperative day) 6 and POD12. Hand massage was given for 2 and half minutes per hand. RESULTS: Pain on POD6 of experimental group II was reduced more than those of control group. Pains on POD12 of both experimental groups were reduced more than those of control group. On POD6, only perceived sleep disturbance (PSD) was significantly different among groups. On POD12, PSD, total sleeping time, and sleep efficiency were more improved in the experimental groups. CONCLUSION: Hand massage was effective on the reduction of pain and sleep disturbance after orthopedic surgery. Applying hand massage on alternate day was effective enough. Also the effects were more obvious after 12 days.

Comparing the Needs of Case Management between Medical Aid Beneficiaries with Simple and Multiple Chronic Diseases.

PURPOSE: The purpose of this study was to compare the needs of case management between Medical aid beneficiaries with simple and multiple chronic diseases (SCD vs MCD). METHODS: The study employed secondary analysis method using a cross-sectional data from 2009 case management service enrollees. Data on 35,862 beneficiaries who have chronic disease(s) were used in the description of chronic disease characteristics, and data on 20,392 beneficiaries, excluding those who have depression and/or disabilities, were used to compare the group differences. RESULTS: Mean age was 68.8+/-11.63 years, and 73.3% were females. MCD group showed an older age, had more women than SCD group. Self-care ability and appropriateness of health care utilization were significantly different between the groups, but there was no difference in health-related quality of life. In subscales, there were significant differences in general health status, depression, symptom management, healthy lifestyle, hygiene and vaccination, and appropriateness of health utilization. CONCLUSION: Different characteristics between patients with simple and multiple chronic diseases indicate that different case management approaches are required for these groups. The study results could be used as a basis for the development of case management model tailored to the characteristics and needs of medical-aid beneficiaries.

Effect of Auricular Acupressure Therapy on Decreasing Smoking rates, Desire for Smoking, and Exhaled Carbon Monoxide in Male College Students.

PURPOSE: This study was conducted to investigate the effect of auricular acupressure therapy on decreasing smoking rates, desire for smoking, and exhaled carbon monoxide in male college students. METHODS: The quasi-experimental, nonequivalent control group pretest-posttest design was used in this study. The participants (n=56) were assigned to the experimental group (n=29) that received auricular acupressure therapy or the control group (n=27) that received placebo therapy. The therapy was applied for 5 days followed by 1 or 2 days of rest. The therapy was repeated 2 times. To identify the effect of the experimental treatments, assessments of smoking rates, desire for smoking and exhaled carbon monoxide were measured. Data were analyzed using chi2 test, Fisher's exact test, t-test and Repeated measure ANOVA with the SPSS version 21.0 program. RESULTS: The experimental group showed significantly lower smoking rates (t=6.60, p<.001), desire for smoking (t=3.37, p=.002) and exhaled carbon monoxide levels (t=6.22, p<.001) after application of the therapy. Significant groupxtime interaction effect was found in exhaled carbon monoxide levels between the experimental group and the control group (F=8.73, p=.001). CONCLUSION: Auricular acupressure therapy was considered to likely be useful as a therapeutic intervention for smoking cessation in male young adults.

Effects of Auricular Acupressure Therapy on the Upper Extremity Function, Perceived Health Status and Activities of Daily Living among Stroke Patients at Home.

PURPOSE: The purpose of this study was to identify the effects of auricular acupressure therapy on the upper extremity function, perceived health status and Activities of Daily Living for rehabilitation among stroke patients at home. METHODS: As a quasi-experimental study, non-equivalent control group pretest-posttest design was used. Data were collected from August 1st to September 31st, 2012. Participants were 28 (Experimental 17, Control 11) stroke patients at home. Using the SPSS/WIN 12.0 program, data were analysed by the chi2-test, Fisher's exact test, Mann-Whitney U test and Wilcoxon signed rank test. RESULTS: Upper extremity function (U=28.00, p=.001) and activities of daily living (U=47.50, p=.029) were significantly higher in the experimental group compared to the control group. However, perceived health status was not significantly different between the two group. CONCLUSION: This study proved that auricular acupressure therapy was effective for upper extremity function and activities of daily living among stroke patients at home.

Developing a Home-based Self-management Support Intervention for Pulmonary Rehabilitation in Patients with Chronic Obstructive Pulmonary Disease.

PURPOSE: The purpose of this study was to develop a home-based self-management support intervention (SMSI) for enhancing pulmonary rehabilitation (PR) in patients with chronic obstructive pulmonary disease (COPD) in Korea. METHODS: This methodological study was conducted by following these 4 steps: constructing the preliminary draft of a home-based SMSI from existing recommendation and a literature review on PR and self-management interventions; testing content validity with 6 experts in COPD; exploring clinical applicability of the intervention by applying it to 4 COPD patients; determining the final intervention. RESULTS: The final intervention included 4 components and contents identified from the literature review as essentials for self-management of COPD patients: education; exercise training and practice including breathing, muscle strengthening and aerobic exercises; cognitive behavioral strategies including informative self-decision making, collaborative goal setting and supportive advice; and action planning for exacerbation. The intervention was designed to be run by a trained nurse and had 8 weekly sessions consisting of three 60-minute face-to-face sessions and five 20-minute phone-call sessions. CONCLUSION: The intervention developed in this study incorporates essential components of self-management, i.e. action plan and cognitive behavioral strategies, and will contribute to enhancing and maintaining effects of PR by increasing self-management in COPD patients.
