Analysis of Functional Independence Status and Home Care Needs among Stroke Patients.

PURPOSE: The purpose of this study was to analyze functional independence and need for home nursing care in stroke patients. METHOD: This was a descriptive study. The subjects comprised 117 stroke patients who were supposed to discharge in less than a week. The instruments used for this study were the functional independence measure (FIM) and the need assessment for home nursing care of stroke patients. The data were analyzed using frequency, mean, t-test, ANOVA, and Pearson correlation coefficient. RESULTS: Pearson correlation analysis revealed that there were negative correlations between subcategories of the FIM and the need for home nursing care in stroke patients. Especially, in case of the subjects who recorded lower scores at self-care they showed higher needs for home nursing care in the domain of physical problem and rehabilitation. CONCLUSION: For the operation of the home nursing care, the protocol for home nursing care is needed to the stroke patients living at home. The FIM instrument is recommended as a useful scale in order to assess the disability for the stroke patients and the need for home nursing care because this one has correlation with the scale of need for home nursing care.

The Effects of Foot Reflexology on ADL and Fatigue in Stroke Patients.

PURPOSE: This study was to examine the effects of foot reflexology on ADL and fatigue in stroke patients. METHOD: The subjects were 31 stroke patients hospitalized in the Oriental Medicine Hospital of D University from June to November, 2002. Foot reflexology was applied to the experimental group twice a week for 6 weeks, 40 minutes each. For the data analysis, chi2-test was conducted to verify the homogeneity of general characteristics and clinical characteristics, and t-test was done to verify the homogeneity of ADL and fatigue. To examine the relative efficacy of the intervention, ANOVA and ANCOVA were conducted. RESULTS: After foot reflexology, the subjects in the experimental group showed significant improvement in ADL. They also had less physical, psychological, and neurosensory fatigue, which are three areas of fatigue. CONCLUSION: The results suggest that foot reflexology is an effective intervention that helps the body work efficiently, eases stress and strain, and enhances the homeostasis of the body through stimulating the reflex zone of internal organs in the body. Therefore, it is necessary to develop foot reflexology as an unique nursing intervention.

Rehabilitation Need on-Hospital with Disabilities.

PURPOSE: The purpose of this study were to investigate the health status, the currency of rehabilitation therapy, and the patient learning needs on hospital with disabilities. METHOD: The subjects consisted of 87 disabled adults on hospital. Data was collected from February until to June 2005, where they asked structured questionnaires. A descriptive survey design was used and the SPSS 12.0 program was used for data analysis, which included t-test, ANOVA and Duncan's multiple comparison test. RESULT: There are a lot of patient through the transfer from the general hospital and the rehabilitation hospital. Their heath status changed good after hospital admission. Patients took exercise therapy the most, which is one of the rehabilitation therapy. But they need to enough physical therapy because patients have limited time for treatment. The education-need-level was high on hospital with disabilities, especially the need of support and care are the highest on the subscale of patient learning need. There are significant patient learning need differences in income and admission location (p< .05). CONCLUSION: Disabled persons on hospital needs to help and learning exercise by nurses. There should be rehabilitation programs for patients who are ready to leave the hospital. After discharging, there needs to be various rehabilitation services, support and care for the community based rehabilitation.

The Review on Characteristics, Pathophysiology and Risk Factors of Irritable Bowel Syndrome.

PURPOSE: Irritable bowel syndrome (IBS) is frequently yet little understood disease. Review was performed to promote understanding on the characteristics, pathophysiology, and risk factors of IBS. CONTENT: IBS is characterized by abdom in women and people with higher educational and social background, but there are some controversies. IBS is diagnosed by the Rome II or Manning criteria after excluding organic gastrointestinal diseases. The pathophysioloy is explained by abnormal control mechanism of central and enteric nervous system. Mucosal immunity, secretions, and neurotransmitter are also associated with the hypersensitivity and motility change of bowel function. Stress is known as a major triggering factor and contributed to symptoms. Other risk factors are genetic elements, childhood experiences, inflammation, anxiety, depression, diet, and sleep disorders.

PURPOSE: The purpose of this study was to analyze research in the Korean Journal of Rehabilitation Nursing and to suggest further perspective for rehabilitation nursing research. METHOD: In this study we reviewed the contents of 134 articles published in this journal from its beginning year 1998 to 2004. RESULTS: The majority of subjects were adult patients. The type of disease were Cerebrovascular Accident, Neuromuscular disease, Cancer patients, Urogenital disease such as Chronic Renal Failure, Spinal cord injury. Domain of research almost was client, type of research were survey (34.3%), quasi experimental study (20.9%), correlation study (19.4%). Nursing intervention of experimental study was exercise (39.0%), education (17.1%). Place of data collection was hospital (58.2%), position of researcher was professor (67.2%), method of data collection was questionnaire (50.7%). CONCLUSION: There has been a lack of written consent, child rehabilitation, measurement tool development, qualitative research. More studies about rehabilitation nursing are needed to extend the role and fields for professional nursing.

A Study of Fatigue and Daily Living Activity in Patients with Chronic Liver Disease.

PURPOSE: The purpose of this study is to understand the level of fatigue and daily living activities and to confirm their relationship in patients with chronic liver disease. METHOD: The sample consisted of 182 patients with chronic liver disease with a mean age of 42.4 years, who visited the D University Hospital and a S clinic from February, 2004 to March in 2004. Data were collected by a self reporting questionnaire on the selected variables such as fatigue and daily life activities. Collected data were analyzed by using SPSS program for descriptive statistics, Pearson's correlation coefficients, t- test, ANOVA. RESULTS: Subjects showed the mild level of fatigue and slightly high level of daily living activities. They also showed a negative relationship between fatigue and daily living activities. CONCLUSION: It is necessary to develop nursing intervention program to reduce the level of fatigue for improving the level of daily living activities in patients with chronic liver disease.

Study on the Factors Affecting the Sleep Disorder of Hemodialysis Patients.

PURPOSE: The purpose of this study is to investigate the degree of sleep disorder and its related factors in hemodialysis patients. METHOD: The subjects of this study consisted of 210 hemodialysis patients. Data were analyzed by descriptive statistics, t-test, ANOVA, pearson's correlation coefficient, or stepwise multiple regression analysis by using SPSS/PC+ program. RESULT: Sleep disorder degree averaged 2.24. Stress of hemodialysis patients averaged 2.52. Depression degree of hemodialysis patients averaged 1.14. There was significant difference in sleep disorder according to age, marriage status, education, family income and help-givers. Sleep disorder was significantly associated with stress and depression. Stress, depression and marriage status explained 31.3% of the variance in the sleep disorder. CONCLUSION: In conclusion, the sleep disorder of hemodialysis patients is the nursing problem. The sleep disorder degree relates with stress, depression. Especially, it is strongly affected by stress among them.

Roles and Functions of the Rehabilitational Nurse Practitioner Expected by Nurses and Doctors in Rehabilitation Hospital.

PURPOSE: This study aims to identify the role and function of the RNP (rehabilitational nurse practitioner) expected by nurses and doctors. METHOD: This study was a survey. The data were collected 188 nurses and 21 doctors who worked for disabled patients in the rehabilitation hospital during months of June, 2004 and August, 2005. RESULTS: 98.4% of nurse and 61.9% of doctors agreed at opening of RNP course. The major role of RNP expected by nurses were educator, counsellor and case manager. The major role of RNP expected by doctors were direct care, self care promoter & exercise and emotional care. There was a significant difference about the need for opening of RNP course and major role and function of RNP between the group of nurses and doctors. CONCLUSION: The results of this study showed that the need for opening of RNP was identified and the major role of RNP was educator, counsellor, case manager and direct care. So there is a need for further research about major role of RNP related to various setting including rehabilitation hospital, nursing home, home care etc.

A Study on the Relationships between Unhealthy Dietary Habit, Optimistic Bias about Gastric Cancer Occurrence and Self-efficacy in Korean Adult Men.

The purpose of this study was to identify the relationships of optimistic bias about gastric cancer, self-efficacy of healthy dietary behavior and unhealthy dietary habit in Korean adult men. The subjects were 394 men aged from 20 to 64 who lived in Seoul, Kyonggi Do, Kwang-Ju, Jeonnam Do. Data was collected by questionnaire surveys using convenient sampling. The instruments used for this study were extracted and modified from Lee's(2003). The collected data was analyzed using descriptive statistics, Pearson correlation coefficient, and stepwise multiple regression with SPSS/PC 10.0 version. Unhealthy dietary habit in adult men indicated a significantly negative correlation to optimistic bias about gastric cancer(r=-.159, p=.002) and self-efficacy of healthy dietary behavior(r=-.470. P=.000). The most significant predictors influencing unhealthy dietary habit in adult men were age and self-efficacy of healthy dietary behavior. The variance explained was about 24%. These results suggested that men of young age and lack of self-efficacy of healthy dietary behavior are likely to have unhealthy dietary behavior. Therefore. considering age and facilitating self-efficacy are needed in nursing education and intervention for dietary habit change.

Adjustment Experience of Liver Transplant Recipient.

The purpose of this phenomenological study was to understand and describe the essence and the structure of lived experience of liver transplant recipient. Five individual interviews were conducted to gather data regarding their subjective experiences. About 130 significant statements were extracted and these were clusterd into 9 themes. End stage liver disease patients chose their last treatment - liver transplantation. After liver transplantation. recipients experience vague anxiety and uneaseness related to much information and life style change that they had to accept. And they feared to lose their health again. But they felt free from medication and restriction of food. And they satisfied with regaining their physical status. Most of the recipients also had financial difficulties and social restrictions. And they experience uncertainty about their future. But they restored their previous social life. And they experience psychological wellbeing and hope. The results of this study might help nurses who work with liver transplant recipient in establishing and implementing an effective nursing intervention by understanding their lived experience.
