The Effects of Perceived Stress, Marital Intimacy and Mental Health on the Climacteric Adaptation in Middle-aged Women in Urban Areas

Hyun Joo PARK

Korean Journal of Rehabilitation Nursing.2019;22(1):38-47. doi:10.7587/kjrehn.2019.38

PURPOSE: This study is to provide basic data for the development of climacteric adaptation programs by identifying relevant factors based on the degree of climacteric adaptation and perceived stress, marital closeness and mental health in middle-aged women. METHODS: The data were collected from 191 middle-aged women aged 45 to 60 living in the B metropolitan city during July to September 2018. The data were analyzed using the SPSS/WIN 21.0 Statistical Program by descriptive statistics, independent t-test, one-way ANOVA, Scheffé test, Pearson's correlation coefficient, and multiple linear regression. RESULTS: The relevant variables that affect the adaptation of middle-aged women to climacteric were founded as somatization (β=-.29, t=−3.99, p<.001), depression(β=−.24, t=−3.03, p=.003), perceived stress (β=−.19; t=−2.50; p=.013), and spouse's job status (β=.16; t=2.77, p=.006). Overall, approximately 37.8% of the variability in climacteric adaptation was explained by the 4 variables in this study (R2=.378, F=24.12, p<.001). CONCLUSION: Since the middle-aged women in menopause show maladjustment due to negative emotions, it is important for them to control negative emotions and stress. Therefore, it will be necessary to develop a support system through spouse's careful attention and self-help meetings as well as a program of self-awareness of their symptoms for middle-aged climacteric adaptation.

Mi Sook JUNG ; Eun Young OH ; Eun Young CHUNG

Korean Journal of Rehabilitation Nursing.2019;22(1):15-26. doi:10.7587/kjrehn.2019.15

PURPOSE: This study aimed to examine the effects of subjective memory complaints, depression and cognitive function on performance of activities of daily living (ADL) in individuals with mild cognitive impairment (MCI). METHODS: A total sample consisted of 250 MCI patients diagnosed within one year. All participants were assessed with a battery of standardized neuropsychological tests, self-report measures of subjective memory complaints and depression, and performance of basic and instrumental ADL. Correlational and multiple linear regression analyses were conducted to identify the factors associated with performance of basic and instrumental ADL, respectively. RESULTS: Executive function and depression explained 9.4% of the variance in basic ADL. Reduced executive function and greater depressed mood were associated with worse performance on basic ADL. Executive function, subjective memory complaints, and time since onset of cognitive symptoms accounted for 22.2% of the variance in instrumental ADL in individuals with MCI. Lower executive function (t=−2.02, p=.044), greater memory complaints (t=5.36, p<.001), and longer periods of experiencing cognitive symptoms (t=2.24, p=.026) were associated with worse instrumental ADL performance. CONCLUSION: These results may help healthcare professionals develop interventions to improve cognitive outcomes with better understanding of the relationship among cognition, mood, and behavioral performance in individuals with MCI.

Impact of Resistance Training on Obesity index, Lipid Index, Blood Pressure, Pulse Rate, and Bone Mineral Densitometer in Middle-aged Women: A Meta-analysis of Randomized Controlled Trials

Seong Hi PARK ; Chul Gyu KIM

Korean Journal of Rehabilitation Nursing.2019;22(1):1-14. doi:10.7587/kjrehn.2019.1

PURPOSE: We reviewed the effects of resistance training on obesity index, lipid index, blood pressure, pulse rate, and bone mineral densitometer in middle-aged women. METHODS: Randomized, controlled trials lasting 12 weeks investigating the effects of resistance training obesity index, lipid index, blood pressure, and bone densitometer in middle-aged women and published in a peer-reviewed journal up to March 2018 were included. Random- and fixed-effects models were used for analyses, with data reported as weighted means and 95% confidence limits. RESULTS: We included 8 randomized, controlled trials, involving 18 study groups and 335 participants. Overall, resistance training induced a significant low-density lipoprotein reduction [−13.44 (95% CI=−26.71~−0.18)]. No significant effect was observed on other blood lipids, obesity index, blood pressure, pulse rate, and bone mineral density. When study groups were divided according to the period of training, resistance training groups for 24 weeks resulted in a significant decrease in systolic blood pressure [−6.19 (95% CI=−10.95~−1.43 mm Hg)]. CONCLUSION: This meta-analysis supports the potential of resistance training to reduce the level of low-density lipoprotein in middle-aged women. Based on these results, we suggest the application of comprehensive management strategies such as aerobic exercise, resistance exercise, behavior control, and dietary management to prevent obesity in middle-aged women.

Bo Young KIM ; Joo yun LEE

Korean Journal of Rehabilitation Nursing.2019;22(2):142-151. doi:10.7587/kjrehn.2019.142

PURPOSE: Nurses working at rehabilitation hospitals play an important role in interdisciplinary teams. However, conflicts with others within the teams are common. This study (1) explored the frequency and seriousness of role conflicts and determined the relationship between turnover intention and type of role conflict among nurses in rehabilitation hospitals and (2) investigated differences in turnover intention by the type of role conflict. METHODS: Data were collected from 110 nurses in two rehabilitation hospitals. A survey was performed to measure role conflicts including conflicts regarding nursing practice, and conflicts with staff nurses, patients/guardians, staffs in other departments, and doctors. Also, nurses' turnover intentions were evaluated. Data were analyzed using descriptive statistics and cluster analysis. RESULTS: The most common and serious conflicts were developed with patients/guardians, followed by conflicts related to nursing practice. Conflicts in relation to nurses' roles were classified into four categories: ‘High conflict type’, ‘High frequency of conflict type’, ‘High seriousness of conflict type’, and ‘Low conflict type’. The level of turnover intention was increased in this order. CONCLUSION To decrease rehabilitation nurses' intentions to leave a hospital, it is important to reduce role conflicts by providing tailored interventions for each type of conflict and to improve the organizational interrelationship process.

Korean Journal of Rehabilitation Nursing.2019;22(2):124-133. doi:10.7587/kjrehn.2019.124

PURPOSE: This study aimed to examine the levels of compassion fatigue/satisfaction, communication efficacy, and job satisfaction among integrated nursing and care services (INCS) units, and to analyze the factors affecting their job satisfaction. METHODS: This descriptive study was conducted between January 1 and February 15, 2019 with 174 nurses working at INCS units. Data were analyzed using independent t-test, one-way ANOVA, Pearson`s correlation coefficient, and hierarchial multiple regression with SPSS/WIN 23.0. RESULTS: Job satisfaction was significantly different according to age, marital status, monthly income, total clinical career, and job position. Job satisfaction was positively correlated with compassion satisfaction (r=.72, p<.001) and communication efficacy (r=.52, p<.001) and showed a negative correlation with compassion fatigue (r=.-25, p<.001). Factors affecting job satisfaction were compassion satisfaction (β=.61, p<.001) and communication efficacy (β=.18, p=.004). Overall, approximately 58.9% of the variability in job satisfaction was explained by these two variables in this study. CONCLUSION Compassion satisfaction has the effect of maintaining an emotional balance in the nurse's interprofessional relationships. High communication efficacy among professionals in the INCS ward is a necessary factor to increase nurses' job satisfaction. These results suggest that programs to improve compassion satisfaction and communication efficacy are needed to enhance nurses' job satisfaction.
