Relationship between Types of Role Conflict and Turnover Intention in Nurses Working at Rehabilitation Hospitals

PURPOSE: Nurses working at rehabilitation hospitals play an important role in interdisciplinary teams. However, conflicts with others within the teams are common. This study (1) explored the frequency and seriousness of role conflicts and determined the relationship between turnover intention and type of role conflict among nurses in rehabilitation hospitals and (2) investigated differences in turnover intention by the type of role conflict. METHODS: Data were collected from 110 nurses in two rehabilitation hospitals. A survey was performed to measure role conflicts including conflicts regarding nursing practice, and conflicts with staff nurses, patients/guardians, staffs in other departments, and doctors. Also, nurses' turnover intentions were evaluated. Data were analyzed using descriptive statistics and cluster analysis. RESULTS: The most common and serious conflicts were developed with patients/guardians, followed by conflicts related to nursing practice. Conflicts in relation to nurses' roles were classified into four categories: ‘High conflict type’, ‘High frequency of conflict type’, ‘High seriousness of conflict type’, and ‘Low conflict type’. The level of turnover intention was increased in this order. CONCLUSION To decrease rehabilitation nurses' intentions to leave a hospital, it is important to reduce role conflicts by providing tailored interventions for each type of conflict and to improve the organizational interrelationship process.

Effects of Job Embeddedness, Nursing Work Environment, and Nursing Professionalism on Turnover Intention in Nurses Working at Rehabilitation Hospitals

PURPOSE: This study was conducted to identify the effects of job embeddedness, nursing work environment, and nursing professionalism on turnover intention in nurses working at rehabilitation hospitals. METHODS: This descriptive study employed a cross-sectional research design. Questionnaires were collected from May to June, 2019. Participants were 200 nurses from three rehabilitation hospitals in Seoul and Gyunggi province. Data were analyzed using independent t-test, ANOVA, Pearson correlation, stepwise multiple regression. RESULTS: Mean scores for turnover intention, nursing work environment, nursing professionalism. and job embeddedness were 3.21±0.65, 2.90±0.43, 3.06±0.24, and 3.11±0.40, respectively. Turnover intention was negatively correlated with job embeddedness, nursing practice environment, and nursing professionalism. Nursing work environment was the most important predictor in turnover intention, followed by job embeddedness. CONCLUSION The findings in this study indicated that we need to investigate the level of job embededness, nursing work environment, and nursing professionalism in nurses working at rehabilitation hospitals. Hospital managers should develop and apply appropriate policies and strategies to reduce nurses' turnover intention, improve nursing work environment and enhance the level of job embededness.

Factors Influencing Nurses' Job Satisfaction in Integrated Nursing and Care Services Unit: Focused on Compassion Fatigue, Compassion Satisfaction and Communication Efficacy

PURPOSE: This study aimed to examine the levels of compassion fatigue/satisfaction, communication efficacy, and job satisfaction among integrated nursing and care services (INCS) units, and to analyze the factors affecting their job satisfaction. METHODS: This descriptive study was conducted between January 1 and February 15, 2019 with 174 nurses working at INCS units. Data were analyzed using independent t-test, one-way ANOVA, Pearson`s correlation coefficient, and hierarchial multiple regression with SPSS/WIN 23.0. RESULTS: Job satisfaction was significantly different according to age, marital status, monthly income, total clinical career, and job position. Job satisfaction was positively correlated with compassion satisfaction (r=.72, p<.001) and communication efficacy (r=.52, p<.001) and showed a negative correlation with compassion fatigue (r=.-25, p<.001). Factors affecting job satisfaction were compassion satisfaction (β=.61, p<.001) and communication efficacy (β=.18, p=.004). Overall, approximately 58.9% of the variability in job satisfaction was explained by these two variables in this study. CONCLUSION Compassion satisfaction has the effect of maintaining an emotional balance in the nurse's interprofessional relationships. High communication efficacy among professionals in the INCS ward is a necessary factor to increase nurses' job satisfaction. These results suggest that programs to improve compassion satisfaction and communication efficacy are needed to enhance nurses' job satisfaction.

Effects of Delirium Management Program on Nursing Practice and Nurses' Stress in Comprehensive Nursing Care Service Units

PURPOSE: This study was conducted to examine the effects of a delirium management program on nursing practice and nurses' stress in comprehensive nursing care service units. METHODS: A non-equivalent control group pretest-posttest design was used. The participants were 70 nurses in comprehensive nursing care service units. The nurses were assigned to either a experimental group (n=35) or a control group (n=35). The experimental group received a delirium management program. Data were analyzed using χ² test, Fisher's exact test, independent t-test and ANCOVA. RESULTS: The experimental group showed higher nursing practice than the control group (F=7.86, p=.007). The experimental group showed lower nurses' stress than the control group (F=10.73 p=.002). CONCLUSION The findings indicated that a delirium management program was an effective intervention to improve nursing practice and reduce nurses' stress in comprehensive nursing care service units.

Effects of Awareness to Well-dying, Knowledge and Attitudes toward Advance-directives on Attitude toward End-of-life Care in Nursing Students

PURPOSE: This study was to explore the significant factors that influence attitudes toward end-of-life (EOL) care in nursing students. METHODS: A total of 169 nursing students in four universities completed the self-reported questionnaires which included general characteristics, well-dying awareness, knowledge and attitude toward advance-directives, and attitudes toward EOL care. RESULTS: The mean scores were 2.91±0.41 for well-dying perception, 6.61±2.13 for Advance Directives knowledge, 2.95±0.27 for Advance Directives attitudes, and 3.05±0.26 for EOL care attitudes. Factors affecting EOL care attitudes were analyzed by multiple regression. As a result, Advance Directives attitudes (β=.36, p<.001), major satisfaction (β=.30, p<.001), and experience of living with aged (β=.15, p=.014) were identified as factors affecting EOL care attitudes. The explanation power of this regression model was 37.0% and it was statistically significant (F=25.76, p<.001). CONCLUSION Attitudes toward EOL care can be improved by establishing the positive attitudes toward ‘Advance Directives’ and building well-dying perception in nursing students.

Relationships between Osteoporosis Knowledge, Outcome Expectations for Exercise, and Self-efficacy for Exercise in Elderly Women with Osteoporosis Aged over 60

PURPOSE: This study was conducted to identify the relationships between knowledge about osteoporosis, outcome expectations for exercise, and self-efficacy for exercise in older women with osteoporosis. METHODS: This was a cross-sectional descriptive study. Participants were 148 women aged 60 years and older who were diagnosed with osteoporosis. Data were collected using self-report questionnaires. Data were analyzed using descriptive statistics, t-test, ANOVA, Mann-Whitney U test, Kruskal-Wallis test, and Pearson's correlation coefficient. RESULTS: Statistically significant differences in knowledge about osteoporosis were found in age, education level, household income, type of household, chronic disease, medication, alcohol consumption, and perceived health status. Statistically significant differences in outcome expectations for exercise were observed in age, education level, household income, type of household, chronic disease, medication, body mass index, and perceived health status. Statistically significant differences in self-efficacy for exercise were observed in age, household income, chronic disease, and medication. In addition, it was found that knowledge of osteoporosis and self-efficacy for exercise were positively correlated with outcome expectations for exercise. CONCLUSION Based on this study, the development of a physical activity program is necessary by considering knowledge about osteoporosis, outcome expectation for exercise, and self-efficacy for exercise.

Factors Influencing Health Behavior Adherence in Middle and Old Aged Men Who Underwent Percutaneous Coronary Intervention

PURPOSE: The purpose of this study was to identify influencing factors on health behavior adherence in middle and old aged men who underwent percutaneous coronary intervention. METHODS: With a convenience sampling, data were collected from 140 middle and old aged men who underwent percutaneous coronary intervention at two general hospitals. A structured questionnaire was used to measure husband-wife harmony, self-efficacy, and behavior adherence. Data were analyzed by descriptive statistics, Pearson's correlation coefficient, and stepwise multiple regression. RESULTS: Health behavior adherence was significantly correlated with husband-wife harmony (r=.59, p<.001) and self-efficacy (r=.54, p<.001). There was also a significant correlation between husband-wife harmony and self-efficacy (r=.48, p<.001). Overall, approximately 43.7% of the variability in health behavior adherence was explained by these two variables in this study (F=54.85, p<.001). CONCLUSION The findings suggest that husband-wife harmony and self-efficacy could be important factors affecting health behavior adherence in middle aged men who underwent percutaneous coronary intervention. Therefore, nursing interventions for them should focus on increasing husband-wife harmony and self-efficacy.

Relationships between Symptom of Restless Legs Syndrome, Depression, Somatic Symptom and Activities of Daily Living among the Elderly

PURPOSE: The purpose of this study was to investigate relationships between the symptom of restless legs syndrome (RLS), somatic symptom, depression, and activities of daily living (ADL) among the elderly. METHODS: A descriptive approach was taken in this study. Data were collected from May 23 to July 5, 2019 and 140 elderly people replied to the questionnaire. The data were analyzed by descriptive statistics, t-test, One-way ANOVA, Pearson's correlation coefficient, and χ² test. RESULTS: 48.6% of the respondents had RLS symptom and the severity of RLS symptom was 9.57±11.55. RLS symptom had a positive correlation with depression (r=.538, p<.001), with somatic symptom (r=.528, p<.001), with ADL (r=.308, p<.001), and with IADL (r=.360, p<.001). Depression had the most significant correlation with RLS symptoms. There were significant differences in depression, somatization, ADL, and IADL according to the RLS severity. CONCLUSION The results of this study showed that many elders experienced mild to severe RLS symptom. Also, the elderly with RLS symptom had more severe depression, somatic symptom, ADL, and IADL than those without RLS symptom in this study. Depression, somatic symptom, and ADL could be deteriorated if the RLS symptom is not diagnosed early.

Development and Application of an Infection Control Education Program in Long-Term Care Hospitals

Purpose: The purpose of this study was to develop an infection control education program (ICEP) for infection control practitioners (ICPs) in long-term care hospitals (LTCH) and to test the effectiveness of the program. Methods: The ICEP was developed based on the Analysis, Design, Development, Implementation, and Evaluation (ADDIE) model. Focus group interviews, Borich needs assessment, and location of focus models were used to assess educational needs. To test the effectiveness of the developed ICEP program, one-group pretest-posttest design was used. Participants were 269 ICPs working at LTCH. Knowledge, skills, awareness, self-efficacy, and teaching efficacy on infection control were measured before and after the program using questionnaires. A paired t-test was performed to analyze the data. Results: Compared to pre-test, there was a significant improvement in knowledge (47.07±10.22 vs, 76.99±0.88, p<.001) and skills (32.11±12.48 vs. 62.11±14.08, p<.001). Awareness of infection control, self-efficacy, and teaching efficacy were also significantly improved. Conclusion We developed an ICEP for ICPs working at LTCH. Based on the results of this study, the ICEP is effective in improving infection control knowledge, skills, self-efficacy, and teaching efficacy for the ICPs. It is suggestive to provide an ICEP to improve hospital infection control activities.

The Influences of Grit, Emotional Labor and Organizational Intimacy on Nurses' Intention to Stay in Comprehensive Nursing Care Service Units

Purpose: The purpose of this study was to identify the impact of grit, emotional labor and organizational intimacy on nurses' intention to stay to work in comprehensive nursing care service units. Methods: A cross-sectional descriptive design was used. Participants were 192 nurses working in comprehensive nursing care service units at a general hospital. Data were collected using self-report questionnaires, and analyzed using the SPSS/WIN 23.0 program for descriptive statistics, independent t-test, one-way ANOVA, post-hoc Scheffé test, Pearson's correlation, and multiple regression. Results: Intention to stay was positively correlated with grit (r=.28, p=.002), opportunity of forming (r=.17, p=.022), and expansion of intimacy (r=.24, p=.001). The influencing factors on nurses' intention to stay in comprehensive nursing care service units were the work department (β=.23, p=.002) and grit (β=.21, p=.003). Grit and work department explained 13.0% of variances of nurses' intention to stay. Conclusion The results of this study suggest that nurse's grit plays an important role in continuing to work in a comprehensive nursing service unit. It is recommended to develop a program that includes strategies for improving grit.
