Clinical effect of Yi-yuan moxibustion on the bladder dysfunction after incomplete spinal cord injury

Objective:To research the clinical effect of Yi-yuan moxibustion on the bladder dysfunction after incomplete spinal cord injury Method:Forty patients with urinary retention were randomized into treatment group and control group,20 cases in each group.The two groups were treated by rehabilitation training,and the treatment group received Yi-yuan moxibustion,once a day,5d a week,8 weeks in total.Bladder volume and residual urine were tested by B-scan ultrasonography.After 8 week treatment,the curative effect was assessed.Result:One week after all treatments,the results of B-scan ultrasonography showed that bladder volume and residual urine volume of both groups are were improved than before.The curative effect after 6 weeks treatment suggests that the effective rate in control group was 55% and the treatment group was e 90%.The difference was statistically significant (P<0.05).Conclusion:Yi-yuan moxibustion is a kind of good therapy to ameliorate urinary retention caused by spinal cord injury.

The maximal lipid oxidation rates in spinal cord injury patients with different exercise intensities

Objective:To observe the lipid oxidation rates in spinal cord injury (SCI) patients with different exercise intensities.Method:Thirty sedentary patients with SCI were selected to perform incremental exercise load tests (baseline test and Fatmax test) with upper limb power vehicle.Meanwhile,both tests included respiratory gas analysis.In baseline tests,the workload was started at 5W and increased by 10W every 3 min,with 60 rmp until patients exhausted.The Fatmax test included 5 stages of workload and every stage kept 6 min.The initial workload was 5W,the fifth workload was corresponded to the load when respiratory quotient was 1 in the baseline test,the other workload was set according to the average of the initial and fifth workload.All lipid oxidation rates were measured by oxygen uptake (VO2) and respiratory quotient (RQ) in the tests.Result:With the increase of exercise intensity,lipid oxidation rates in SCI patients increased at first and then decreased.The peak of lipid oxidation rates (5.46±0.75 μmol/kg/min) arrived at the intensities of 34.9%±2.3% VO2 peak.After that the lipid oxidation rates decreased significantly as 3.92±0.69,2.85±0.61,0.58±0.19 μmol/kg/min at 41.83±2.18,52.29±1.73 and 61.32±1.98% VO.2 peak accordingly(P<0.05).Conclusion:During the rehabilitation of SCI patients,we should pay attention to the intervention from low intensity exercise to maximize lipid oxidation rates,reducing the incidence of obesity in SCI patients.

A study on shoulder lesion and injury in shoulder pain stroke patients

Objective:Using sonography to clarify the shoulder lesion and injury and to evaluate the relationship between sonographic findings and muscle strength of stroke patients with shoulder pain.Method:One hundred and forty three patients with shoulder pain were divided into two groups according to muscle strength of hemiplegic shoulder.Of which,76 patients were in the muscle strength of hemiplegic shoulder below MMT grade 3 group and 67 patients were in the muscle strength of hemiplegic shoulder equal or above MMT grade 3 group.All the patients received ultrasound examination of their hemiplegic shoulders.Result:The total occurrence rate of shoulder lesion and injury was 74.8%(n=107).The occurrence rate of subacromial-subdeltoid bursal hydrops or bursitis was 41.3%(n=59).The occurrence rate of hydrops or inflammation of long head of biceps tendon sheath was 24.5%(n =35).The occurrence rate of tendinitis of rotator cuff was 22.4%(n=32).The occurrence rate of partial or complete tear of rotator cuff was 20.3%(n=29).There was no statistical correlation between muscle strength of hemiplegic shoulder and severity of shoulder lesion and injury(P>0.05).Conclusion:The occurrence rate of shoulder lesion and injury after stroke was very high.There was no correlation between the severity of shoulder lesion and injury and the muscle strength of hemiplegic shoulder.For the function rehabilitation of upper extremity of stroke hemiplegic patients it is beneficial to find the shoulder lesion and injury earlier and administer effective therapy.

Notes on the Landmark Development of CBR in China

Community-based rehabilitation has a history of 22 years in China.In spite of the preliminary achievement and experiences obtained by the pioneering and trial projects,it was not until the beginning of the new century that CBR in China experienced a landmark development.This iS characterized by a number of favorable pohcy by the government for CBR;the mechanism facilitating primary health care with CBR,a new work-force system of CBR reorganized with better motivation and efficiency,99 demonstration projects of CBR bringing about benchmark promotion effect and the active involvement in CBR by social resources agents.

New Concepts and Evidence Based Practice in Physiotherapy:Examples from Stroke Rehabilitation

Objective:The purpose of this paper is to give an oveuview of new concepts and evidence based physiotherapy practice in stroke rehabilitation in the acute,post acute and chronic stage of stroke.Background:Historically physiotherapy has developed through the years from a focus on health,beauty,and equilibrium between spiritual,moral and physical powers,to a client centered service to people and populations to develop,to maintain and to restore maximum movement and functional ability throughout the lifespan.Still the health perspective is strong and divided into first,second and third prophylaxis.New concepts like Evidence Based Medicine,Evidence Based Practice,International Classification of Function and research within neurophysiology have had a deep impact on physiotherapy services.practice and education.The highest levels of documentation are Meta analyses and Randomised Controlled Trials,and today many of the physiotherapy methods used in rehabilitation of neurological conditions are tried out in different trials,bringing up-todate knowledge into practice.This paper fo-cuse8 on rehabilitation of persons with stroke and physiotherapy methods in particular.Result:Physiotherapy in the acute stage improves motor function and enhances mobility.Rehabilitative efforts within the first few weeks as opposed to later favors better recovery.Type of physiotherapy in the acutestage is task oriented training with a focus on intensity and variability.Post acute therapy-based rehabilitation services targeted towards stroke patients living at home appear to improve independence in personal activities of daily living.In the chronic stage there is good evidence that aerobic exercise is beneficial for improving aerobic capacity inpeople with mild and moderate stroke.Progressive resistance strength training programmes reduce musculoskeletal impairment after stroke,without increasing tone or spasticity.

Skin Impedance at Acupuncture Point Dingchuan in Subjects with and without Asthma

Objective: Asthma can be a disabling disease and despite advances in pharmacology, the prevalence of this condition globally remains high and accounts for a significant proportion of public health care costs. While pharmacology is the mainstay of asthma managemen4 drug side effects have promoted alternative therapeutic interventions, such as acupuncture. Acupuncture points have a lower skin impedance compared to non-acupuncture points. Health impairment is associated with changes in skin impedance at system-specific acupuncture points. Detection of skin impedance changes may assist in the early diagnosis of asthma and monitoring the effectiveness of therapeutic interventions for this condition. Method: This study compared skin impedance at acupuncture point Dingchuan (EX-BI), in 92 subjects with normal health (47 subjects, age 32.6±1.67yr) and those diagnosed with asthma (45 subjects, age 42.4±1.80 yr). Skin impedance was measured using a 2-electrode impedance meter bilaterally at EX-BI, 0.5 "curt" lateral to the lower border of 7th cervical vertebra. Result: The study showed that skin impedance was significantly higher at acupuncture point EX-B1 in subjects with asthma (29.4±21 kΩ) compared to subjects with normal health (13.8±7.9 kΩ) (P=0.013). Skin impedance was negatively correlated to forced expiratory volume in 1 second (FEV1, r=-0.59, P=0.012 in females; and r=-0.68, P=0.015 in males). A receiver operator characteristic (ROC) curve revealed an optimum cut-off point of 35 kΩ for male and 10 kΩ for female subjects. Conclusion: We conclude that EX-B1 skin impedance is higher in patients with asthma and skin impedance might be a possible adjunctive parameter for assisting diagnosis and monitoring asthmatic status.

The Practice of Physical Medicine and Rehabilitation in subSaharan Africa and Antarctica:A White Paper or a Black Mark?

Objective: To explore the practice of PM&R in subSabaran Africa and Antarctica. Method: Medline searches, membership data searches, fax survey of medical schools, Internet searches, and interviews with experts. Result:The continents are dissimilar in terms of climate and govemment. However both Antarctica and subSaharan Africa have no PM&R training programs, no professional organizations, no specialty board requirements, and no practicing physicians in the field. Since there is no known disabled child on Antarctica and adults are airlifted to world-class health care, the consequences of this deficit are minimal there. However the 788,000,000 permanent residents of subSabaran Africa including approximately 78 million persons with disability are left unserved. Conclusion:Antarctica is doing fine. Africa is in a crisis. Local medical schools, hospitals doctors, and persons with disability; along with foreign volunteers, aid groups, and policymakers can impact the crisis. However govemment-specifically national ministries of health-is ultimately responsible for the health and wellbeing of citizens.

Cardiovascular responses between post myocardial infarction patients with different levels of ST segment depression during exercise

Objective: To investigate selected cardiovascular responses of post myocardial infarction patients(PMIP)during exercise within the context of different levels of ST segment depression. Method: Forty-six male recent PMIP performed a graded exercise test on a motorized treadmill during which time blood pressure and heart rate were measured and a 12 lead electrocardiogram (ECG) was monitored continuously. They were then subdivided into those ST segment depression <1.0 mm at exercise stage Ⅱ (group 1) and those who had ST segment depression >1.0 mm at this stage (group 2). Result: The results showed that the patients with a larger degree of ST segment depression at the given work load exhibited higher a value of rate-pressure product (RPP).Conclusion: RPP may be of particular value in estimating the clinical response to exercise when ECG monitoring is not available.

A comparison between multi-directional mechanical traction and longitudinal traction for treatment of lumbar disc herniation:a randomized clinical trial with parallel-group design

Objective: To evaluate the clinical effectiveness of multi-directional mechanical traction (MT) for the treatment of patients with lumbar disc herniation (LDH) compared with longitudinal traction (LT) as control. Method: This prospective, single-blind, randomized clinical trial was performed in Qi Lu Hospital, Shandong University from January 2008 to December 2008. One hundred and twenty outpatients with LDH were randomly divided into MT group or LT group. MT group was treated with computer-controlled multi-directional mechanical traction.LTgroupwastreatedwithlongitudinaltraction.RolandMorrisLowBackPainandDisability Questionnaire (RMDQ), visual analogue scale (VAS), and straight leg raising (SLR) angle were measured for every patient pre-, 4 weeks post-, and 1 year post-treatment.Result: The results of clinical observations showed significant improvements inRMDQ, VAS, and SLR angle assessments (all P<0.05) in both groups 4 weeks post- and 1 year post-treatment compared with pre-treatment. Score of RMDQ in MT group was significantly lower than that in LT group(P<0.05), however, there was no significant difference between two groups in VAS score and SLR angle (P>0.05). The differences in improvement ratios between two groups were not significant (all P>0.05). The clinical outcomes were negatively correlated with patient's age and disease duration.Conclusion: The effect of MT is equivalent and probably superior to that of LT in improving the symptoms and clinical findings of patients with LDH.

A Post-Discharge Service for Brain Injury Patients in Hong Kong

To establish an affordable and easily accessible post-discharge telephone follow-up service for people with brain injuries and their caregivers,and thus to provide an evidence base for developing cost-effective continuous care,region-wide.Method:Thirty-five adult patients with brain injuries and 26 caregivers were recruited from Neurosurgical Unit at QueenMary Hospital in this study.Participants were administered a structured telephone interview at the fourth week post-discharge,and again at the twelfth week post-discharge.Patients and caregivers at risk of poor adjustment were referred for further medical consultation or psychological intervention.Patients' physical and cognitive functioning were investigated while both patients and caregivers' emotional functioning and perceived socialsupport were explored.Satisfaction of the current service provided was also examined.Result:Majority of patients reported significant improvement in physical condition.A sizable number of patientsexperienced cognitive difficulties and reported insufficient psychosocial support received following discharge fromhospital.Majority of caregivers reported compromising health,emotional and social functioning in caregiving.Bothpatients and caregivers reported satisfaction with the service with caregivers reported higher satisfaction.Conclusion:Findings from current study showed that both patients of brain injuries and caregivers benefited from the post-discharge telephone follow-up service.Both of them agreed that such service helped to identify peopleat risk of poor adjustment,provide timely advice and emotional support.Our findings also indicated that caregiving significantly impairs caregivers' physical,emotional,and social functioning,suggesting a need to offer them advice on care skills,role or living adjustments,and stress management.
