Optimize Clinic Management with Clinical Appointment

Objective] To study the impact of outpatient appointment supposed on the management of outpatient department and whole hospital. [Methods] Survey and research the average outpatient registration volume, patients' registration waiting time and the patients' satisfaction monthly during May 2011 to May 2014 when outpatient appointment was applied, and analyze the impact of outpatient appointment supposed on the management of outpatient department and whole hospital by comparing the results 3 years ago(during April 2008 to April 2011) when outpatient appointment was not applied. [Results] The number of patients with an appointment was on the rise, the total number of outpatients increased with the increase of outpatient appointment, and patients' registration waiting time was significantly shortened with higher satisfaction. The difference had statistical significance( P<0.05). [Conclusion] By outpatient appointment services, outpatient management has been optimized, with the increase of outpatients; the hospital benefits grow, and meanwhile the patients' satisfaction is improved.

Children with Functional Abdominal Pain Treated with Shaoyao Gancao Decoction

Objective] To observe the clinical effect of Shaoyao Gancao Decoction on treating children functional abdominal pain to offer reference for further observation. [Method] Choose 200 children patients in pediatric outpatient department of Changxing County TCM Hospital, randomly divide them into treatment and control groups in order, n=100. The treatment one was administered revised Shaoyao Gancao Decoction, the control one orally took Medilac-Vita; both for 4 courses, then observe the cure effects. [Result] In treatment group, total effective rate 94.00%; for control one, it was 86.00%. The comparison of both had difference of statistical meaning( P<0.05). On comparing their abdominal pain relieve, in treatment group, there's more in first 2w and showed marked summit value in the first week; while in control one, there's no marked summit value; so the treatment group was more obvious than other one on relieving pain in the first 2w(P<0.05). The side effect occurrence rate of treatment group was less than other one, the difference between them had statistical meaning(P<0.05). [Conclusion] The revised Saoyao Gancao Decoction has marked cure effect on children functional abdominal pain, worth clinical promotion.

The Relationship between CT Perfusion Imaging and TCM Syndrome of Liver Cancer Patients

Objective] To discuss the relationship between CT perfusion imaging and TCM syndrome of liver cancer patients. [Methods]Through testing the Perfusion parameters of Lesions, surrounding liver tissue and liver tissue perfusion in the distance, we may explore the correlation among perfusion parameters, TCM syndrome of primary liver cancer, then explore the phase rule.[Results] There exists significant difference between perfusion parameters and TCM syndrome of liver cancer patients. Hepatic arterial perfusion(HAP), portal venous perfusion(PVP):deficiency syndrome of both liver and kidney yin>syndrome of heat-damp>syndrome of qi stagnation and blood stasis>syndrome of hepatic stagnation and spleen deficiency;Hepatic perfusion index(HPI):syndrome of hepatic stagnation and spleen deficiency>syndrome of qi stagnation and blood stasis>syndrome of heat-damp>deficiency syndrome of both liver and kidney yin. Child-Pugh classification: syndrome of hepatic stagnation and spleen deficiency(5.34 ±1.46),syndrome of qi stagnation and blood stasis(6.82±0.94),syndrome of heat-damp(8.34±1.12),deficiency syndrome of both liver and kidney yin(9.01±1.19).There exists significant difference between Child-Pugh classification and TCM syndrome of liver cancer patients(P<0.05). The result shows that a high positive correlation between AF, PI and Child-Pugh classification(P<0.05);There exists a negative correlation between PF and Child-Pugh classification(P<0.05).[Conclusion] CT perfusion parameters can be used as an objective indicator of middle-late stage of TCM syndrome of liver cancer patients.

Clinical Observation on the Treatment of Chronic Gouty Arthritis of Feng Shi Yu Zu Zheng by Qu Feng Huo Luo Xie Zhuo Tang

Objective] To observe the clinical efficacy of using Qu Feng Huo Luo Xie Zhuo tang to treat the chronic gouty arthritis of Tan Zhuo Zu Zhi Zheng. [Methods] From June 2013 to December 2014, 116 patients who got chronic gouty arthritis of Tan Zhuo Zu Zhi Zheng were chosen from the orthopedic outpatient cases in our hospital. All patients were equally divided into treatment group and control group by using random number table. The 58 patients of treatment group were treated with Qu Feng Huo Luo Xie Zhuo tang while the other 58 patients of the control group were treated with allopurino for 2 months. We analyzed the value of the Blood Uric Acid(BUA) and clinical efficacy before treatment and 2 months after treatment.[Results] The results showed that value of the BUA of two groups had no significant difference( P>0.05) before treatment, after 2 months, the value of the BUA of treatment group was lower than control group, the value of the BUA of two groups had significant difference( P<0.05).The rate of effective cases in treatment group was 87.9% while the control group was 68.9%, the clinical efficacy of the treatment group was better than the control group.There was statistical difference in the results of treatment between the groups( P<0.05).[Conclusion] Using Qu Feng Huo Luo Xie Zhuo tang to treat the chronic gouty arthritis of Tan Zhuo Zu Zhi Zheng could get good effect,and it was better than allopurinol which was often used in western medicine.It was worth to be researched and observed in clinic.

Clinical Analysis of Yiqidaozhi Decoction on Spinal Fracture Dislocation Postoperative Abdominal Distention Pain

Objective]To investigate the clinical efficacy of Yiqidaozhi decoction on spinal fracture dislocation patients postoperative abdominal distension pain. [Methods]From April 2012 to July 2014 the inpatient treatment of spinal fracture dislocation postoperative abdominal distension pain 172 cases patients,according to the random number table method combined with the admission number ,patients were divided into observation group(n=97) and control group(n=75),patients in observation group were given Yiqidaozhi decoction,while the control group was given conventional western medicine treatment,observed two groups patients after 48h treatment abdominal distension pain ,during the 5d treatment degree of pain and TCM syndrome integral improved etc.[Results]The observation group the total effective rate was 95.88%,significantly higher than 70.67%in the control group,the difference was statistically significant(P<0.05);The patients in the observation group after treatment 1,2,3,4d TCM syndrome integrals were (12.16 ±2.15) points,(7.28± 1.32) points,(5.11 ±1.18) points and (3.53±1.01) points respectively,significantly lower than the control group,the difference was statistically significant (P<0.05);Observation group after treatment 5d TCM symptoms integral compared with control group had no statistical significance( P>0.05);observation group treatment 1,2d TCM symptoms integral decline was obviously higher than that of the control group,the difference was statistically significant(P<0.05);The patients in the observation group treatment 1,2,3d VAS scores were (5.36±1.03) points,(4.56±0.95) points and (1.74±0.87) points respectively lower than that of the control group,the level of the same period,for the difference was statistically significant(P<0.05).[Conclusion]Yiqidaozhi Decoction on spinal fracture dislocation postoperative abdominal distention pain has better therapeutic effect ,and helps relieve pain.

Research on Herbal Medicine Application Regulation of Professor Wu Liangcun in Gastric Cancer Treatment

Objective]To summarize herbal medicine application regulation of Professor Wu Liangcun in gastric cancer treatment. [Methods]Collect 116 gastric cancer patients from Chinese medicine clinic system during January 1,2014 to December 31, 2014 and establish clinical prescription underlying database. Apply descriptive analysis to drug cumulative frequency, drug-induced taste and nature and apply cluster analysis to 42 drugs using more than 10 times. [Results] In the 116 gastric cancer patients,159 kinds of herbs were used ,the cumulative frequency of herbal medicine was 1816.GanCao was the herb with highest usage frequency, and then was FuLing, ShanYao, TaiZishen;According to accumulated frequency, herbs with tonifying function were at the top position(herbs with tonifying qi function were most), and then was the herbs with function of herbs with the function of clearing excessive heat, herbs with function of herbs with the function of relieving dyspepsia, herbs with function of herbs with the function of regulating qi. In terms of herbs' nature and taste, herbs with sweet taste, plain nature were used most. Cluster analysis divided all used herbs into 5 sections, involving prescriptions Sijunzi decoction, Erchen decoction, and couplet medicines DaoYa with GuYa, JiNeijin with MiHoutao, SheShecao with XianHecao and so on. [Conclusions] Professor Wu Liangcun treats gastric cancer with the law of tonifying qi, the main prescription is the same as Sijunzi decoction, emphasizing nourishing Yin and clearing away the heat-evil, protecting gastric, accompanied with promoting digestion and regulating qi.

Ms. Cheng Zhiqing’s Clinical Experience in Treating Hypertension with Insomnia in Menopause

Objective] To sum up Pro. Cheng Zhiqing’s clinical experience in treating hypertension with insomnia in menopause. [Method] Select 3 classical cases of the disease above treated by Pro. Cheng Zhiqing for analysis and discussion, and summarize her differentiation thought and treatment features. [Result] In her view, such disease is mostly concerned with liver, as well as related to heart, kidney, etc., the pathological course concerns mother-organ disorder involving its child-organ, so the therapy shall center on stretching liver, clearing liver, smoothing liver and tonifying liver, also purging child-organ in excess, nourishing mother-organ in deficiency, treating multi-organ in the same time, therefore to get better clinical effect. [Conclusion] For the hypertension with insomnia in menopause, Pro. Cheng Zhiqing is good at grasping disease mechanism, revising famous and recipe decoctions in generations, with close compatibility of medicines, definite cure effect, worth reference.

Qiu Changlin's Experience of Syndrome Differentiation and Treatment on Pregnancy Complicating Myasthenia Gravis

Objective] To conclude the teacher Qiu's clinical experience and academic points on diagnosing and treating pregnancy with myasthenia gravis(MG). [Method] By reviewing relevant literature of traditional Chinese medicine, studying in clinic with teacher and tidying medical records, systematic mining and constructing teacher Qiu's clinical experience and academic points on etiology and pathogenesis, syndrome differentiation, medication thinking, drug characteristics, the taboo of drugs and combining traditional Chinese and western medicines treatment of pregnancy with myasthenia gravis(MG).[Results]Teacher Qiu thinks, pregnancy with MG involving spleen and kidney deficiency is the pathological process of pregnancy and physio-logical process of MG; Clinical syndrome differentiations can be divided into three types like temper deficiency, spleen and kidney deficiency, qi and Yin deficiency; establish the therapeutic rules of nourishing qi to invigorate spleen, tonifying the spleen and kidney and tonifying qi and yin. Respectively adding and subtracting Buzhong Yiqi Decoction with Taishan Panshi San, Buzhong Yiqi Decoction with aconitum lizhong pill and Taishan Panshi San, Buzhong Yiqi Decoction with sengmai san and Taishan Panshi San for syndrome differentiation treating the disease, nourishing qi to invigorate spleen and fetal irritability are the main treatments of the disease. Buzhong Yiqi Decoction and Taishan Panshi San are the basic prescriptions. Large dose of the astragalus membranaceus, atractylodes macrocephala koidz, Codonopsis pilosula,epimedium gradiflorum morr to tonify kidney strengthening spleen and supporting Qi in the prescription, combining traditional Chinese medicine with western medicine can reduce the use of western medicine and reduce the adverse reaction of western medicine. [Conclusion]The teacher Qiu's experience of syndrome differentiation and treatment on pregnancy complicating myasthenia gravis is rich, efficacy is got and has less side effect. It states that Chinese medicine treatment of pregnancy with MG has certain advantages and is worth being popularized and inheriting.

Study on Four Seasons’Influence on the Six Meridians Theory in Shanghan Lun and the Pulse System

Objective]To study the correspondence between human and universe theory in Shanghan Lun starting with Zhongjing ’s doctrine of treatment based on four seasons,analyse the influence of the changing seasons on giving names to the six meridians by Zhongjing ,research on the influence of pulse theory based on four seasons in Huangdi Neijing on the pulse system includes the six meridians system ,summarize the rules of the four seasons’ influence on the six meridians system in Shanghan Lun,and return to Zhongjing’s original viewpoint of four seasons and their yin and yang condition. [Method]By studying the Treatise on Four Seasons treatment Dafa Six Classics and veins in the school system, restore Treatise on Heavencorresponding way of thinking. [Result] It finds that seasons law is the core of six distinguished by its Zhongjing school system clock. [Conclusion] Studying the Four Seasonslaw andseasons therapy Dafa can know Treatise on Six by the corresponding season and six by the significance and reflects theTreatise onlearning system from four seasons pulse method.

Analysis of Related Factors in Compliance of Stroke Patients by Acupuncture Treatment

[Objective] To survey the compliance and related factors of our hospital stroke patients by acupuncture treatment in recent years. [Method] Selection of stroke patients with stable vital signs after rehabilitation treatment from Jan.1,2012 to Dec.31,2013 in our hospital as the objects of study, the demographic characteristics of patients were collected by hospital information system, the injury degree of living ability, depression and the degree of family support for patients were assessed by recovery scale, and the correlation between above factors and compliance were analyzed by comprehensive statistical software.[Result] In 168 patients, only 72 (42.86%) patients had a good compliance, the compliance of women was better than that of men(χ2=7.74, P=0.005), the compliance of better educated patients was better than that of less educated patients(P<0.05), the compliance of mental health patients was better than that of depressive patients(OR=1.731, P=0.031), the compliance of family support condition rated as good patients was better than that of other patients(OR=1.070, P=0.014), and other influence of factors on compliance had not a statistically significance(P>0.05). [Conclusion] The patients with better education, better family economic condition, milder depression and more families support, had the better compliance. In the course of daily acupuncture treatment, the medical staff should pay more attention to the health education to the patients and their families in order to help patients have a good attitude for treatment; the psychological care should be implemented and patients mentality should be adjusted at any time; a good family and social support system for patients should be built by the help of medical staff.
