Clinical Study on Treatment of 85 Cases of Gouty Arthritis with Tong Feng Lotion

One hundred and fifty cases of gouty arthritis were divided into two groups.Treatment group(85 cases) were treated with Tong Feng lotion and control group(65 cases)with external application of Voltaren. Results indicated that Tong Feng lotion could improve clinical symptoms and function of joints, and decrease blood sedimentation,reaction protien C,blood uric acid and blood viscosity; the total effective rate was 89.42% in the treatment group and 75.38% in the control group, with significant difference between the two groups(P<0.05); there was no obvious relation between therapeutic effects and TCM syndrome types.

Effect of Shenqi Fufang(参芪复方)on Pathological Morphology and Serum Inflammatory Factors of Gastrocnemius in Type 2 Diabetic Rats

Objective To explore the effect of Shenqi Fufang (参芪复方,SF) on skeletal muscle damage of type 2 diabetes mellitus (T2DM) and its possible mechanism.Methods Eighteen rats with spontaneous type 2 diabetes mellitus (GK) were randomized into the model group,SF group and sitagliptin group with six rats in each group,and six more Wistar rats were selected as the control group.Each group was given high fat diet for 8 weeks to build T2DM model except for the control group.At the same time,the control group and the model group were given normal saline 5 ml/(kg-d) by gastric perfusion,the SF group was given SF extract 1.44g/(kg · d) by gastric perfusion,and sitagliptin group was given sitagliptin phosphate suspension 16ml/(kg · d) by gastric perfusion.Eight weeks later,the levels of serum insulin-like growth factor 1 (IGF-1),tumor necrosis factor-αr (TNF-a) and interleukin-1β (IL-1β)were measured in each group,the gastrocnemius muscle was taken out to get the wet weight and observe the pathological changes,and the expression of p70s6k1 protein was detected.Results Compared with the control group,the wet weight of the gastrocnemius muscle,the serum IGF-1 level and p70s6k1 protein expression in the model group significantly decreased (P < 0.05),while the levels of serum TNF-α and IL-1β significantly increased (P < 0.05).Through pathological observation,there existed gastrocnemius muscle cells atrophy,a larger area of edema and interstitial lymphocyte infiltration.Compared with the model group,the wet weight of the gastrocnemius muscle,IGF-1 content and p70s6k protein expression in the sitagliptin group and the SF group significantly increased,the contents of TNF-α and IL-l β significantly decreased (P <0.05),and there were no obvious muscle cell atrophy and edema,and also little inflammatory cell infiltration.There was no significant difference between the sitagliptin group and SF group (P > 0.05).Conclusion SF could reduce the diabetic skeletal muscle injury;the potential mechanism seems to reduce the inflammatory factor content,improve IGF-1 resistance and maintain the skeletal muscle mass.

Clinical Observation on Heat Sensitive Moxibustion in Treating 30 Cases of Simple Obesity with Spleen-kidney Deficiency Type

Objective To examine the clinical effect of heat sensitive moxibustion in treating simple obesity with spleen-kidney deficiency type.Methods Ninety simple obesity patients with spleen-kidney deficiency type were randomized into a acupuncture group,a heat sensitive moxibustion group and a combination group,with 30 cases in each group.The heat sensitive moxibustion group was given moxibustion by self-made heat sensitive moxa stick.Heat sensitization acupoints were sought at patients' lumbosacral portion and hypogastrium.Two or three heat sensitization acupoints were selected each time.Patients were treated for 5 times each week,each time for 45min.In the acupuncture group,acupoints of Zhongwan (RN12),Shuifen (RN9),Qihai (RN6),Guanyuan (RN4),Mingmen (DU4),Yaoyangguan (DU3),bilateral Tianshu (ST25),Daheng (SP15),Daimai (GB26),Pishu (BL20),Weishu (21) and Shuidao (ST28) were selected.Seven to ten acupoints were selected each time.Even reinforcingreducing method was used.Needle was retained for 30min.During needle retention,manipulating the needles once every 10min.Patients were treated by 5 times each week.The combination group was given both acupuncture and heat sensitive moxibustion.The methods were the same as above.One month was a course.Each group was treated for 3 courses.Patients' body weight,waistline and height were recorded before and after treatment.Body mass index (BMI),body fat percent and waist to height ratio were calculated before and after treatment.The clinical effect was evaluated.Results After treatment,body weight,BMI,body fat percent and waist to height ratio in each group decreased (P < 0.05).After treatment,body weight,BMI,body fat percent and waist to height ratio in the combination group was lower than those in the acupuncture group and the heat sensitive moxibustion group (P < 0.05);while the difference between the acupuncture group and the heat sensitive moxibustion group was not significant (P >0.05).The total effective rate in the acupuncture group was 73.3%,with 76.7% in the heat sensitive moxibustion group,and 86.7% in the combination group.The clinical effect in the combination group was superior to that in the acupuncture group and in the heat sensitive moxibustion group.Conclusion Heat sensitive moxibustion therapy might be effective in treating simple obesity with spleen-kidney deficiency type.Moreover,the clinical effect seemed better if combined with acupuncture.

Analysis of the Influencing Factors of Post-stroke Depression: Based on Machine Learning

Objective To determine the influencing factors of post-stroke depression by machine learning.Methods Stroke patients' medical records (688 cases eligible) were extracted from record system,including age,gender,pulse manifestation,complexion,tongue quality,fur,Chinese medicine intervention,body mass index (BMI),blood pressure,blood glucose,blood triglyceride,blood total cholesterol,smoking history,drinking history,depression family history,stroke lesion site in imaging,as well as the final depression judgment.Single rule algorithm (1R) was adopted to learn.The risk factors influencing post-stroke patients' depression in extracted information were determined.Then the cases collected were divided into the training dataset (500 cases) and the test dataset (188cases).Optimal discriminant results were obtained by random forest model.Results Single rule algorithm showed that the most important influencing factor of post-stroke depression was stroke lesion site.By computer speculation,stroke lesions in the frontal and temporal lobes were most prone to post-stroke depression.Basal ganglia,brain stem,cerebellum,medulla oblongata and occipital lobe lesions were less likely to cause depression.The accurate classification rate could amount to 88.95% (612/688 cases).Random forest model determination was made in the former 500cases in the training dataset.The total correct rate of determining depression was 98.2%.The total correct rate of determination in 188 cases of the test dataset was 99.47%.Six hundred and eighty-eight patients' data were learnt by random forest model.The total correct rate was 98.84%.The importance measure results showed that top 3 important indexes of post-stroke depression were lesion site,Chinese medicine intervention and depression family history.Conclusion Patients with lesions in the frontal & temporal lobes and depression family history were most prone to post-stroke depression.

Systematic Review and Meta-analysis on Effect of Tai Chi Exercise on Cognitive Function of the Aged

Objective To evaluate the effect of Tai Chi exercise on cognitive function of the aged.Methods Searched China National Knowledge Infrastructure database,Wanfang data knowledge service platform,Chongqing VIP Chinese science and technology periodical database,China biomedical literature database,PubMed,The Cochrane Library and Embase.Collected the clinical research literature in which the elder population practiced Tai Chi and cognitive function outcomes were researched.Conducted Cochrane bias risk assessment.Conducted systematic review and Meta-analysis for the effective information.Results Included 8 randomized controlled trials in total.All studies were described as random,but only two studies described the random method,with one study with blinding.The overall methodological quality was low.Meta-analysis results showed that,the cognitive function outcome evaluation in the aged with cognitive function defect,the overall cognitive function [MD =0.91,95% CI (0.37,1.46),I2=0,P=0.001],language fluency [MD=2.17,95%CI (0.88,3.45),I2=0,P<0.001] and delayed recall test [MD =0.7,95% CI (0.29,1.11),I2 =0,P <0.001] evaluation in the Tai Chi group were significantly better than that in the control group,with no significant improvement in memory span [MD =0.16,95% CI (-0.14,0.45),I2 =55 %,P =0.30].In the outcome indicators evaluation of the healthy elder people,the overall cognitive function evaluation [MD =0.7,95% CI (0.11,1.29),P =0.02] in the Tai Chi group was significantly better than the control group.In language fluency evaluation [MD =0.32,95% CI (0.01,0.64),I2 =0 P =0.05],the difference was not statistically significant,but there is a potential advantage;in evaluation of execution of test line A (TMT-A) [MD =1.53,95% CI (0.1,2.96),I2 =26%,P =0.04],the Tai Chi group was better than the control group;in evaluation of execution of test line B (TMT-B) [MD =1.24,95%CI (-5.20,7.67),I2 =94%,P =0.71] the difference between the Tai Chi group and the control group was not significant.Conclusion Tai Chi could effectively improve cognitive function of the healthy elder people and those with cognitive impairment.

"Discussion on LI Shimao's Academic View of ""Tracing the Origin and Differentiating the Pulse and Syndrome"""

This article summarizes LI Shimao's academic view of tracing the origin and differentiating the pulse and syndrome in six aspects.1) Emphasizing pulse in diagnosis:he put forward the new theories of pulse diagnosis such as differentiating syndrome by pulse diagnosis outline theory,deficiency and excess pulse diagnosis outline theory and interpreting pulse mechanism with Qi and blood outline theory.2) Emphasizing sweat promotion method in the three methods (including sweat promotion,emetic method and purgative method):he promoted sweat with acrid-warm drugs not only in the treatment of excess cold syndrome,but also in the treatment of deficiency cold syndrome.3) Emphasizing Yang deficiency in cold damage syndrome:he regarded Yang deficiency as an important reason for the attack of externally-contracted wind-cold.4) Emphasizing fire constraint in warm disease:he was the first to pinpointed that the nature of warm disease is stagnated heat,he was good at treating warm disease with clearing,venting and enriching methods of which venting is the core and he invented Xinjia Shengjiang San (新加升降散) to treat stagnated heat;5) Emphasizing treating phlegm and stasis in the complicated diseases:he thought the difficult and complicated diseases in modem medicine were similar to the phlegm syndrome and stasis syndrome in Chinese medicine,so he liked modifying Daotan Tang (导痰汤),ZHANG Zhongjing's formulae,WANG Qingren's formulae for removing blood stasis and Taohong Siwu Tang (桃红四物汤) to treat phlegm and stasis syndromes;6) Emphasizing emergencies in rescue period:he thought Chinese medicine did a great work in treating emergencies,and modern scientific and technological methods should be used in studying emergencies in Chinese medicine.

Property and Effect in Chinese Ancient Herbalism

The construction of traditional theory of Chinese herbalism cannot do without the accumulated understanding of herb effect in clinical practice.Meanwhile,it closely relates to the natural history of using different theories of herb property to explain effect and mechanism at different stages.The compilation of Inner Canon of Yellow Emperor 《(黄帝内经》) and Shennong Bencao Jing 《神农本草经》) represent the initial formation of traditional herbalism in Chinese medicine.These two books have a relatively complete description of property and effects of herbs,but with different focus of concerning.After Shennong Bencao Jing,explaining the effect from property of herbs gradually become the developing trend and basic characteristics of herbalism.During the Ming-Qing Dynasties,publication of a large number of herbal annotation works further highlights this feature.

Theory Exploration Nature of Chinese Herb: Based on Analogy Thinking

By consulting ancient medical books on theory of nature of Chinese herb,the author makes confluence analysis on nature of Chinese herb based on analogy thinking guidance and finds that analogy thinking is widely used in theories of Chinese herb nature,including herb naming,four-nature & five-flavor,trend,channel tropism,toxicity,processing,etc.Herb naming based on analogy thinking includes naming by shape-properties,herb nature,flavor,function and growing environment.Four-nature & five-flavor includes determining by shape-properties,growth and substantial-function.Trend includes determining clear-turbidity-ascend-descend from Yin-yang,discussing ascenddescend from thin or thick flavor and determining the trend from shape-quality.Channel tropism includes channel tropism by shape,five-phase channel tropism theory and channel-collateral Zang-fu viscera tropism theory.Toxicity includes determining the toxicity from judging Yin-yang and four-nature,as well as explaining the toxicity from combining five-flavor.Processing includes processing consulting herb nature and flavor,as well as processing according to the nature of accessories.Based on three grades of image,sign and imagery,ancient physicians enrich the cognition of herb nature and form a dynamic Chinese herb nature system advocating the harmony between man and universe.

Evolution of Pathogenesis Concept in Contemporary Chinese Medicine Theory

This article compares and analyzes the content and theoretical structure of the concept of pathogenesis from the national textbooks of Basic Theory of Chinese Medicine and Diagnostics in Chinese Medicine from the first to the ninth version.The authors consider the mechanism as the main content of pathogenesis concept in contemporary Chinese medicine.During the construction of this concept,the concept of pathology has a profound influence,wh ich differs a lot from the emphasis on key in ancient pathogenesis content,mainly reflected in clinical diagnosis and treatment ideas transferring from dealing with key issues to dealing with abstract concept.This change is associated with the impact of western medicine on modern Chinese medicine,emphasizing the mechanism and so on,so that the ancient Xiang (象) concept is marginalized.

Literature Research of Mild Cognitive Impairment Syndrome Distribution Characteristics

Objective To explore the Chinese medicine syndrome characteristics of mild cognitive impairment (MCI).Methods The papers of MCI syndrome research were reviewed and collected in China National Knowledge Infrastructure,Wan Fang Data and VIP Data from January of 1990 to December of 2014.Statistical analysis was made on the Chinese medicine syndrome types and syndrome factors.SPSS 17.0 software was adopted to make cluster analysis.Combined with experts' experience,related symptoms to the syndrome factors were carried out.Results Totally 32 papers were included.Mter terminology normalization,there were 24 syndrome types of MCI.Top 5 syndromes with high frequency were syndrome of orifices confused by phlegm,syndrome of deficiency of kidney essence,syndrome of deficiency of both Qi and blood,syndrome of internal exuberance of heat toxin and syndrome of blood stasis blocking brain.In syndrome factors of disease location type,kidney and brain covered the highest proportion,30.83% and 30.00%.In syndrome factors of disease cause and character types,Qi deficiency covered the most,16.50%.According to results of cluster analysis,combined with experts' experience,15 syndrome factors were extracted,including 69 symptoms.Conclusion Chinese medicine syndrome types of MCI were mainly syndrome of orifices confused by phlegm and syndrome of deficiency of kidney essence.The disease locations were mainly kidney and brain.The disease character was Qi deficiency.
