Application of autologous fat granules graft in improving facial appearance

Objective To explore the methods and effects of improving facial appearance on volume defect and aging,and recovery of aesthetic looks by autologous fat granules graft.Methods We designed the filling areas according to gender,age,individual face characteristics and desire of the case before operation.By liposuction technology with low pressure from the trunk,fats were taken with cleaning and purifying.Then pure fats were injected to the marked areas of the face by multi-level and multi-tunnel injection.It was able to supply the facial volume for lost soft tissue,to improve the aging appearance and to achieve smooth facial contour.Results All cases were followed-up for 3 months to 2 years,facial appearances were satisfied or improved,without fat liquefaction,infection,scleroma and other complications.Only a few cases needed complementary transplantation.The overall results were satisfactory.Conclusions Autologous fat granules graft is a safe and effective facial soft tissue filling technology.It can effectively improve facial aging appearance,restore natural,young and beautiful looks,and be worth for clinical application.

Application of the parietal branches of superficial temporal artery island flap in complex scalp defects

Objective To investigate the application of the parietal branches of superficial temporal artery island flap in the complex scalp defects.Methods A parietal branches of superficial temporal artery island flap on the ectatic scalp flap was designed to repair the complex scalp defects in 25cases and the repairing effect was observed.Results The island flaps were survived completely in 24patients,in which 1 patient had partial necrosis because of the flap tension was too large,but healed after local dressing and debridement.After followed up 6～ 12 months,the color and texture of the flap were the same to the surrounding normal scalp,and the shape was satisfactory.The flap donor site of hair growth was good,with well healing and no obvious complications.Conclusions The parietal branches of superficial temporal artery island flap can repair the complex scalp defects with the flexible flap design and movement.The flap survives well and the repair area is large.The flap and the surrounding scalp connects good.Therefore,it is a good method strongly recommended for small area complex scalp defects repair in clinics.

Correction of traumatic eyeball depression by bone grafting through a conjunctiva approach

Objective To explore the corrective effect of traumatic eyeball depression through bone grafting without extra facial skin incision.Methods Six cases of traumatic fracture of all around eye socket with obvious eyeball depression deforming were treated.Under the local anaesthesia,the corresponding conjunctiva approaching was chosen according to the site of orbital fracture.Bone grafting with medial conjunctiva approach was chosen to the medial eye socket wall fracture,the lateral conjunctiva approach was chosen to the lateral wall fracture,and the inferior conjunctiva approach was chosen to the inferior wall fracture.The size and the shape of bone transplant should be estimated by the CT films previously.Results All the six cases were obviously improved.As the surgery was performed under the local anaesthesia,the movement of eyeball and the vision could be adjusted and be kept watch on timely.The surgery was less trauma and easy recovery,without new skin incision and eyeball functioning affection for all six cases.Conclusions The surgical approach is one of a way of reforming eyeball depressing.

Epicanthus correction by subciliary incision epicanthoplasty

Objective To explore an operative method which can adequately enlarge the inner canthus with inconspicuous scar and perfect shape based on the transverse incision for epicanthus correction,and to evaluate the short-term results and long-term effects comprehensively.Methods The surface projection of tears port medial point was designed as a new inner canthal point,the full thickness skin from new inner canthal point was cut to the free edge of epicanthus,and an outward prolonged incision was made along the lower eyelid edge,separating the subcutanous dissociation or cutting off part of displaced orbicularis,so to relieve the skin tension of inner canthal and reset skin,suturing the new inner canthal point and lower eyelid edge.Results 120 cases were followed up for more than one year,the complication rate at different postoperative time points and patient satisfactory rate one year after operation were compared.12 cases had mild scar formation,from six months to one year after operation,the scar gradually became softened and then inconspicuous.22 cases had a little skin fold at the inner canthal in postoperational one month.2 cases were unsatisfied with the shape of inner canthal,Others were satisfied with therapeutic efficacy,the correction was sufficient with inconspicuous scar and 1ess recrudescence.Conclusions Epicanthus correction by subciliary incision epicanthoplasty is a simple and effective method and suitable for all types of epicanthic fold,and it has the advantages of sufficient enlarged inner canthus,inconspicuous scar and 1ess recrudescence.

Clinical application of modified Abbe flap in repair of moderate defects of the upper lip

Objective To evaluate the application of modified Abbe flap in repairing moderate defects of the upper lip and time point to divide the pedicle.Methods Classic Abbe flap was modified in its design,pedicle cutting and dividing time,which was used to repair moderate defect of the upper lip in 12 cases.Surgery was divided into two phases:one with modified Abbe flap surgery was performed for the combined nasal deformity repair simultaneously,and then the pedicle was divided 9days after surgery.Results 12 patients underwent modified Abbe flap.No vascular complications were found in these flaps.Upper lip shape was well and satisfactory functional recovery,corresponding improvement in nasal appearance.Conclusions The surgery that the modified Abbe flap with the pedicle is divided 9 days after surgery is very simple.On one hand,it greatly improves the patient's appearance and function of the upper lip,improve the overall shape of midface,on the other hand,dividing pedicle time is significantly shorter than in the past,specially reducing the suffering of patients and duration.It is particularly suitable for unilateral and bilateral cleft lip of the upper lip on secondary moderate deformities and combined nasal deformities.

Comparison of effectiveness between micro-plasma technology and 2940 nm Er ∶ YAG laser in treatment of burn scar

Objective To compare the effectiveness,indications and side effect between microplasma technology (MPT) and 2940 nm Er ∶ YAG laser for treatment of burn scar.Methods 265 patients with hypertrophic scar were treated with MPT (MPT group,N=136) and Er ∶ YAG laser (Er ∶ YAG laser group,N=129).The patients were treated 2-6 times.Results The significant response rate was 73.52 ％ and 44.95 ％ in MPT group and Er ∶ YAG laser group; the cure rate was 28.67 ％ and 15.50 ％ in MPT group and Er ∶ YAG group,respectively.There was no statistically significant difference between two groups (P＞0.05).For moderate burn scar patients (93 cases),the significant response rate was 62.79 ％ and 10.00 ％ in MPT group and Er ∶ YAG laser group,respectively.There was significant difference between two groups (P＜0.05).Conclusions MPT and Er ∶ YAG laser has been proven to be effective and safe for the treatment of burn scar.The effectiveness of MPT is superior to Er ∶ YAG laser in treatment of moderate burn scar.

Efficacy of pulsed dual-wavelength 595 and 1064 nm laser in treatment of ulcerated infantile hemangioma

Objective To observe the efficacy of pulsed dual-wavelength 595 and 1064 nm laser in treating ulcerated infantile hemangioma.Methods Lesions from 40 patients were treated with the laser.The distribution of lesion sites was in buttuck and perianal area (13 patients),vulvar lips (7),groin and inside thigh (5),scrotum (4),neck (3),scalp (2),lip (2),trunk and limbs (4).All lesions including ulcer and non-ulcer were conducted with 595 nm plused dye leaser at the first therapy.Later,595 nm pulsed dye leaser was continued or performed with Group 1 or Group 2 parameter of 595 and 1064 nm multiplex mode until the treatment was finished according to the thickness of the lesion.The frequency of treatment was one to six times.The interval of the treatment was four weeks.Results The ulcer from 39 lesions healed gradually in 1 to 2 weeks (average 9.5 days) after the first treatment.These ulcer lesions were healed four weeks after the first time of therapy.The recovery rate of ulcerated lesion was 97.5 ％.For the whole lesion,after 1 to 6 treatments,13 lesions showed excellent result,19 lesions showed good result,and the effective rate was 80 ％.Pain relieved 2 to 7days (average 3.5 days) after the first laser therapy in 35 patients who accompanied with pain.Pain score decreased from 1.875 to 0.125 before and after treatment,with significant difference(P＜0.05).The tolerance of the treatment was good and no side effect was observed.Conclusions 595and 1064 nm dual-wavelength laser is a safe and effective tool for treating ulcerated infantile hemangioma.

Curative effect of Cynergy dual wavelength laser in treatment of skin vascular pathological changes in children

Objective To explore the curative effect and adverse reactions through Cynergy dual wavelength laser to cure skin vascular pathological changes in children.Methods Cynergy dual wavelength (595 nm and 1064 nm) laser were used to treat the skin vascular pathological changes in children (650 cases) including strawberry-shaped hemangioma (398 cases) and port wine stain (252 cases).Anergy density was 5-15 J/cm2 and 30-80 J/cm2.The curative effect and adverse reaction and also the relationship between the port wine stain curative effect and the age of patient,location,and colour of skin lesions were analyzed.Results In total 650 cases after 3-7 times treatments under suitable pulse width and energy density,the curative effect for strawberry-shaped hemangioma was 92.2 ％,that for pink type port wine stain was 77.9 ％,that for purple type port wine stain was 38.9 ％,and that for thickening type port wine stain was 14.3 ％ ; the overall adverse reaction rate was 2.3 ％.After test with x2 analysis,the curative effect of port wine stain also varied depending on the age of patient,location,and colour of skin lesions.The younger the age of patient and shallower of colour of skin lesions,and the better the curative effect.The curative effect for location around the eyes was better than frontal face and limbs.Conclusions Application of Cynergy dual wavelength laser in treatment of the skin vascular pathological changes in children has notable curative effect and low adverse reaction.It is a new curative technology and also safe and reliable.It is worthy of popularization and application.

Clinical evaluation of laser efficacy on familial aggregation hemangioma

Objective To investigate the distribution characteristics and clinical features of familial aggregation hemangioma to improve the level of the diagnosis and treatment.Methods Total 1202 cases of vascular disease were treated in this department from June 2006 to June 2011.The clinical data and family distribution characteristics in 36 cases of familial aggregation hemangioma were analyzed and their effects of laser treatment were evaluated.Results 36 cases were familial aggregation hemangioma,whose incidence was 2.99 ％ in 1202 cases of vascular disease cutis; and the ratio of male to female was 1 ∶ 1.There were 9 cases of parents suffering from hemangioma.But the incidenceof the next generation suffering from vascular disease was 28.5 ％.Among these 36 cases of familial aggregation hemangioma,33 cases underwent the long pulse 1064 nm Nd ∶ YAG laser and optimized pulse light combination therapy.The effective rate was 100 ％.Conclusions There may be a genetic predisposition in hemangioma.The application of long pulse 1064 nm Nd ∶ YAG laser and optimized pulsed light treatment to hemangioma cutis can obtain satisfactory results.The intervention should be taken in early stage.

Detection of decorin expression in keloid with fluorescent quantitative-PCR

Objective To detect the expression and content of decorin in fibroblasts of keloid to deeply reveal the mechenism and the role of decorin plays in scar formation.Methods Fibroblasts of keloid,normal scar and normal skin were cultured in vitro,and the morphology,activity,apoptosis of fibroblast were observed under light microscope and electron microscope; the mRNAs of decorin and TGF-β1 were detected and analyzed with real-time fluorescent quantitative-PCR (FQ-PCR).Results Fibroblasts of keloid showed irregular morphology,larger size and disorder arrangement.There were a large number of mitochondria,swelling rough endoplasmic reticulum,and euchromatin-rich in nucleus of fibroblasts,suggesting the protein synthesis of keloid fibroblast was very active.Compared with normal skin,the expression of decorin was significantly lower in keloid fibroblast; On the contrary,the expression of TGF-β1 was significantly higher in keloid fibroblast than in normal scar and normal skin.Conclusions Compared with normal skin,the expression of decorin in keloid fibroblast is significantly lower.Lower content of decorin in early stage of wound healing may induce weakly suppression of proliferation and synthesis of fibroblast,and up-regulate the activity of TGF-β1,which promotes the proliferation,migration and excessive collagen synthesis of the fibroblast of keloid.Thus,decorinis an suppressor factor of keloid formation.
