Methods and values of suction drainage following secondary total ear reconstruction with an expanded post-auricular marsupial flap

Objective To discuss continuous negative pressure suction and different negative pressure monitoring after the external ear reconstruction operation stage Ⅱ,aiming to improve the security of the plastic surgery.Methods A lot of 87 microtia patients aged 6-15 years were randomly selected as control group from Feb.2010 to Mar.2012,and other 87 cases of microtia were also randomed as experimental group from Apr.2012 to Apr.2014.The experimental group adopted the continuous negative pressure suction and the pressure value was set as 3.99-5.32 kPa (30-40 mmHg);the control group adopted the method of traditional intermittent negative pressure drainage and the suction pressure was set as 3.99-7.98 kPa (30 60 mmHg).After the operation,the two groups were observed and compared under continuous negative pressure suction.The capacity,color changes,length of time,and pressure value of the drain were monitored and observed,finding out their correlation with the operation effectiveness.Results For the experimental group,the incidence of subcutaneous hydrops and skin flap necrosis was lower (P＜0.05),and the primary healing rate was higher than the control group (P＜0.01).The capacity of drain was relevant to the type of the plastic surgery,different from 5 to 20 ml;the color changed gradually from blood color to faint yellow;the extubation time was generally from 7 to 10 days.The optimal pressure value was 3.99-5.32 kPa (30-40 mmHg);there was proportional relation between the effectiveness of monitoring drainage and the operation effectiveness.Conclusions Continuous negative pressure suction monitoring after operation contributes to improve the plastic surgery effectiveness,and the patients are satisfied with the result.

Clinical efficacy of repairing minor cleft lip with small incision

Objective To introduce a method of functional reparation for cleft lip with mini-approach.Methods We designed triangle flap in junctional zone between skin and mucosa,combined with intraoral incision approach,by means of repairing the orbicularis oris muscle,to obtain the reconstruction of labial arch,philtrum column and nasal wing.Results All cases in this study obtained perfect healing,with good augmentation at floor of the nose.Eight cases presented with good looking of philtrum and improvement in the shape of nostril.Conclusions Use of the method of functional reparation for cleft lip with mini-approach can decrease scar on skin,and hence decrease the contraction deform,as well as nice reconstruction of labial arch,philtrum column and nasal wing.

A new technique of modified levator muscle prolongation treatment of iatrogenic upper eyelid retraction

Objective To evaluate the effect of correcting upper eyelid retraction in patients with iatrogenic ophthalmopathy by a new technique of modified transsection and longitudinal suture levator muscle prolongation treatment.Methods The traditional levator muscle flap trapezoidal incision was changed into horizontal incision,According to the amount of upper eyelid retraction we designed a transverse incision with 1 ∶ 2 in the central part of levator aponeurosis.Along the line of incision was designed transverse incision of Mulller's muscle combined with levator aponeurosis complex was designed,with longitudinal suture of incision.Results Five patients (6 eyes) with iatrogenic upper eyelid retraction were treated by this method.The follow up was performed for 3 to 12 months.All were satisfied with good cosmetic results,the degree of bilateral corneal exposure was symmetrical,with no recurrence and complications,except for 1 cases of unilateral upper eyelid retraction width slightly asymmetry.Conclusions The application of modified levator muscle prolongation treatment is not only quite simple,easy to adjust in operation,but also with reliable effect and less complication.It is effective for iatrogenic upper eyelid retraction range from mild to moderate degrees of correction.

Effect of three-dimensional remodeling surgery for upper eyelid coronal S-shaped curve rejuvenation

Objective To explore the transformation of three-dimensional curve,and to observe effect of three-dimensional remodeling surgery in order to find an ideal method to achieve upper eyelid rejuvenation.Methods Retrospective analysis was conducted after surgical correction for three-dimensional reconstruction in 48 patients with upper eyelid that were female,aged 58 to 76 years;all of them had simultaneous surgery of both eyes.Using double eyelid crease incision to remove loose and redundant skin,we transplanted the herniated fat of the medial orbital septum to the deep surface of the orbicularis oculi muscle in the distinct bony structure of the extra-supraorbital margin,folded and lifted orbital septum fascia;at the same time lifted eyebrow through eyebrow incision,transplanted and filled dermis and muscle fascia flap to remodel the transverse S-shaped stereo curve of the coronal plane of the upper eyelid.Results A lot of 41 cases were followed up more than 6 months from 48 patients,except that one patient had a second surgery due to double eyelid asymmetry after one year,eyelid rejuvenation in 40 patients were improveed.Effect of 33 cases was satisfactory.Satisfaction rate was 80.49％.Less satisfaction were 6 cases,accounting for 14.63％.No satisfaction was 1 case,ac counting for 2.44 ％.Conclusions The surgery can reshape the concavity inside,the full crown outside of upper eyelid by the skin removing,fat shifting,leather fat muscle flap filling,and restore the upper eyelid to three-dimensional full round young shape.It is an ideal surgical method for eye anti-aging treatment.

Upper eyelid lipogranuloma following facial autologous fat injection: A retrospective study of 8 cases

Objective To investigate clinical characteristics and treatment of upper eyelid lipogranuloma cases that developed after facial autologous fat injection.Methods This retrospective study involved 8 patients who presented with upper eyelids symptoms (final diagnosis of lipogranulo ma) and had history of facial autologous fat injection.The collected data included information on patient sex,age,clinical presentation,laboratory findings,treatment and outcome.Results All 8 cases were managed by exploration and surgical removal through double eyelid surgery.All patients showed complete resolution over 1-12 months follow up period.Conclusions Lipogranuloma can develop in the upper eyelid after autologous fat injection into the face.and surgical removal can yield satisfactory outcomes in cases of repeated failure of conservative therapies.

Effectiveness of TGF-beta 1 neutralizing antibody against the capsule contracture of silicone prosthesis

Objective To study the effects and mechanism of immediate application of transformation growth factor beta 1 (TGF-β1) neutralizing antibody postoperatively to prevent the fibrous capsule formation in animal model,and to observe the different effects between two concentrations of TGF-β1 neutralizing antibody.Methods Ten ml of smooth round silicone prostheses were implanted subcutaneously in the back of rats.Then 25 μg/0.1 ml or 50 μg/0.1 ml TGF-β1 neutralizing antibody were injected around the implanted prostheses in the experimental group,while the control group was injected with PBS 0.1 ml.On day 7,14 and 28,the thickness of capsule measured;The density of collagen;and the expression of TGF-β1,alpha smooth muscle actin (α-SMA),and fibronectin (FN) was detected.Results After 28 days,the average capsule thickness of the control group was significantly higher in the experimental group,and the difference between the control group and the 25 μg experimental group had statistical significance.TGF-β1 expression and the number of α-SMA positive fibroblasts in control group were significantly higher than that in the experimental group,the difference was statistically significant.In the control group,the expression of FN and collagen density was slightly higher than the two experimental groups,but the differences among the three groups showed no statistical significance at each time point.Conclusions Immediate application of the TGF beta 1 neutralizing antibody in the the implant prosthesis can reduce the thickness of the capsule to some extent.

Effects of lycium barbarum polysaccharide on UV-mediated DNA strand breakage damage in HSF cells

Objective To investigate the protective effect of lycium barbarum polysaccharide (LBP) on DNA damage of HSF cells induced by UV.Methods We established the model of UV induced photo damage in HSF cells.We detected the viability of HSF cells by using MTT colorimetry.The UV absorption spectrum of LBP was also measured by UV spectrophotometer.The level of ROS was detected by DCFH-DA fluorescent probe method.Comet assay was employed to evaluate the DNA strand breakage damage.Results When the concentration of LBP was less than or equal to 300μg/ml,there was no significant effect on the proliferation of HSF cells (P＞0.05).When the concentration was more than 300 μg/ml,it could inhibit the cell proliferative activities (P＜0.05).Compared to the UV groups,UV+LBP groups can respectively improve the cell proliferation activity (P＜0.05).The absorbance was slight range 280 from 400 nm.Compared with the UV group,the relative fluorescence intensity and the migration distance of UV+ LBP groups were significantly decreased (P＜0.05).Conclusions Lycium barbarum polysaccharide can effectively inhibit the proliferation activity and protect the breakage of DNA strand induced by UV,which is probably due to its action of removing free radicals.

Clinical effect of large speckle modulated Q1064nm laser combined with Xiaoyao pills in treatment of chloasma

Objective To investigate the clinical efficacy of large spot modulated Q 1064 nm laser combined with Xiaoyao pills in the treatment of chloasma.Methods 90 patients with chloasma were treated in the hospital.The patients were randomly divided into 3 groups:treatment group were treated with Q 1064 nm laser combined with large spot Xiao Yao pill;the control group 1 was treated with large spot Q 1064 nm laser only,and control group 2 was treated with Xiaoyao Pill only.Results After 8 weeks of treatment,the effective rate was higher in the treatment group than the control group 2 (P＜0.05);after 16 weeks of treatment,the efficacy was better in the treatment group than the control group 1 and control group 2 (P＜0.05).The combination of the two methods can improve the curative effect and inhibit the recurrence.Conclusions The treatment of chloasma with large speckle modulated Q 1064 nm laser combined with Xiaoyao pills is effective and worthy of clinical promotion.

Effects of synthetical methods in the treatment of keloid

Objective To evaluate comprehensive therapy in the treatment of keloid effect and the patients' satisfaction.Methods From 2002 to 2015,a total of 523 patients with comprehensive treatment,according to the treatment the patients were divided into group A (surgery combined radiotherapy group) and group B (operation with corticosteroid hormone therapy group),group C (corticosteroids in combination with radiotherapy group),and the therapeutic effect of patients with satisfaction was evaluate.Results Total effective rate of three groups of patients were 85.7％,84.0％,64.9％;the efficiency in group A was higher than that of group B and group C;there was statistically significant difference between group A and C (P＜0.05),but there were no significant difference between the group A and group B (P＞0.05).All the patients were follow up for one years and the adverse reaction in the three groups was slight and happened in the 2 month after cure.And the adverse reaction was all self-cure in the final follow up.Difference was statistically significant between three groups of patients' satisfaction,group B better than in group A and group C;there was no significant difference between group A and group B (P＞0.05);the difference between group B and C group was statistically significant (P＜0.05).Conclusions Three kinds of comprehensive therapy effect are obvious,among which surgical adjuvant radiotherapy effect is best,being worth to recommend for clinical use.

Clinical efficacy of CO2 fractional laser on suborbital wrinkles

Objective To investigate the efficacy and side effects of carbon dioxide (CO2) fractional laser for the treatment of suborbital wrinkles.Methods Twenty-one female patients with suborbital wrinkles received a single treatment of CO2 fractional laser at fluences of 20-30 mJ with density of 10 ％-15 ％.Patients were photographed standardly and evaluated at baseline,one and six months after the treatment.Wrinkle improvements were graded in 5 levels:1 (no improvement or worse),2 (1％ 25％ improvement),3 (26％-50％ improvement),4 (51％-75％ improvement),and 5 (76％-100％ improvement).Patients rated the pain intensity by a visual analog scale (VAS,0 10).The side effects,such as hyperpigmentation,were observed and recorded.Results After one month follow-up,3-level improvement was observed in 7 patients (33.3％),2-level in 9 patients (42.9％);and after six months,3-level improvement was observed in 9 patients (42.9％),2-level in 8 patients (38.1％),but there was no statistically significant difference between one month and six months (P ＞0.05).The overall patient satisfactory rate was 81.0％.The average pain of VAS was 4.24± 1.73,7 patients (33.3％) had hyperpigmentation which faded within three months,and no patients had scars.Conclusions Single CO2 fractional laser treatment can improve the suborbital wrinkles in a cerrain extent with few side effects.
