A survey on test results of HBsAg,ALT and anti-HIV,anti-HCV,anti-TP antibodies among voluntary blood donors in Chongqing from 2008 to 2012

Objective To analyze the test results of hepatitis B virus surface antigen(HBsAg) ,alanine aminotransferase(ALT) and anti-hepatitis C virus (HCV) ,anti-human immunodeficiency virus (HIV) ,anti-treponema pallidum (TP) antibodies among voluntary blood donors ,and to provide basis for recruiting low-risk voluntary blood donors and reducing blood abandonment . Methods 551 133 blood samples derived from voluntary blood donors which had accepted enzyme-linked immunosorbent assay (ELISA) screening in Chongqing Blood Center from 2008 to 2012 were collected and were subject to HBsAg ,ALT and anti-HIV , anti-HCV ,anti-TP antibodies detection .Laboratory information management system ITSWELL was employed to read ,save and gather the test results ,and whether the samples were qualified was determined .Results Total of 37 534(6 .81% ) substandard blood samples were detected .Among them ,the substandard rates of HBsAg ,ALT and anti-HCV ,anti-HIV ,anti-TP antibodies were 1 .10% ,3 .79% ,0 .51% ,0 .33% and 1 .08% ,respectively .Conclusion Strengthening the screen of blood detection reagents and building the team of voluntary blood donors will contribute to the safety of clinical blood transfusion .

Evaluation on application of electrolyte module in OLYMPUS AU 5421 Automatic Biochemical Analyzer

Objective To evaluate the performance of electrolyte module in OLYMPUS AU5421 Automatic Biochemical Analy-zer in K+ ,Na+ ,Cl- detection .Methods Electrolyte module in OLYMPUS AU5421 Automatic Biochemical Analyzer was employed to detect K+ ,Na+ ,Cl- ,and their precision ,accuracy ,linearity range ,cross-contamination and the test results of instruments were compared .The linear regression equation and regression coefficients were calculated according to the methods of Clinical and Labo-ratory Standards Institute(CLSI) EP6-A ,EP9-A2 documents .Results The within-run precisions of K + ,Na+ ,Cl- were 0 .328% , 0 .306% ,0 .343% ,respectively ,and their between-run precisions were 0 .780% ,1 .012% ,0 .985% ,respectively .Their regression coefficients of linear equation were all greater than 0 .999 ,and their cross-contamination rate were 1 .066% ,0 .812% and 0 .858% , respectively .Conclusion Electrolyte module in OLYMPUS AU5421 Automatic Biochemical Analyzer modules has advantages of good precision ,accuracy ,linear range ,and low cross-contamination rate .

Investigation on influences of vitamin C,KCL and dicynone on 3 kinds of blood cross matching method

Objective To comprehend the influences of vitamin C ,KCL and dicynone on blood cross matching of saline and mi-crocolumn gel and to compare them with polybrene crossing matching .Methods Under the conditions with and without the specific antibody ,KCI injection ,dicynone injection ,vitamin C injection and normal saline were added for conducting the immunological reac-tion with corresponding RBC .The influences of 3 kinds of drug on different medium cross matching methods of different mediums . Results The low concentration of vitamin C and KCL does not affect polybrene cross matching ,dicynone makes the experiment to generate pseudoagglutination ;the high concentration of these 3 kinds of drug can cause the test to fail .The sensitivity of the saline medium method is slightly lower ,the influence of vitamin C and KCL on the test is inapparent ,while in the KCL method ,with the dilution of antibody ,the agglutination intensity is weakened and even disappeared .Conclusion 3 kinds of drug all have influences on the mierocolumn gel cross matching method ,especially KCL .

Specific mutations of ampD gene leading to the emergence of derepressed high-production of AmpC β-lactamase

Objective To reveal the influence factor of ampD specific mutation on the transformation of AmpC β-lactamase from non-high-productive type to derepressed the high-productive type in Enterobacter cloacae. Methods The selected cbromatin-mediated derepressed high-productive strains of Enterobacter cloacae and genomic DNA were extracted, and genomic ampD was amplified by using PCR, and then sequenced and confirmed by specific mutation. Use site-directed mutagenesis to mutate the wild-type Enterobacter cloacae for the above site. The changes of AmpCβ-lactamase types were detected by the improved Cefotaxime three-dimensional test. Results Of the 121 strains of Enterobacter cloacae,15 strains were ehromatin-mediated derepressed high-productive type,8 strains had significant mutations and 7 strains of site-directed mutagenesis were established. Ecl MA(274 inser-ting A)、Ecl MC(327 missing C)and Ecl MF(27 inserting G)were changed from non-high-productive type to derepressed-high-pro-ductive type,but Ecl MB(371 inserting T), Ecl MD(515 missing C), Ecl ME(324 C→A), Ecl MG(238C→A 302T→A)did not change in AmpC β-lactamase types. Conclusion The significant mutations that could lead to the transformation of AmpC β-lacta-mase were the Frame-shift mutations that could create the change of large region of amino acid. However, other mechanisms for the derepressed high-productive AmpC β-lactamase were not excluded in the study.

The correlation of HBV outer membrane large protein and HBV-DNA and its clinical significance

Objective To detect HBV outer membrane large protein(HBV-LP) and HBV-DNA, and to explore their correlation in judging the pathogenetic condition and HBV replication, as well as its clinical significance. Methods Serum level of HBV-DNA was quantitatively detected by u-sing real-time polymerase chain reaction, and while HBV-LP and HBV markers were measured with enzyme-linked immunosorbent assay(ELISA) in infected sera from 238 patients with hepatitis B. Re-suits There was significant difference of detection rate between HBV-DNA and HBV-LP(P<0. 05).The expression of HBV-DNA and HBV-LP had significantly difference among various patterns of HBV serological markers(P<0.05). HBV-LP expression was markedly correlated with the logarithm of HBV-DNA level(r=0. 677,P<0.01). Conclusion There is a prefect correlation between the copies of HBV-DNA and the level of HBV-LP, but HBV-LP expression can not still reflect the status ofHBV replication.

Survey on strain distribution and drug resistance in patients with infection of burn from 1993 to 2007

Objective To report and analyze the changes of bacterial species and resistance to antibiotics in the burn center from 1993 to 2007. Methods Bacterial identification and susceptibility test were performed by applying VITEK-AMS system. Results A total of 4 762 strains were isolated,including 60 species of 21 genera. Pseudomonas aeruginoasa (PA) was the most common organism (36. 79%), and its isolating rate was 36. 79%; in 2006, the highest isolating rate of PA was 51.6%.The isolating rate of Staphylococcus aureus was 14.09 %, next to that of PA; followed by Enterococci faecalis, Acinetobacter, Escherichia coli, Enterococci faecium, Enterobaeter cloacae and Staphylococ-cus epidermidis, and the 8 kinds of germs shared 78. 12% of bacterial isolates (3 746/4 762). The an-tibiotic resistance rate of Pseudomonas aeruginosa was increasing year by year; it was vitally resistant to the third generation eephalosporins and imipenem. Conchtsion The predominant species of bacteria from burn center is Pseudomonas aeruginosa, and RA isolates are multi-resistant to antibiotics. The i-solating rates of Enterococeus faeealis and Acinetobacter are increasing. This implied the importance of rational and correct use of antibiotics about control of burn infections as well as avoidance of abusage of broad-spectrum antibiotics.

Implications of partial plasma coagulation factor activity in acute coronary syndrome

Objective To investigate the activity of partial plasma coagulation factor activity and its clinical significance in occurrence of acute coronary syndrome(ACS). Methods The activities of co-agulation factors(FⅤ.C, FⅦ :C, FⅧ :C, FⅩ:C and FⅪ :C) were measured in 100 patients with acute myocardial infarction(AMI group) and 69 patients with unstable angina pectoris in acute stage(UAP group), the results were compared with those of 120 cases of stable angina pectoris(SAP group) and 80 healthy controls(heahhy control group). The relationship was analyzed between activities of coagu-lation factors and A MI as well as acute stage UAP. Results The patients with acute myocardial infarc-tion and unstable angina pectoris in acute phase had significantly higher plasma FⅤ.C, FⅦ :C, FⅧ :C, FⅩ:C and FⅪ :C compared with those of healthy and SAP subjects(P<0.05) ; there were no sig-nificant differences of the above activities between SAP group and healthy control group(P>0.05).Conclusion Significantly higher activation of FⅤ.C, FⅦ :C, FⅧ :C, FⅩ:C and FⅪ :C and a hyper-coagulabale state exist in acute coronary syndrome(ACS) patients in acute stage.

Drag resistance diversity of 1731 Pseudomonsd aeruginosa at Xinxiang City

Objective To explore the drug resistance diversity of pseufomonsd aeruginosa.Methods We did sensitivity experiments by 23 antibiotic of 1731 pseudomonad aeruginosa identified from clinic during 1997 to 2008,Results In 1 731 pseudomonad aeruginosa,the samples were main from phlegm,accounted for 58.5%.the second from secretion accounted for 24.7%.The drug resistance of pseudomonad aeruginosa for ampicillin、cefazolin and cefuroxime sodium were all higher than 90%,the second were trimethoprim/sulfamethoxazole 83.2%.The antibacterial medicines of drug resistance higher than 70%were netilmicn(75.2%)、ofloxacin(70.4%)、tetracycline(74.1%)、choramphenicol(72.0%).The antibacterial medicines of drug resistance Iower than 30%were Cefoperazone/Sulbactam、cefepime、imipenem、ceftazidme.In 12 years,the drug resistance diversity had enhanced prevalent trend.The enhanced speed of proportion drug was slow in 2004~2007,but it was fast again in 08 year.Conclusion The pseudomonad aeruginosa have become one of the most clinical routine nosogenetic germs.It showed single and anti muhiresistance of antibioties seriously.In order to improve antibacterial treating effect,we should inspect the drug resistance continuously in our area.We could select rationally antibacterial medicine according to the result of medicine sensitivity experiments.

Clinical application of microcolumn gel immunoassay in transfusion-related experiments

Objective To explore the applicable value of microcolumn gel immunoassay(MGIA) in transfusion-related experiments(blood typing, cross matching, antibody titration), and carry out its methodical evaluation. Methods Fifteen anti-Rh D positive samples from 55 Rh D negative patients, twenty-one cases of discrepant blood typing and twenty-nine cross matching positive samples were studied by applying MGIA and routine tube test simultaneously. Antibody titer data from 853 pregnant women with type O were brought into statistics. Results In 55 Rh D negative samples, the positive rate of Rh D antibody tested by MGIA and tube test was 27.27%(15/55) and 21.82 % (12/55) respec-tively,and the samples with titer 1:4 above got the coincident results by the two methods. For twen-ty-one discrepant blood typing samples, 11 cases with weak reaction showed discrepant results by MGIA;however, all samples showed weak reaction by tube test. In 29 samples with cross matching positive by MGIA,2 cases were confirmed to be negative by tube test. For the pregnant women with type O,IgG antibody titer abnormal ratio was 33.18%(144/434) if their respective husbands was with type A,33.17%(139/419) if their respective husbands was with type B.Conclusion MGIA is charac-teristic of convenience, fastness and reliability of test results, but its false-positive or false-negative should be paid attention to. When abnormal results occur, traditional tube test as well asmicroscopic examination should be performed in parallel.

The expressions of matrix metalloproteinase-9 and tissue inhibitor of metalloproteinase-1 in the endometrium of women with unexplained infertility

Objective To study the expression situation of matrix metalloproteinase-9(MMP-9) and tissue inhibitor of metalloproteinase-1 (TIMP-1) in the endometrium of women with unexplained infertility and normal endometrium, and to explore the relationship between MMP-9/TIMP-1 expres-sion and unexplained infertility. Methods Immunohistochemieal assay(SP method) was employed to deteet the expression of MMP-9/TIMP-1 in 20 cases of impaired endometrium(unexplained infertility group,endometrial plantation window phase)and 20 cases of normal endometrium(heahhy control group). Results There were different levels of MMP-9/TIMP-1 expression in kytoplasms of glandular epicytes and stromal cells of all endometrial samples. The expression of MMP-9/TIMP-1 was signifi-cantly weaker in unexplained infertility group than that in healthy control group(P<0.05). Conclusion Low expression of MMP-9/TIMP-1 in the endometrial plantation window phase may be one of impor-tant factors for unexplained infertility.
