Bingyang LIU ; Ranhua JIANG ; Ping LI ; Cong LIU ; Ling LI

Journal of China Medical University.2017;46(5):434-438,443. doi:10.12007/j.issn.0258?4646.2017.05.013

Objective This study aimed to identify high?risk groups for metabolic syndrome(MS)among Chinese children and adolescents by calculating the cutoff values of adiposity measures and insulin resistance(IR). Methods From December 2010 to January 2011,data from 933 Chinese subjects(492 boys and 436 girls, aged 11?16 years)from Liaoyang City,China, were included. Questionnaires were administered, and da?ta such as anthropometric parameters(height, waist circumference, and hip circumference),blood pressure, fasting blood glucose level, blood lip?id level, serum uric acid level, and fasting insulin level were collected. Receiver?operating characteristic(ROC)curve analysis was used to deter?mine the optimal cutoff waist?to?height ratio(WHtR),waist?to?hip ratio(WHR),homeostasis model assessment?insulin resistance(HOMA?IR) index, triglyceride level, glucose index(TyG index),and serum uric acid(SUA)level for identification of children and adolescents with MS. Results The AUC of WHtR,WHR,TyG index,HOMA?IR index, and SUA level were larger than 0.5,with P values of<0.01. The optimal cutoff WHtR and WHR derived from the ROC analysis were respectively 0.48(87.0%,84.1%)and 0.84(83.3%,70.3%)for boys and 0.46(95.1%, 81.8%)and 0.80(90.5%,71.8%)for girls. Conclusion This study demonstrated that WHtR and WHR are useful predictive markers of MS , while HOMA?IR,TyG index, and SUA level are not. The usefulness of these criteria needs to be verified by further evaluation.

Journal of China Medical University.2017;46(5):439-443. doi:10.12007/j.issn.0258?4646.2017.05.014

Objective To investigate the effect of FCGR3A polymorphisms on NK cell function. Methods Peripheral blood samples from can?cer patients were collected and FCGR3A polymorphisms were confirmed by PCR. In vitro proliferation rates,ADCC activity, and expression of NK cell activating receptors were compared under trastumab stimulation. Results This study showed that the wild?type FCGR3A exhibited a higher affinity to trastumab along with better NK cell proliferation and ADCC activity than the mutant type. Compared to the patients with wild?type FC?GR3A,the proliferation rates of NK cells in patients with the mutant type were reduced by approximately 8?fold. In addition, the expression of NK cell activating receptors in patients with wild?type FCGR3A was higher than in patients with the mutant type. Conclusion Mutations in FC?GR3Areduce NK cell function, causing a poor reaction to monoclonal antibody.

Treatment of Symptomatic Ureteral Calculi during Pregnancy

Shutao TAN ; Bin WU

Journal of China Medical University.2017;46(5):453-456. doi:10.12007/j.issn.0258?4646.2017.05.017

Objective To present our experience with the management of symptomatic ureteral calculi during pregnancy and to compare the effi?cacy of two treatments:double?J stent insertion and ureteroscopy. Methods In a retrospective study between January 2007 and January 2015,38 pregnant women were treated for symptomatic ureteral calculi in our hospital. The patients were divided into two groups based on whether they re?ceived double?J stent insertion(n=21)or ureteroscopic lithotripsy(n=17). We collected and compared the patients 'characteristics and treat?ment outcomes, such as success ratio, rate of complications, surgical time, medical costs, and hospitalization time. Results In the double?J group, the insertion of a double?J stent was possible for 17 of 21 patients(success rate,81.0%). In the ureteroscopy group,15 of 17 patients had complete calculi fragmentation(success rate,88.2%). Complications in the double?J stent retention group were slightly higher than in the ureteroscopic litho?tripsy group. The medical costs and hospitalization time required to perform ureteroscopic lithotripsy were significantly higher compared to double?J stent insertion. Patients in the double?J group contended with stones after delivery. Conclusion Both of double?J stent insertion and ureteroscopic lithotripsy are effective for the treatment of symptomatic ureteral calculi during pregnancy. The success rate of ureteroscopy ,which can directly dis?rupt stones, was higher. However, double?J stent insertion costs less and is simple and convenient. The technique that is chosen to be performed de?pends on the patient's practical situation and personal preference.

Xiaolin TANG ; Chunling PAN ; Chen LI

Journal of China Medical University.2017;46(5):449-452,462. doi:10.12007/j.issn.0258?4646.2017.05.016

Objective To explore the effect of dexamethasone(Dex)treatment on the expression of interleukin(IL)?6 and IL?8 in human peri?odontal ligament cells(hPDLCs). Methods hPDLCs were subjected to one of the following treatments for 24 h or 48 h:10-9 mol/L Dex;10-6 mol/L Dex;10μg/mL Porphyromonas gingivalis(P. g)?lipopolysaccharide(LPS);10-9 mol/L Dex+10μg/mL P. g?LPS;10-6 mol/L Dex+10μg/mL P. g?LPS;or 0.1%absolute ethyl alcohol(control). Protein and mRNA expression was detected using ELISA and real?time PCR ,respectively. Results At 24 h and at 48 h,IL?6 and IL?8 protein expression in the 10-9 mol/L Dex?and 10-6 mol/L Dex?treated groups was significantly lower than that in the control group(P<0.05). At 48 h,IL?6 mRNA expression in the 10-9 mol/L Dex?and 10-6 mol/L Dex?treated groups was signifi?cantly lower than that in the control group(P<0.05),while IL?8 mRNA expression in the 10-6 mol/L Dex?treated group was significantly higher than that in the control group(P<0.05). At 24 h and at 48 h,IL?6 protein and mRNA expression in the 10-9 mol/L Dex+10μg/mL P. g?LPS?treated group and the 10-6 mol/L Dex+10μg/mL P. g?LPS?treated group was significantly lower than that in the 10μg/mL P. g?LPS?treated group (P<0.05). At 24 h,IL?8 protein expression in the 10-9 mol/L Dex+10μg/mL P. g?LPS?treated group and the 10-6 mol/L Dex+10μg/mL P. g?LPS?treated group was significantly lower than that in the 10μg/mL P. g?LPS?treated group(P<0.05),while no such significant difference exist?ed at 48 h. At 48 h,IL?8 mRNA expression in the 10-6 mol/L Dex+10μg/mL P. g?LPS?treated group was significantly higher than that in the 10μg/mL P. g?LPS?treated group(P<0.05). Conclusion Dex inhibits the innate and P. g?LPS?induced expression of IL?6 in hPDLCs. However, Dex exerts profound effects on IL?8 expression, and treatment with high doses of Dex may promote IL?8 expression over an extended period.

Shiyang WANG ; Ye YANG ; Jialin ZHANG

Journal of China Medical University.2017;46(5):444-448. doi:10.12007/j.issn.0258?4646.2017.05.015

Objective To investigate the clinical diagnosis and treatment strategies for retroperitoneal paragangliomas and to enhance the diag?nostic and therapeutic levels of retroperitoneal paragangliomas. Methods The clinical data of 49 patients having retroperitoneal paragangliomas were retrospectively analyzed. We studied 21 males and 28 females between an age range 17 and 72 years(mean 45.6 years). Hypertension was observed in 19 patients,9 patients suffered from lumbago or abdominal pain, and an abdominal mass was noted in 2 cases. Physical examination re?vealed paragangliomas in 17 patients. The positive rate of localization was 77.5%(31/40),96.0%(47/49),and 100%(10/10)using B?ultra?sound, computed tomography, and MRI respectively. Serum catecholamines were measured in 19 patients,of which 16 cases(84.2%)showed ele?vated levels. Results In our study,49 patients underwent open surgery,5 patients showed multiple tumors, and 56 tumors were found during sur?gery. The most common tumor location was periaortic and pericaval(71.4%,40/56). There were 51 cases of surgery in 49 patients. 54 tumors of 49 cases were totally resected. The tumors measured 1.5?20.0 cm in diameter with a mean diameter 6.7 cm. 75%tumors had integral pelos,14.2%(8/56)invaded the surrounding organs and vessels, and 10.7%(6/56)required resection of adjacent organs. We followed up 43 patients between 6 and 138 months after surgery. Postsurgical recurrence was noted in 4 patients, and 2 patients developed metastases. One patient died from metas?tasis during the follow?up period. Conclusion Surgical resection might be the most effective method for treatment of retroperitoneal paraganglio?mas. Sufficient preoperative and perioperative management might be the key point of successful surgery. Lifelong follow?up is important as recur?rent tumors need resection.

Yun XIANG ; Yunxiao SHANG ; Miao LI

Journal of China Medical University.2015;(6):552-556,561.

Objective To explore the effect of Lactobacillussalivariuson the number of CD4+CD25+Foxp3+Treg cells and expression of transform?ing growth factorβ1(TGF?β1)in asthma Balb/c mice. Methods Thirty?two female Balb/c mice were randomly divided into four groups:the nor?mal control group, the asthma group, the Lactobacillus salivarius group, and the asthma combined Lactobacillussalivariusgroup. Acute asthma mod?el was established by the ovalbumin challenge method. After extraction of primary spleen cells, flow cytometry was used to test CD4+CD25+Foxp3+Treg/CD4+T ratio in spleen lymphocytes. The levels of IL?4,IFN?γand TGF?β1 in the spleen cell culture supernatant were measured by ELISA method. Results The level of Th2 cytokine(IL?4)in the spleen cell culture supernatant of the asthma group was significantly higher than that of the control group(P<0.05),however,the level of Th1 cytokine(IFN?γ)was significantly lower than that of the control group(P<0.05). The ex?pression level of Th2 cytokine(IL?4)in the Lactobacillussalivariusintervention group was significantly decreased compared with the asthma group, and the Th1 cytokine(IFN?γ)expression level was elevated compared with the asthma group(P<0.05). The level of TGF?β1 in the Lactobacillus salivarius intervention group was higher than in the asthma group(P<0.05). The proportion of CD4+CD25+Foxp3+Treg/CD4+T in spleen lympho?cytes in the asthma group was lower than that in the control group(P<0.05),and was higher in the Lactobacillus salivarius intervention group than in the asthma group(P<0.05). Conclusion CD4+CD25+Foxp3+Treg was associated with the pathogenesis of asthma. Lactobacillus salivarius could adjust Th1/Th2 imbalance and reduce asthma inflammation through up?regulation of CD4+CD25+Foxp3+Treg and TGF?β1 expression.

Peng MIAO ; Lin GUAN ; Chengzhong XING

Journal of China Medical University.2015;(6):528-532,537.

Objective To systematically assess the relationship between oral administration of aspirin and survival benefit for patients with colorec?tal cancer(CRC)by meta?analysis. Methods Relevant studies were identified through searching PubMed and EMBASE. Random?effects model was derived to composite the pooled hazard ratio for overall mortality and CRC?specific mortality. The subgroup analysis was conducted for included data, and the bias analysis was reported. Results Thirteen studies on aspirin therapy were finally included in this meta?analysis. The overall surviv?al benefit associated with oral administration of aspirin represented an HR of 0.83(95%CI:0.71?0.96). Oral administration of aspirin was also asso?ciated with CRC?specific survival(HR=0.77,95%CI:0.64?0.93). No evidence was observed of an association between prediagnostic aspirin use and CRC overall survival(HR=1.00,95%CI:0.85?1.18)or CRC?specific survival(HR=0.97,95%CI:0.83?1.13). Conclusion These findings provide further indication that post?diagnostic aspirin therapy can improve CRC prognosis.

Shaoting HU ; Shengjin LI ; Qin HUANG

Journal of China Medical University.2015;(6):562-564,572.

Objective To investigate the effect of myeloid differentiation factor 88 inhibitor ST2825 on the autophagy of THP?1 cells infected by re?combinant mycobacterium smegmatis. Methods The myeloid differentiation factor 88 inhibitor ST2825 was applied on the THP?1 cells infected by recombinant mycobacterium smegmatis, and three groups were defined:the test group with ST2825 treatment, the control group without ST2825 treatment, and the blank group. Autophagosomes were observed under the fluorescence microscope, and the mRNA expression of Beclin?1 gene and Bcl?2gene was analyzed by RT?PCR. Results Compared with the control group, the number of autophagy fluorescent dots in the test group was ob?viously reduced(P<0. 05),and the expression levels of Beclin 1 gene and Bcl?2 gene were declined as indicated by the RT?PCR detection. Con?clusion The myeloid differentiation factor 88 inhibitor ST2825 might inhibit the autophagy of THP?1 cells through interfering the separation of Be?clin?1 and Bcl?2.

Journal of China Medical University.2015;44(8):704-708,713.

Objective To discuss the diagnostic value of two-dimensional speckle tracking imaging in the evaluation of left ventricular rotation and torsion in pre-apical hypertrophic cardiomyopathy(PAHCM)patients. Methods A total of 26 patients with PAHCM,26 patients with hyperten-sive left ventricular hypertrophy(HLVH)and 26 healthy volunteers were recruited for the study. Two-dimensional echocardiographs were performed carefully. Two-dimensional images were obtained from the left ventricular apical four-chamber section,two-chamber section and two short-axis sec-tions,including mitral valve and apical levels. The peak subendocardial rotation(endo-rot),epicardial rotation(epi-rot),transmural torsion(mural-tor)and bulk rotation(bulk-rot)of each short-axis section were measured and analyzed using QLAB 9.1 software. Results In the mitral level, there was no statistically significant difference in endo-rot,epi-rot,mural-tor and bulk-rot between the PAHCM group and normal control group (P>0.05). There was statistically significant difference in endo-rot,epi-rot,mural-tor and bulk-rot between the PAHCM group and HLVH group (all P<0.05). There was statistically significant difference in endo-rot,epi-rot,mural-tor and bulk-rot between the HLVH group and normal control group(all P<0.05). In the apical level, there was no statistically significant difference in epi-rot among three groups(P>0.05). There was statisti-cally significant difference in endo-rot,mural-tor and bulk-rot between each two groups(P<0.05). There was statistically significant difference in G-tor between each two groups(P<0.05). The receiver operating characteristic curve analysis showed that the accuracy of differential diagnosis of G-tor PAHCM was high(the area under the receiver operating characteristic curve is 0.89),the sensitivity was 73.08%,and the specificity was 92.31%. Conclusion Two-dimensional speckle tracking imaging could accurately and quantitatively measure the left ventricular rotation and tor-sion in PAHCM patients. The G-tor could accurately screen and identify between PAHCM and HLVH.

Penglian WU ; Yan LI ; Xinhui FU ; Lianghe DU ; Dongyu WANG

Journal of China Medical University.2015;44(8):694-698,703.

Objective To investigate the effects of PJ34,a poly ADP-ribose polymerase(PARP)inhibitor,on the expression of matrix metallopro-teinases-9( MMP-9 )and Claudin-5 in ischemic cortex of rats with focal cerebral ischemia-reperfusion(I/R)injury. Methods The focal cerebral ischemia-reperfusion model of middle cerebral artery occlusion(MCAO)was established by intraluminal suture . PJ34 was injected intraperitoneal-ly. Blood-brain barrier(BBB)permeability was quantitatively determined by Evans blue assay. Infarct volume changes were observed by 2,3,5-tri-phenyltetrazolium chloridedyeing(TTC)staining. The expression of the MMP-9 and Claudin-5 in rats of cerebral cortex were measured by immuno-histochemistry assay and western blot analysis . Results Compared with sham group, the expression of MMP-9,the contents of EB and infarct vol-ume increased progressively over time after I/R,and reached maximum levels at 48 h(P<0.05);The expression of Claudin-5,the contents of EB and infarct volume reduced significantly over time after I/R,and reached the minimum levels at 48 h in model group(P<0.05). Compared with model group, the expression of MMP-9,the contents of EB and infarct volume was reduced significantly and the expression of Claudin-5,the con-tents of EB and infarct volume was increased at the same time point in PJ34 group(P<0.05). Conclusion PJ34 maintained the blood-brain barri-er permeability by inhibiting the expression of MMP-9,and increasing the expression of Claudin-5,which had neuroprotection on cerebral ischemia-reperfusion injury.
