Risk Assessment and Intervention of Ergonomic Risk Factor in Beekeeping

OBJECTIVES: This study investigated musculoskeletal symptoms in upper limbs according to the working environment (job stress) of dental hygienists and examine their relationship with upper limb functions. METHODS: The subjects include 198 dental hygienists in dental hospitals and dental clinics in Pusan and Gyeongnam. The questionnaire was consisted of general characteristics of the subjects, job stress, musculoskeletal symptoms in upper limbs and function (Disability Measurement Tool for Upper Extremity Disorders-11, DASH-11). RESULTS: The study was analyzed their musculoskeletal symptoms in upper limbs according to their general characteristics and found that the symptoms occurred in the neck (39.4%), the shoulders (54.6%), elbows (14.7%), and the hands (50.0%). Job stress was associated with upper limb functions (DASH-11) (model 3, B=5.210, p=0.012) and repeated elbow bending and spreading posture was associated with DASH-11 (model 3, B=6.561, p=0.029). Elbow symptoms were associated with DASH-11 in the upper limbs (B=10.679, p=0.003). CONCLUSION Dental hygienists are experiencing limitations of upper limb function due to job stress. In particular, even if the correction of their uncomfortable posture is significantly related to the job stress and upper limb function, in order to improve the upper limb function of the dental hygienist, efforts to reduce the job stress as well as the uncomfortable posture are necessary.

Analysis of Medical Expenses Structure for Patients on Percutaneous Coronary Intervention by Medical Security Type

OBJECTIVES: The purpose of this study was to analyze whether there are differences in medical expenses according to medical security type in the use of medical services with high disease burden such as coronary intervention. METHODS: Chi-square test and covariance analysis(ANCOVA) were conducted to identify the differences in the characteristics and costs according to medical security type of 1,904 patients who underwent coronary intervention in a university hospital from 2011 to 2012. Hierarchical regression analysis was conducted to determine whether the cost affects medical expenses. RESULTS: In the medical aid group, the proportion of women, those without a job, those without a spouse, and those who received hemodialysis was high, length of stay was high, patients using the emergency room and those who died was high. The medical aid patients were significantly higher in the non-benefit medical expenses, optional medical expenses, physician and admission, meals, medications and injections. National health insurance patients were significantly higher in procedure. The medical security type was found to be significant as a variable affecting the medical expenses. CONCLUSIONS Provision of medical expenses should be managed in advance by providing prevention and education services for the vulnerable, and care services in the region should be provided to suppress the occurrence of medical expenses due to the increase in the number of days spent. In addition, it is necessary to support medical expenses to prevent unsatisfactory medical services from occurring for non-benefit and optional care.

Usefulness of Community Health Survey for Regional Disparity Study in Gunsan-si, Jeollabuk-do

OBJECTIVE: In Gunsan, Jeollabuk-do, Korea, we wanted to determine if the sluggish local economy could affect citizens' health behaviors, especially mental health. METHODS: We divided Gunsan-si into 5 living areas and conducted Small-Area Estimations and confirmed the modified compound estimation value using the 2013-2017 Community Health Survey data and population data from Gunsan-si. RESULTS: The health behaviors and mental health of the residents of the western living area(Soryong-dong, Misung-dong), which is an industrial hub of Gunsan, had deteriorated or decreased compared to those of other regions. CONCLUSIONS Although there are limitations in analyzing the community health survey data using the small-area estimation method, it could be useful data for evaluating regional gaps and health level.

Factors associated with Experience of Diagnosis and Utilization of Chronic Diseases among Korean Elderly : Focus on Comparing between Urban and Rural Elderly

OBJECTIVES: The purpose of this study was to identify and compare the difference and related factors with general characteristic and health behaviors, a experience of diagnosis and treatment of chronic diseases between rural and urban among elderly in Korea. METHODS: We used the data of Community Health Survey 2017 which were collected by the Korean Center for Disease Control and Prevention. The study population comprised 67,835 elderly peopled aged 65 years or older who participated in the survey. The chi-square test, univariate and multivariate logistic regression analysis were used to analyze data. RESULTS: We identified many significant difference of health behaviors, an experience of diagnosis and treatment with chronic diseases between rural and urban. Compared to urban elderly, the odds ratios (ORs) (95% confidence interval) of rural elderly were 1.136 (1.092–1.183) for diagnosis of diabetes, 1.278 (1.278–1.386) for diagnosis of dyslipidemia, 0.940 (0.904–0.977) for diagnosis of arthritis, 0.785(0.736–0.837) for treatment of arthritis, 1.159 (1.116–1.203) for diagnosis of cataracts, and 1.285(1.200–1.375) for treatment of cataracts. In the experience of diagnosis and treatment of chronic diseases, various variables were derived as contributing factors for each disease. Especially, there were statistically significant difference in the experience of diabetes diagnosis, arthritis diagnosis, cataract diagnosis and dyslipidemia except for hypertension diagnosis (p<0.01) between urban and rural elderly. There were statistically significant differences in the experience of treatment for arthritis and cataract (p<0.01), but there was no significant difference in the experience of treatment for hypertension, diabetes, dyslipidemia between urban and rural elderly. CONCLUSION Therefore, it would be necessary to implement a strategic health management project for diseases that showed significant experience of chronic diseases with diagnosis and treatment, reflecting the related factors of the elderly chronic diseases among the urban and rural areas.

The development and evaluation of curriculum for developing physician’s competencies in public health

Purpose: The purpose of this study is to develop and evaluate the curriculum, that was the continuing professional development program, for cultivating physician’s competencies in public health. Methods: This study was conducted through in-depth interview and survey in the frame of ADDIE, from 2 May to 30 June in 2019. Participants were 7 physicians by in-depth interview and Respondents were 46 physicians worked in public health by survey. Results: The results were analyzed and described in the frame of ADDIE model. In the stage of Analysis, physician’s core competencies in public health was presented to practical education, management of organization and labor force, communication with community, and director’ role of public health center in the community as core competencies by in-depth interview. The skill–knowledge-attitude competencies were highest in order in the survey for placing priority (by Borich score) of 29 core competencies. In design stage, the educational objective of curriculum was established, that is to develop the competencies of knowledge, skills, and attitude needed for physician of public health in 21th century. In development and implementation stage, it is important to decide to lecturer, to get in advance educational materials to do the maintenance and management of curriculum. In the stage of evaluation, the educational satisfaction was high on the whole and educational effect was statistically significant. Conclusions This study was an initial study of Korean doctors, and it aims to pursue competency-based education as a continuing professional development (CPD) beyond continuing medical education (CME) including knowledge, skills, and attitudes.

Regional Variation of EQ-5D Index and Related Factors in Community Health Survey: Major Role of Psychosocial Factors in Korea

Objectives: As an ecological study, this study was performed to identify the community-level variation of health related quality-of-life (HRQOL), and to explore the factors that explain the variation, using 2017 Korean Community Health Survey (KCHS) data. Methods: Community health indicators of KCHS, which are correlated with the EQ-5D index of Si-gun-gu districts, were selected as independent variables. Multiple linear regression model was used to derive factors that explain regional variations in the EQ-5D index. Results: The EQ-5D index variation in 229 districts nationwide was 1.1 times for extremal quotient (EQ) and 1.0 for coefficient of variance (CV). The Si-gun-gu districts with the EQ-5D index in the lower 25% were more distributed in the province (27.7%) than in the metropolitan area (20.3%). As a result of multiple linear regression analysis, the depressed mood experience rate, perceived stress rate, suicide ideation rate, and physician diagnosed arthritis rate were derived as major factors of the variation. Conclusions In order to reduce the gap in HRQOL between the districts, the priority of local health policies should be placed on the above factors including psychosocial factors.

Policy Compliance and Smoking related Behavior Change in a Smoke-free Apartment

Objectives: The purpose of this study is to identify the policy compliance according to the socio-demographic characteristics, stress, satisfaction of relationship, Health Belief Model and characteristics of smoking behaviors and knowledge related smoking of the residents living in the Smoke-free apartment and to investigate the change of the smoking related behavior before and after the implementation of the policy. Methods: The subjects of the survey were 293 residents aged 19 or older who reside in apartments six months after the designation of Smoke-free apartment located in Daejeon. All collected data were analyzed using T-test, ANOVA and Hierarchical multiple regression of SPSS for windows(ver 24.0) program. Results: The policy compliance was higher in women and those who lived for less than two years, and non-smokers were significantly higher. The policy compliance of the whole subjects was related to satisfaction with the surrounding relationship, perceived disability about smoking cessation, frequency of smoking and the amount of smoking. The policy compliance of the whole subjects was related to the satisfaction of the surrounding relationship, the perceived disability about smoking cessation, and the amount of smoking. In addition, after the implementation of the gold research station designation policy, it had a positive impact on smoker’s smoking behavior and resulted in reducing the frequency of secondhand smoke among residents. Conclusions In order to increase the compliance of the non-smoking policy It is necessary to continuously expand the indoor smoking cessation zone policy, develop policies and supplement the health belief model components.

Factors related to COVID-19 Incidence and Mortality rate in Gyeongsangbuk-do, Korea

Objectives: Gyeongsangbuk-do has entered a super-aged society with 20.7% of the population aged 65 and older. As of April 30, 2020, the death rate of COVID-19(3.8 people) in Gyeongsangbuk-do is higher than the national mortality rate (2.3 people), and the fatality rate of COVID-19 by age accounts for more than half of the total of 58.6%, so it is time to propose to prevent infectious diseases in the event of additional infectious disease disasters COVID-19. Methods: We collected daily data on the number of confirmed cases and deaths due to COVID-19 from 19 February to 30 April 2020. The data collected was evaluated using the SPSS 21.0 statistical package. Results: As a result of comparing the incidence and death-related factors of confirmed patients in Gyeongsangbuk-do, there were significant differences in age group (p<0.001), underlying disease (p<0.001), and residence type (p<0.033). Conclusion Factors affecting the mortality rate of confirmed patients in Gyeongsangbuk-do have been combined with individual level factors(age, gender, underlying disease), which means individual characteristics that have existed since before the disease, and regional level factors(Type of Residence), which are external factors that enable the use of medical resources. Therefore, each local government is required to establish preventive measures considering individual and regional level factors.

Factors Affecting Post-traumatic Growth of Maritime Police Officers

Objectives: The purpose of this study was to identify factors influencing the posttraumatic growth of maritime police officers. Methods: The subjects include 293 maritime police officers that received basic and professional education at the Korea Coast Guard Academy on June 17~July 31, 2019. Data was analyzed in frequency, percentage, mean, standard deviation, t-test, variance analysis, Pearson's correlation analysis, and multiple regression analysis with the SPSS Version 21.0 program. Results: The findings show that the maritime police officers scored mean 39.72±16.56 points in posttraumatic growth. marital status(β=.194, p=.002), smoking(β=.152, p=.010), regular exercise(β=-.185, p<.001), total duration of service(β=-.154, p=.033), resilience(β=.310, p<.001), social support (β=.250, p<.001), and collective culture(β=.158, p=.004) had impacts on their posttraumatic growth. Conclusion The findings of the study demonstrate that the posttraumatic growth of maritime police officers was under the influence of supports from families and peers, working environment, and culture as well as their individual characteristics.
