A Comparative Study on Health Risk Factors between Cancer Patients and Healthy People.

PURPOSE: The purpose of this study was to provide the basic data of a comprehensive counter-plan to promote health for people by comparing various factors related to cancer outbreak factors including general characteristics, health-related lifestyles, health promoting behaviors, perceived healthy status, life event stress and health care seeking behaviors with cancer patients and health people. METHOD: The study was designed to be a retrospective- comparison-survey-study and its data was collected through 5 types of questionnaires from September 2003 to December 2003. RESULTS: Cancer patients had lower education and household income, and many of them had no occupation. Cancer patients had a higher rate of smoking and drinking periods. Exercise level was higher in healthy people. However, Cancer patients were better in sleep and rest. In comparison with health promoting behaviors, it was higher in healthy people than in cancer patients but the two groups had no significant differences statistically. The perceived health status was higher in healthy people than in cancer patients. The health care seeking behaviors were higher in healthy people than in cancer patients. Cancer patients and healthy people's life event stress had no significant differences statistically. CONCLUSION: As a result of comparative analysis of cancer patients and healthy people's health risk factors, smoking, drinking period, regular exercise and health care seeking behaviors were suggested as direct or indirect risk factors for cancer patients. These finding can be applied to health promoting behavior programs to keep and promote optimal health status as well as to prevent cancer disease.

Mood State, Instrumental Activities of Daily Living and Attitude toward Aging according to the Existence of a Spouse.

PURPOSE: This study examined the mood state, activities of daily living, and attitude toward aging in the elderly according to the existence of a spouse and investigated the relation between these factors. METHOD: The subjects consisted of 190 elderly people, who were classified into two groups: those with and without a spouse. The data was collected from March to December 2004 using a structured questionnaire that included general characteristics, mood states, instrumental activities of daily living (IADL), and an attitude toward aging scale. The collected data was analyzed using the program SPSS, including descriptive statistics, chi2-test, ANOVA, Scheff's test, and the Pearson correlation coefficient. RESULT: 1. There were significant differences between the two groups in mood state (t=-3.349, p=.001), anxiety-depression (t=-3.350, p=.001), and anger (t=p=.003) in the mood state subscales, and in attitude toward aging (t=2.514, p=.013). 2. There was a significant positive correlation between vigor and IADL (r=.253, p=.012) in the elderly with a spouse. 3. When there was a spouse, there was a significant difference according to gender (t=2.587, p=.012) in IADL. Without a spouse, there were significant differences in mood states according to education level (F=3.315, p=.023), in anxiety-depression according to the presence of illness (t=2.156, p=.033), in vigor according to age (F=3.439, p=.020) and education level (F=5.285, p=.002), and in IADL according to monthly income (F=3.322, p=.023) and the presence of illness (t=2.172, p=.032). CONCLUSION: An individualized approach is needed for elderly people that considers living arrangements and the existence of a spouse.

An Effect of the Secondary Stroke Prevention Education Program on Self-care of Acute Ischemic Stroke Patients.

PURPOSE: This study was conducted to examine the effects of the secondary stroke prevention education program to inhibit the recurrence of the acute ischemic stroke patients, and to maintain and promote knowledge about stroke, self-efficacy and self-care. METHOD: This study was designed to take a quasi-experimental pre- and post-test with the nonequivalent control group. The experimental group consists of 20 patients and control group consists of 20 patients. The experimental group was applied the secondary stroke prevention education program. In order to verify the effects of the secondary stroke prevention education program, knowledge about stroke, self-efficacy and self-care scale were measured before the intervention and 4 weeks, 12 weeks after discharge. The tools for measuring knowledge about stroke, self-efficacy and self-care are developed by the researcher. The data was analyzed by SPSS win 10.0 program using chi2-test, Fisher's Exact Test, t-test, Kolmogorov- Smirnov Z, and Repeated Measures ANOVA. RESULT: There was a statistically significant difference in knowledge about stroke (F=4.021, p=.026), self-efficacy(F=6.096, p=.018), and self-care(F=8.026, p=.007) between the experimental and the control group after intervention. CONCLUSION: It is considered that the program can be used as an effective nursing intervention in clinical practice.

A Study of Development and Evaluation of Tutorial Management Strategy for Web-based Nursing Education.

PURPOSE: This study was attempted and completed in order to settle down and qualitatively improve web-based nursing education by evaluating effect and managing strategy of tutor. METHOD: The development of tutor's managing strategy was based on "The Self-regulated Learning" and "The supportive Learning", then it was applied to 79 learners in one of the cyber-learning centers. After applying the tutor's managing strategy, self-regulated learning scale, attitude for school, preference for computer and academic achievement were evaluated. The development of tutor's managing strategy for web-based nursing education are consisted of participation promotion, psychological support and motivation, recognition and promotion strategy of learning activity, management strategy of evaluating stage. RESULT: The levels of learner's self-regulated learning, recognition, behavior, attitude on the school and learning achievement were meaningfully increased in statistics after applying for the managing strategy of tutor. The motivation level and learning participation kept high scores from the beginning with no significant statistical changes. CONCLUSION: It is required to develop an educational program for cultivating well-educated tutors in order to help the effective learning process of nurses based on understanding characteristics of learners.

Evaluating the NGC(TM) Evidence Based Guideline of Prompted Voiding for Use in Korea.

PURPOSE: The purpose of this study was to evaluate the applicability of the evidence based guideline for prompted voiding by Lyons & Specht (2001) in National Guideline Clearinghouse(TM) for use in Korea based on the experts' opinions. METHOD: The target expert group consisted of 8 registered nurses, 6 physicians, and 5 nursing professors who are experts in urinary incontinence. This study used a questionnaire survey. The appropriateness, applicability, and the present application of each recommendation in the guideline were analyzed with descriptive statistics using the SPSS program, with content analysis based on the experts' opinions. RESULT: The scores on each recommendation's appropriateness showed the high degree of agreement among nurses, physicians, and nursing professors. However, the recommendation for 'use of oxybutinin' showed the lowest score as 5.89. It was notable that the most recommendations scored lower for applicability compared with appropriateness. The reasons for lower scores for applicability were the lack of clinicians' knowledge of assessment and management, and the lack of resources in clinical settings in Korea. CONCLUSION: This study will augment the understanding of the actual urinary incontinence management in Korean clinical settings and can be used as the baseline data for further study of tailoring international guidelines into local and national clinical settings.

The Effect of Chest Meridian Massage on Post- Anesthetic Recovery of General Anesthesia Patients.

PURPOSE: This study aims at confirming the effects of the chest meridian massage on the post-anesthetic recovery of general anesthesia appendectomy patients. METHOD: The research was post-test designed as a nonequivalent control group among quasi-experiments tested. The data was collected from June 20, 2003 to October 14, 2003 at a hospital in P city. In the collected data, using SPSS Win 10.0 program, these general features were analyzed by real numbers and percentages; the homogeneity among variables by chi2-test and t-test and the research hypothesis by the t-test. RESULT: Hypothesis 1; The experimental group receiving the treatment of the chest meridian massage will have a much higher post-anesthetic recovery score than that of the control group not receiving it was supported(t=2.544, p=.014). Hypothesis 2; The experimental group receiving the treatment of the chest meridian massage will have a much shorter time of stay in the recovery room than that of the control group not receiving it was supported(t=-4.919, p=.000). CONCLUSION: According to these results, it may be concluded that Chest Meridian Massage is effective in producing a higher level of post-anesthetic recovery score and helps appendectomy patients reduce the time of stay in the recovery room. Therefore the chest meridian massage can be considered as an intervention therapy for directly nursing general anesthesia appendectomy patients.

The Meaning of Death Seen by Nursing Students through Art Works.

PURPOSE: To understand the meaning of death seen by nursing students through art works. METHOD: The qualitative research method and the content analysis were used. The text were essays written by 42 senior nursing students about their impressions on death through art works. RESULT: Statements were classified into six categories and twenty six themes. The six categories include definitions of death, feelings about the death of a main character, responses to the death of the main character, feelings of significant others about the death of the main character, feelings of participants in this study, and oaths of the participants in this study. CONCLUSION: It is shown that participants recognize their position as nursing students, although they may not escape the fear of death, will learn to cope with death, and the dying in a suitably professional manner. In this respect, the study is considered to be helpful for the students in learning the knowledge and information which are needed for hospice nursing care more effectively.

Features Related to Smoking and Psychological Characteristics of Coronary Artery Patients Associated with the Stage of Change in Smoking Cessation.

PURPOSE: The purpose of this study was to investigate differences in smoking related characteristics and psychological features of coronary artery patients by the stages of change in smoking cessation behaviors. METHOD: Subjects for this survey were 97 patients who were smoking when doctors diagnosed them to have coronary artery diseases. RESULT: Subjects were distributed 21.6% in precontemplation stage, 24.7% in contemplation stage, 17.5% in preparation stage, 19.6% in action stage, and 16.5% in maintenance stage respectively. The numbers of previous attempts to quit smoking of subjects in precontemplation stage(mean=3.00, SD=3.71) and contemplation stage(mean=2.63, SD=2.32) were significantly lower than that of subjects in preparation stage(mean=5.82, SD= 6.20). Benefit scores of subjects in maintenance stage were significantly greater than those of subjects in precontemplation stage. Self-efficacy, barriers, seriousness, and nicotine dependency were not significantly different by the stages of change. Number of signs and symptoms related to smoking which subjects were experiencing were not significant by the stages of change too. CONCLUSION: Future intervention programs for smoking cessation should be focused on the strategies to enhance the realization of health benefits that patients might acquire from smoking cessation.

Phenomenological Research on the Experience of Weight Control Among Nursing Students.

PURPOSE: This study was conducted to discover the essence and the structure of the experiences of weight control by nursing students. METHODS: Participants were 12 nursing students who had tried losing weight at C university in Busan, Korea. The data were collected from June 17 to August 6, 2006. For the purpose of this study, focus group discussions and in depth interviews were employed. RESULTS: The collected data were analyzed by using Giorgi's method. As the results of the analysis, the following four components of experience were derived: My appearance looked abnorml; Measures for self-realization; Pursuit of my own way only; Feelings of accomplishment and self confidence. CONCLUSION: It is necessary to develop an effective weight control program with minimized side effects that are harmful to health, based on the components of the nursing students' experiences in weight control that were identified in this study.

Symptom Clusters in Patients with Breast Cancer.

PURPOSE: The purpose of this study was to identify symptom clusters in patients with breast cancer and to investigate the associations among them with functional status and quality of life (QOL). METHODS: A convenient sample of 303 patients was recruited from an oncology-specialized hospital. RESULTS: Two distinct clusters were identified: A gastrointestinal-fatigue cluster and a pain cluster. Each cluster significantly influenced functional status and QOL. Based on these two clusters, we identified subgroups of symptom clusters using K-means cluster analysis. Three relatively distinct patient subgroups were identified in each cluster: mild, moderate, and severe group. Disease-related factors (i.e., stage, metastasis, type of surgery, current chemotherapy, and anti-hormone therapy) were associated with these subgroups of symptom clusters. There were significant differences in functional status and QOL among the three subgroups. The subgroup of patients who reported high levels of symptom clusters reported poorer functional status and QOL. CONCLUSION: Clinicians can anticipate that breast cancer patients with advanced stage, metastasis, and who receive mastectomy, and chemotherapy will have more intense gastrointestinal-fatigue or pain symptoms. In order to enhance functional status and QOL for patients with breast cancer, collective management for symptoms in a cluster may be beneficial.
