Research on noise exposure control theory for the protection of the naval ship crew exposed to occupational noise

Objective To investigate hearing conservation theory for the protection of the naval ship crew exposed to occupa -tional noise , so as to provide scientific evidence for the development of noise protection measures .Methods Occupational noise expo-sure level was studied in the light of sound theory and equivalent principle of sound energy .Results A mathematical formula for the de-termination of occupational noise exposure level was thus brought about .The following countermeasures were practical approaches for the control of noise exposure levels:the decrease of environmental noise , the selection of better ear attenuators , the persistent wearing of ear attenuators or mufflers , the decrease of daily noise exposure time , and the perfection of noise protection regulations .Conclusion The mathematical formula for the determination of occupational noise exposure level forms a close coverage of the noise protection methods .

Inhibitive effect of total alkaloids from Pinellia ternate on the proliferation of human hepatic carcinoma cells

Objective To investigate the inhibitive effect of total alkaloids from Pinellia ternate ( TAPT) on the proliferation of human hepatic carcinoma strain QJY-7703.Methods The proliferation inhibition of human hepatic carcinoma cells treated with TAPT was measured with methyl thiazolil tetracolium colorimetic (MTT) method and plate clone formation assay to detect the inhibitive effect of different concentrations of TAPT on the growth of QJY-7703 strains.Single cell gel electrophoresis (SCGE) was used to detect QJY-7703 cell DNA damage induced by TAPT .Results When compared with the control group , in vitro proliferation of QJY-7703 cells treated with TAPT was decreased significantly , and it was positively correlated with drug dosage and medication time , and statistical sig-nificance could be seen, when comparisons were made (P<0.01,P<0.05).In SCGE detection, significant differences could be seen in cell tail DNA content , tail length and tail movement in the TAPT experimental group , when compared with those of the control group (P<0.01), and a dose-dependent relationship was quite marked .Conclusion In vitro, a certain dosage of TAPT could obviously in-hibit the proliferation of QJY-7703 cells, the mechanism of which might be correlated to DNA damage induced by TAPT .

Research on the relationship between surgical treatment combined with drug therapy and the recovery of hepatic function following tumor resection of primary liver cancer

Objective To explore the relationship between surgical treatment combined with drug therapy and the recovery of hepatic function following tumor resection of primary liver cancer ( PLC) , and to provide reasonable recommendations for clinical treat-ment of the disease .Methods One hundred and twenty cases , which were diagnosed as primary liver cancer and given tumor resection from January 2009 to December 2010 in our hospital were selected and analyzed retrospectively .There were 82 cases that had complete records and could be used for effective analysis .Based on "the China National Formulary"(2010) and the mechanism of drugs , hepa-to-protectants were divided into 6 categories:⑴A=anti-inflammatory drug , B=membrane system stabilizing drug , C=combined de-toxification drug, D=enhanced energy supply drug, E=regeneration promotion drug and F =the category of other drugs.Results (1) When the A+D treatment profile was compared with the C +D treatment profile, the k value of total bilirubin (TB) was P=0.046 and the k value of aspartate aminotransferase (AST) was P=0.035.The C+D treatment profile was superior to that of the A +D treatment profile.(2) Positive correlation could be noted between the k values of ALT and AST (P<0.01).(3) The k value of TB and AST were negatively correlated with the blocking time of portal vein , cirrhosis type and other variables .Conclusion After tumor resection of the primary liver cancer , the C +D treatment profile was superior to the A +D treatment profile in enhancing recovery of hepatic function .

CTA scan of arteria ethmoidalis before functional endoscopic sinus surgery

Objective To make CTA scan of arteria ethmoidalis by 256-ranked spiral CT before FESS , so as to provide evi-dence for surgery.Methods CTA scan of arteria ethmoidalis was made in 40 cases, then, 2-dimentional and 3-dimentional analyses were performed at the work station to distinguish anterior ethmoidal artery from posterior ethmoidal artery , and statistics was then made to ana-lyze their starting points ( O1-O3 segments of ophthalmic artery ) .Then, the distance between anterior ethmoidal artery and anterior nasal spina, and also the angle between the connect line and the nasal base were measured respectively .Results All the 40 cases displayed an-terior ethmoidal artery (75 vessels), 24 cases displayed posterior ethmoidal artery (39 vessels).Seventy anterior ethmoidal arteries star-ted from O3 segment of the ophthalmic artery , accounting for 93.3%, 5 anterior ethmoidal arteries started from ophthalmic artery arch , ac-counting for 6.7%.Twenty-two posterior ethmoidal arteries started from ophthalmic artery arch , accounting for 56.4%.Ten posterior eth-moidal arteries started from O 2 segment of the posterior ophthalmic artery , accounting for 25.6%.Seven posterior ethmoidal arteries star-ted from O3 segment, accounting for 17.9%.Asymmetry could be noted bilaterally in the same patient .The measured distance between the anterior ethmoidal artery and anterior nasal spina was (85.1 ±3.1)mm, and the angle between the connect line and the nasal base was 49.3 ±0.8°.Conclusion Imaging of artery ethmoidalis by the 256-ranked Spiral CT was of high resolution , and related data obtained fa-cilitated a better knowledge of artery ethmoidalis before surgery , and was helpful to the prevention of hematorrhea in FESS .

Application of Doppler color sonography in the diagnosis of branchial cleft cysts

Objective To analyze the features of Doppler color sonography in the diagnosis of branchial cleft cysts and to pro -vide evidence for clinical and differential diagnoses .Methods Retrospective analysis was made on Doppler color sonography images of 10 cases of branchial cleft cysts confirmed by surgery and pathology , then, they were compared with the surgical/pathological results ob-tained.Results In the 10 cases of branchial cleft cysts diagnosed by Doppler color sonography , 9 cases were cystic masses and 1 case was uneven hypoecho solid nodule , with a coincidence rate of 70%between Doppler color sonography diagnosis and surgical /pathologi-cal results obtained .Conclusion In the diagnosis of branchial cleft cysts , Doppler color sonography was an accurate and reliable meth-od, which could tell the relationship between echoes and surrounding tissues .It was an important accessory method for clinical and dif-ferential diagnoses , and surgical excision as well .Besides, it could offer an important indication on the location of branchial fistula , therefore it could provide good evidence for accurate surgical performance .

Causes of laparoscope cholecystectomy converted to open laparotomy

Objective To explore the causes and preventive measures of laparoscope cholecystectomy (LC) converted to open laparotomy.Methods A retrospective analysis of the clinical data in 23 cases (3.79%) from the 607 cases of laparoscope chole-cystectomy conversed to laparotomy from January 2005 to December 2010, was made in our study .Results The conversion was due to following causes:pericholecystic adhesion (11 cases), calot′s triangle unclear (5 cases), cystic artery hemorrhage (2 cases), bile duct lesion (2 cases), anatomic variation (2 cases), carcinoma of gall bladder (1 case).Laparotomy was successfully used in all the cases.Conclusion Full awareness of anatomic variation , good knowledge of LC indications and skillful manipulation were the key to the reduction of converted laparotomy .

Clinical observation on the effect of ranitidine bismuth citrate on peptic ulcer in elderly patients

Objective To evaluate the effect and safety of ranitidine bismuth citrate on gastric ulcer in elderly patients . Methods Fifty-three elderly patients were diagnosed as gastric ulcer by upper gastrointestinal endoscopy and were treated with raniti -dine bismuth citrate, at a dose of 0.35g once, twice a day, for a treatment course of 6 weeks.Close observations were made on the effects and adverse reactions of the treatment .Results Following termination of the treatment course , the patients were given upper gas-trointestinal endoscopy .Results showed that 45 cases were healed , 6 cases were either improved or alleviated , and 2 cases almost re-mained unchanged .Further examination of the last 2 cases revealed that they were carcinoma of stomach .Observation of all the 53 cases indicated that there were no serious adverse reactions in them .Conclusion Treatment with ranitidine bismuth citrate could produce good effect in the elderly patients with gastric ulcer .If regular treatment failed to respond , one should be highly aware that carcinoma of stomach might have developed .

Clinical effects of different methods to improve cervical ripening at the third trimester of pregnancy

Objective To observe the clinical effects of different methods to improve cervical ripening at the third trimester of pregnancy, so as to improve the success rate of labor induction and reduce the cesarean section (CS) rate.Methods One hundred and twenty predelivery pregnant women hospitalized from January 2011 to December 2012 were selected randomly for the study .All the subjects were primiparae , without vaginal delivery contraindications and with signs of induced labor and cervical scores ≤4.Following introduction to different methods of promoting cervical ripening , 66 cases voluntarily selected to use the Cook Medical′s Cervical Ripe-ning Balloon and were assigned as the balloon group .The other 54 cases selected oxytocin for the induction of labor and were assigned as the oxytocin group .Bishop scores , CS rates and infection risks were observed both before and after promotion of cervical ripening , and were compared between the two groups .Results There were significant differences in cervical scores and CS rates between the bal -loon group and the oxytocin group , while no significant differences could be seen in infection rates between the 2 groups (P>0.05). Conclusion Cook Medical′s Cervical Ripening Balloon could produce better effect on cervical ripening than simple intravenous infu -sion of oxytocin .It could increase the success rate of labor induction and decrease the CS rate , but without increase in infection risk .

Diagnosis and treatment of 43 patients with advanced renal tuberculosis

Objective To summarize the diagnostic and therapeutic experience of advanced renal tuberculosis , so as to in-crease diagnostic accuracy and therapeutic efficacy of advanced renal tuberculosis .Methods Clinical data of 43 cases of medium or advanced renal tuberculosis admitted into our Department from January 2005 to January 2012 were analyzed retrospectively .Results Irritation signs of bladder , gross hematuria , and lumbodynia were the most commonly-seen symptoms of advanced renal tuberculosis , with incidence rates being 81.4%, 65.1%, and 44.2% respectively.In laboratory detection, acid-fast stains on urinary sediment seemed to have diagnostic value.Medical scanning indicated that the diagnostic accordance rate of CT was the highest (81.4%), and statistical significance could be noted , when compared with ultrasonic examination or KUB +IVU(P<0.05).However, no statistical significance could be seen, when it was compared with MRU(P>0.05).Five cases underwent purely anti-tuberculosis treatment and 2 of the cases improved during medication , but were deteriorated after drug withdrawal .There was one case of autonephrectomy , and 2 cases failed to respond to anti-tuberculosis treatment .Thirty-eight patients underwent surgical treatment , of whom 34 patients underwent nephrectomy , 4 patients underwent nephroureterectomy with bladder cuff .There were 32 cases of open surgery , of which 6 cases under-went laparoscopic surgery .All the 38 cases were cured .Conclusion Urinary tract CT examination has a high diagnostic value for medi-um or advanced renal tuberculosis .Patients with medium or advanced renal tuberculosis mostly required surgical therapy for final cure of the disease.If the contralateral renal function was good , the affected kidney could be removed .And it was not advisable to perform par-tial nephrectomy or percutaneous catheter drainage of tuberculous abscess .

Surgical treatment of children with deep burns

Objective To explore the clinical features and surgical treatment regimens in the treatment of children with deep burns.Methods A total of 20 cases of deep burns involving 0 to 6-year-old children with a total body surface area ( TBSA) of 3%-45%were admitted in our hospital from 2008 to 2011 and were comprehensively treated with early fluid resuscitation , effective wound surface management , selection of proper surgical regimens and systemic nutritional support .Results After one surgery , wound surface of all the 20 cases healed completely , following surgical treatment .In the 4 cases of electrical injury , skin graft contracture and restric-tion of flexion and extension of nine fingers could obviously be noted;in another 2 cases of condyle recipient site , scar proliferation and skin graft contracture were quite obvious;and in the remaining 12 cases, skin grafts in the limb surface healed quite well after surgery . Conclusion Satisfactory recovery of physiological functions and better outward appearance could be achieved in child patients following effective treatment of wound surface and surgical repair of wound surface coupled with systemic nutritional support .
