Analysis of related risk factors of delayed cerebral vasospasm on patients with subarachnoid hemorrhage

Abstract:
Objective To investigate the related risk factors of delayed cerebral vasospasm (DCVS) on patients with subarachnoid hemorrhage and provide references for prevention and treatment of SAH complicated with DCVS.Methods The clinical data of 80 SAH cases hospitalized from January 2009 to January 2011 were analyzed retrospectively and the related risk factors of SAH complicated with DCVS wereanalyzed.Results Among of 80 patients,40 patients were complicated with DCVS,and the total concurrent rate was 50％(40/80).DCVS concurrate of patients with twice or more bleeding was 83.33％(10/12).DCVS concurrent rate of patients with age ≤ 60 years old was 57.63％(34/59).DCVS concurrent rate of patients with smoking history was 73.33％ (11/15).DCVS concurrent rate of patients with Fisher grade≥ Ⅲ level was 76.19％ (32/42).DCVS concurrent rate of patients with Hunt-Hess grade ≥ Ⅲ level was 66.00％(33/50).DCVS concurrent rate of patients with white blood cell count ≥ 15 × 109/L was 76.47％( 13/17).DCVS concurrent rate of patients with time duration from onset to operation (time window) ＞ 24 h was 91.30％ (21/23 ).All the values had significant differences compared with those of patients with less than twice bleeding,age ＞ 60 years old,no smoking,Fisher grade Ⅰ - Ⅱ level,Hunt-Hess grade Ⅰ - Ⅱ level,white blood cell count ＜ 15 × 109/L and time window ≤24 h [44.12％( 30/68),28.57％(6/21 ),44.62％(29/65),21.05％(8/38),23.33％(7/30),42.86％(27/63),33.33％(19/57)](P＜0.05 or ＜0.01 ).DCVS concurrent rate of patients with different gender,with or without hyperglycemia and hypertension history had no significant difference (P ＞0.05).Conclusions The risk factors of SAH complicated with DCVS are bleeding more than twice,age ≤60 years old,with smoking history,Fisher grade ≥ Ⅲ level,Hunt-Hess grade ≥ Ⅲ level,white blood cell count ≥ 15 × 109/L and longer time window.
