Abstract:
Diabetes mellitus, as a metabolic disease that poses a serious threat to human health, imposes a heavy burden on society. Research on its pathogenesis and treatment strategies aligns with national health strategies. Islet transplantation holds great promise for functionally curing diabetes mellitus, but traditional islet transplantation is limited by challenges such as donor shortage and early graft loss, limiting its widespread adoption. In recent years, stem cell-derived therapies have become a research focus. Technologies such as chemically induced pluripotent stem cells, mesenchymal stem cells, and islet autotransplantation from adult pancreatic progenitor cells address the shortage of donor sources. Meanwhile, the application of new transplantation sites, such as underneath the abdominal anterior rectus sheath, provides greater space and a microenvironment conducive to vascularization for the graft, significantly reducing the risk of early loss and improving functional stability. For immune rejection, new technologies such as islet encapsulation and co-transplantation of immune regulatory cells and islets have also demonstrated potential to reduce immune attacks and improve therapeutic efficacy. This review systematically discusses the latest advances in islet transplantation for diabetes mellitus treatment from three perspectives: transplantation sources, transplantation sites, and immune regulatory mechanisms.
