Abstract:
Objective To evaluate the efficacy of ibrutinib in patients with mantle cell lymphoma (MCL) and chronic lymphocytic leukemia (CLL)/small lymphocytic lymphoma (SLL), and assess the risk of cardiovascular adverse events. Methods PubMed, the Cochrane Library, Web of Science, and ClinicalTrials.gov were searched for studies on ibrutinib in lymphoma till December 2025. Hazard ratios (HRs) with 95% confidence intervals (CIs), relative risks (RRs), or odds ratios (ORs) with 95% CIs were used to compare progression-free survival (PFS) and overall survival (OS) between the ibrutinib group and the control group and evaluate the risk of ibrutinib-related adverse events, including hypertension and atrial fibrillation. Results A total of 10 studies involving 3 190 patients were included, with 1 620 patients in ibrutinib group and 1 570 patients in control group. Compared with the ibrutinib group, the control group had a significantly higher risk of PFS events (HR=2.93, 95% CI 2.36-3.64, P<0.01) and OS events (HR=1.64, 95% CI 1.41-1.90, P<0.01), indicating that PFS and OS were significantly better in the ibrutinib group than in the control group. The objective response rate (ORR) was significantly higher in the ibrutinib group than in the control group (RR=3.68, 95% CI 1.61-8.40, P=0.002). In addition, the ibrutinib group had a higher risk of hypertension (RR=2.29, 95% CI 1.69-3.10, P<0.01) and atrial fibrillation (RR=4.92, 95% CI 3.10-7.80, P<0.01). Conclusion Ibrutinib significantly improved prognosis in patients with MCL and CLL/SLL, but it was associated with significantly increased risks of hypertension and atrial fibrillation.
