Abstract:
OBJECTIVE To investigate the efficacy and safety of ropivacaine in transversus thoracis muscle plane block (TTMP) combined with pecto-intercostal fascial plane block (PIFB) for postoperative analgesia in median sternotomy.METHODS A total of 70 patients who underwent median sternotomy at the Central Hospital of Enshi Tujia and Miao Autonomous Prefecture, from December 2025 to April 2026 were enrolled and randomly divided into the TTMP group (group T, n =35) and the TTMP combined with PIFB group (group TP, n =35). Both groups received 40 mL of 0.3% ropivacaine under ultrasound guidance. group TP received bilateral TTMP combined with PIFB, while group T received bilateral TTMP alone. The total consumption of sufentanil within 48 hours postoperatively, analgesic indicators (intraoperative total sufentanil consumption, time to first patient-controlled analgesia pump press, number of effective analgesia pump presses), recovery indicators (postoperative extubation time, length of ICU stay, time to first flatus, time to postoperative drainage tube removal, total length of hospital stay), circulatory indicators (heart rate and mean arterial pressure at different time points), postoperative pain scores [Visual Analog Scale (VAS) scores at rest and during movement at different time points], and the incidence of adverse reactions (nausea and vomiting, CO 2 retention, delirium, respiratory tract infection) were compared between the two groups.RESULTS One patient in each group withdrew from the study. The total consumption of sufentanil within 48 hours postoperatively, all analgesic indicators, all recovery indicators, and VAS scores at rest and during movement in group TP were significantly better than those in group T ( P <0.05). There were no statistically significant differences in circulatory indicators or the incidence of most adverse reactions between the two groups ( P >0.05), but the incidence of postoperative CO 2 retention in group TP was significantly lower than that in group T ( P <0.05).CONCLUSIONS Compared with TTMP alone, ropivacaine in TTMP combined with PIFB provides effective analgesia, promotes early recovery, and has good safety in patients undergoing median sternotomy.
