Abstract:
OBJECTIVE To investigate the clinical efficacy and renoprotective effects of finerenone combined with Shenkangning capsules in diabetic kidney disease (DKD) patients with spleen-kidney yang deficiency and blood stasis syndrome, and to evaluate the safety.METHODS Clinical data of 150 patients with DKD conforming to the traditional Chinese medicine (TCM) syndrome differentiation of spleen-kidney yang deficiency and blood stasis syndrome were retrospectively collected from the departments of nephrology and endocrinology of our hospital from May 2024 to August 2025. Based on the treatment regimen received, patients were divided into a control group ( n =74) and an observation group ( n =76). All patients in both groups received standardized treatment for DKD. In addition, the control group received Finerenone tablets, whereas the observation group received Shenkangning capsules combined with the same regimen as the control group. Both groups were treated continuously for 3 months. The clinical efficacy was evaluated and compared between the two groups after the 3-month treatment period. Additionally, TCM syndrome score (TCMSS), renal function parameters [serum creatinine (Scr), cystatin C (CysC), and β 2 -microglobulin (β 2 -MG)], proteinuria markers[urinary albumin-to-creatinine ratio (UACR), 24 hour urinary total protein (24 hUTP)], inflammatory markers[high-sensitivity C-reactive protein (hs-CRP), interleukin-6 (IL-6)] before and after 3 months of continuous treatment as well as the incidence of adverse reactions were compared between the two groups.RESULTS After 3 months of treatment, the total effective rate in the observation group was significantly higher than in the control group ( P <0.05). In both groups, the TCMSS, Scr, CysC, β 2 -MG, UACR, 24 hUTP, hs-CRP, and IL-6 levels were significantly lower than those before treatment in the same group ( P <0.05), and all the above indicators in the observation group were significantly lower than those in the control group ( P <0.05). There was no statistically significant difference in the overall incidence of adverse reactions between the two groups ( P >0.05).CONCLUSIONS The combination of finerenone and Shenkangning capsules improved TCM syndromes, reduced proteinuria, and improved renal function and inflammation-related indicators in DKD patients with spleen-kidney yang deficiency and blood stasis syndrome, without increasing the risk of adverse reactions.
