Status quo and influencing factors of cognitive frailty in elderly patients with hypertension

Abstract:
Objective To investigate the status quo of cognitive frailty in elderly patients with hypertension, and to analyze its influencing factors. Methods Three hundred and fifty elderly patients with hypertension in Nantong Third People’s Hospital from December 2023 to December 2025 were consecutively enrolled. The data were collected by baseline data questionnaire, Fried frailty degree scale and mini-mental state examination. The above patients were classified into 131 cases in cognitive frailty group and 219 cases in non-cognitive frailty group. The influencing factors were evaluated by binary logistic regression analysis, and model fitting and collinearity diagnosis were performed. Results Results showed that among 350 elderly hypertensive patients, the incidence rate of cognitive frailty was 37.43%. Cognitive frailty group had higher proportions of patients aged≥75 years, female gender, hypertension duration≥30 years, comorbidities, moderate/poor medication adherence, daily exercise duration<0.5 hours, vitamin D deficiency, depressive symptoms and sleep disorders (P<0.05). Model fitting and collinearity diagnosis suggested that the results were reliable (Hosmer-Lemeshow χ2=8.254, P=0.409>0.05, variance inflation factor<2). Binary logistic regression analysis discovered that age≥75 years old, hypertension duration>30 years, moderate/poor medication adherence, daily exercise duration<0.5 hours and depressive symptoms were risk factors leading to cognitive frailty (P<0.05). Conclusion In elderly patients with hypertension, the incidence rate of cognitive frailty is relatively high, and advanced age, long duration of hypertension, moderate/poor medication adherence, short daily exercise duration and depressive symptoms are independent risk factors.
