Characteristics of diabetic retinopathy in community-dwelling patients with type 2 diabetes mellitus

Abstract:
Objective To investigate the prevalence, fundus characteristics, and risk factors of diabetic retinopathy (DR) in elderly community-dwelling patients with type 2 diabetes mellitus in Changning District, Shanghai. Methods A cross-sectional study was conducted. A total of 308 elderly patients with type 2 diabetes were enrolled using a simple random sampling method. Data were collected via questionnaires and non-mydriatic fundus photography. The chi-square test, t-test, multiple imputation method, and binary logistic regression model were used for data analysis. A two-step cluster analysis based on fundus characteristics was performed to identify subtypes. Results The overall prevalence of DR was 21.36% (66/308), with non-proliferative diabetic retinopathy (NPDR) accounting for 90.91%. Cluster analysis identified four distinct DR subtypes: ischemic, exudative, hemorrhagic, and proliferative. Multivariate analysis revealed that stroke (OR=2.712, 95% CI: 1.192-6.167), glycosylated hemoglobin level (OR=1.475, 95% CI: 1.169-1.860), and a longer duration of diabetes (OR=1.035, 95% CI: 1.005-1.066) were risk factors for DR, whereas good self-care ability (OR=0.379, 95% CI: 0.151–0.949) was a protective factor. Conclusion The prevalence of DR is relatively high among community-dwelling elderly patients with type 2 diabetes and is closely associated with stroke, glycemic control, and diabetes duration. The classification system based on fundus image characteristics provides a new scientific basis for early stratified management and precise prevention of DR.
