BACKGROUND: Medial medullary infarction is a rare entity of cerebrovascular events and the bilateral involvement of the medial medulla is much more infrequent. The characteristic presentations of a medical medullary infarction are the presence of contralateral hemiplegia sparing the face, contralateral sensory loss and ipsilateral hypoglossal paralysis while the presence of quadriparesis is the hallmark of the bilateral medical medulla infarction. Atherosclerosis of the vertebral artery is the most common stroke mechanism.

CASE: We present a case of a Filipino who presented with quadriplegia and was diagnosed with bilateral medial medullary infarction and was demonstrated by magnetic resonance imaging. Likewise, we reviewed the literature of the reported cases of bilateral medial medullary infarction.
