Leiomyosarcoma of the vagina is extremely rare. Incidence occurs in 2%-3% of the uncommon vaginal malignancies. Its timely diagnosis is crucial since any delay would preclude treatment usually leading to advanced grade of the leiomyosarcoma. We are presented with a case of a 40 year old. G1P1 (1001) with a 5 month history of vaginal bleeding associated with anorexia, weight loss and a 5cm x 3cm solid, firm mass at the left posterolateral wall of the vagina. Diagnostic examinations including CT scan, colposcopy, immunostaining were done to rule out differentials for a solid mass in the vaginal canal. She underwent wide excision of the mass and repair  of the rectovaginal defect. Histopathologic evidence showed high grade sarcoma particularly a leiomyosarcoma. Postoperatively, she regained her sexual function, restored her lower genital tract anatomy and reaffirmed her self esteem. Close follow-up with radiotherapy was then recommended as the individualized management in this rare type of malignancy.
