A 76 year-old female, single, nulligravid was admitted for abdominal enlargement. Exploratory laparotomy, right oophorectomy, biopsy of omentum and uterine serosal adhesions, lymph node evaluation, and appendectomy were performed. A mucinous material filling the peritoneal cavity was noted. The right ovary was converted to a biloculated cystic mass. The histopathologic studies revealed mucinous carcinoma, involving the appendix, right ovary,uterine serosa and omentum. The patient underwent six cycles of carboplatin paclitaxel with the plan of interval tumor debulking after. The abdominal enlargement recurred one year after the first surgery.;A slide review revealed mucinous tumor of flow malignant potential, right ovary,mucinous cystadenoma, appendix, disseminated peritoneal adenomucinosis. The patient underwent exploratory laparotomy, evacuation of mucin, total hysterectomy, left salpingoophorectomy, tumor debulking, infragastric omentectomy, and peritonectomy. Intraoperatively,mucinous fluid was noted within the pelvic cavity. A multiloculated multiseptated cystic mass occupied the right adnexal area containing mucinous fluid. The final histopathology report was mucinous tumor of low malignant potential (recurrent), ovary (right). Disseminated peritoneal adenomucinosis; ovary (left), parametria (left),peritoneum and omentum; dissecting mucinosis; pelvic and abdominal peritoneum. The patient was on regular follow-up at the Cancer Institute. She completed five cycles of capecitabine. She has no evidence of disease.Peritonectomy is a procedure that is seldom performed in the field of gynecology, as compared to general surgery.In one large institution, only four peritonectomy procedures were done in one year. This surgical procedure is yet an uncommon procedure for gynecologic cases.
