Role of ultrasound with doppler in the diagnosis of placenta accreta: A meta-analysis.

Abstract:
Objective: To know the role of transvaginal and transabdominal ultrasound with or without Doppler in the presumptive diagnosis of placenta accreta. This study reports on the frequent findings strongly associated with the presence of placenta accreta. Methods: The Medline,Pubmed,Ovid,Science Directs and Cochrane Library (1990-2005) databases were searched for relevant studies, published in English. Those studies fulfilling the pre-defined criteria were included. Each study was re-analyzed using odds ratio, specificity and sensitivity with the ultrasound findings used. Results: Six relevant foreign studies were identified. Atotal subject pooled size of 2,374 women in their 2nd and 3rd trimesters of pregnancy were included in the studp>A previous cesarean section was the most cited risk factor. Among all ultrasound findings, at least 3 criteria or either presence of hypoechoic interface between the uterine serosa and bladder as well as loss of venous flow on the placenta! margin and presence of exophytic mass as the most associated findings with placenta accreta. Pooled odds ratio is 3.6 (95 percent Cl 2.67,4.9).
