Carcinoma cuniculatum or verrucous carcinoma is an uncommon variant of squamous cell carcinoma. Its exact incidence is unknown. Literature search shows that there is no published report of this form of squamous cell carcinoma in local journals. This type of carcinoma occurs on the oral cavity, larynx, genitalia, skin and esophagus. The tumor has minimal metastatic potential. We report the case of an 84-year-old man with a six-month history of solitary erythematous plaque with ulceration extruding hemopurulent discharge on the right scrotal area and multiple erythematous plaques, some with slightly verrucous surfaces on the medial aspect of the right thigh, which rapidly enlarged. Histologic examination is compatible with squamous cell carcinoma. Final diagnosis with clinicopathologic correlation is carcinoma cuniculatum. Radiotherapy was started; however, patient expired. The importance of early diagnosis and treatment is emphasized to prevent mortality from verrucous carcinoma.
