The prevalence of polycystic ovary syndrome phenotypes and its association to infertility, abortion, and cardiometabolic risks: A review of cases from January 2014 to December 2015 in the out-patient department of two tertiary institutions in Metro Manila.

Abstract:
Methodology: An outpatient chart review was done in 2 tertiary institutions in Metro Manila. Purposive sampling was done. PCOS cases from January 2014 to December 2015 were classified into four phenotypes. One-way ANOVA was used to assess the association of phenotypes with infertility, abortion and cardiometabolic risks using absolute values.
Results: PCOS prevalence was 12.29%. There were 195 cases eligible for the study. Phenotype D was the most common phenotype followed by Phenotype A. The least common phenotypes were Phenotypes B and C. More than half were obese with 58% exhibiting central obesity. Analysis of variance showed significant association of Phenotype A to hyperandrogenism (p value < 0.001), hypertension (p value 0.002), impaired glucose tolerance (p vlaue 0.009), diabetes mellitus (p value 0.009) and metabolic syndrome (p value < 0.001). Phenotype D was significantly associated with elevated triglycerides (p vlaue 0.049), LDL (p value 0.014) and VLDL (p value 0.015) and reduced HDL levels (p value 0.006). There was no significant association of phenotypes with infetility (p value 0.22), abortion (p vlaue 0.90), abnormal BMI (p value 0.27) and abdominal obesity (p value 0.115).
Conclusion: Phenotype A was associated with hyperandrogenism, hypertension, impaired glucose tolerance, diabetes mellitus and metabolic syndrome, while Phenotype D was associated with dyslipidemia. Hence, knowledge of PCOS phenotype classification is needed to implement comprehensive therapeutic approach for PCOS patients to prevent early and long-term complications.
