Abstract:
Objective The treatment of calyceal atresia with calculi is challenging as traditional surgical approaches are highly invasive.This article reports a case successfully treated with flexible ureteroscopy, so as to provide reference for clinical practice.Case Report A 58-year-old female patient with a history of extracorporeal shock wave lithotripsy(ESWL)was admitted due to “recurrent right lumbodorsal distending pain for 5 days”.Computed tomography(CT)revealed a 1.1 cm× 0.8 cm calculus in the upper segment of the right ureter and a 1.1 cm×0.4 cm calculus in the right upper calyx, complicated with mild hydronephrosis of the right upper calyx and local renal cortical thinning.Under general anesthesia, flexible ureteroscopy was performed to explore and remove the upper ureteral calculus.The most fragile part of the calyceal neck in the calyceal atresia was incised with holmium laser, followed by the insertion of a ureteral guidewire.The calculus was then fragmented and removed.The procedure was smooth with minimal intraoperative bleeding.A follow-up CT scan on the second postoperative day confirmed complete removal of calculi and unobstructed drainage of the upper calix.Postoperative follow-up showed resolution of lumbar pain, and the patient had no complications or recurrence.Conclusion Ureteroscopic incision and lithotripsy is an effective minimally invasive treatment for upper calyceal atresia with calculi, which can simultaneously relieve obstruction and remove calculi.
