Comparison of Clinical Outcomes Based on the Timing of Reoperation in Patients with Recurrent Mild Exotropia Following Initial Strabismus Surgery

Abstract:
Objective:To compare the natural courses and surgical outcomes in recurred intermittent exotropia (IXT) patients with partial fusion after primary surgery, to provide appropriate follow-up strategies and guidelines for determination of reoperation.
Methods:We performed a retrospective chart review of 78 patients hospitalized between January 2005 and December 2015 who had shown 10–20 prism diopter (PD) IXT after 1 year of primary surgery and had minimum postoperative follow-up of 2 years after initial surgery, 6 months after reoperation.
Results:Thirty-eight of 78 patients underwent reoperation and 40 patients followed up without reoperation. The mean age at second surgery was 4.5 years, 2 years younger and the follow-up period was 2 years longer than observation group. Preoperative deviation at distance was 31. PD and 28.9 PD, about 2.5 PD larger than observation group. The risk factors for reoperation were younger age at first surgery, longer duration period and greater preoperative deviation. There was bimodal peak at 2nd and 5th years of reoperation. Based on the bimodal peak distributions, the patients were divided into early and late reoperation group based on 40 months. There was no difference between the two groups in final outcome.
Conclusion:About half of the patients with mild recur after primary surgery underwent reoperation. If the age was young and angle of deviation was large at primary surgery, the possibility of reoperation was high. The reoperation period did not affect outcomes at final follow-up, and most patients maintained stable state under 10 prism angles of deviation.
