Correlation of CEAP Clinical Class, Sex, Age, Body Mass Index, and Reflux Patterns in Patients with Chronic Venous Disease Based on the 2020 Update of the CEAP Classification System

Abstract:
Objective:This study aims to analyze the association between clinical classes in the newly updated CEAP classification in 2020 and age, sex, body mass index, and patterns of venous valve failure.
Methods:This is a retrospective cross-sectional observational study of patients with symptomatic clinical grade C1 or higher in the lower extremities who underwent lower extremity venous ultrasound between May 2022 and March 2024. Patients with a history of previous lower extremity venous procedures and those without ultrasound evidence of saphenous vein reflux were excluded.
Results:A total of 244 patients (327 lower limbs) were studied, with a male-to-female sex ratio of 1:2.5. The mean age was 56.9 years (23–84), and the mean body mass index (BMI) was 24.5 kg/m 2 . The distribution of CEAP clinical classes was 78 as C1 (24%), 222 as C2 (68%), 4 as C3 (1%), 11 as C4a (2.7%), 2 as C4b (0.6%), and 10 as C4c (3.7%). There were no C5 and C6 lower limbs during the study period. Comparing C1–C3 lower limbs to C4 lower limbs, C4 lower limbs had a higher prevalence in men, a significantly higher BMI, and a substantially higher prevalence of deep venous reflux.
Conclusion:Adding C4c to the C4 subgroup for comparison, clinical grade C4 limbs had a higher prevalence of males, significantly higher BMI, and higher prevalence of deep venous reflux than C1–C3. Further research is needed to include C0–C6 in future studies.
