Changes in High-order Aberrations after Implantation of Enhanced Intraocular Lens in Patients with Retinal Diseases

Abstract:
Purpose:To evaluate clinical outcomes including changes in high-order aberrations (HOAs), after cataract surgery using a monofocal extended-depth-of-focus (EDoF) intraocular lens (IOL) in patients with retinal diseases.
Methods:A total of 29 eyes of 26 patients with cataract and retinal disease underwent cataract surgery using a monofocal EDoF IOL (TECNIS Eyhance® ICB00). The improvement in high-order aberrations, visual discomfort, and intermediate visual acuity after the surgery were investigated.
Results:The total ocular HOA, internal HOA, and coma aberration were significantly reduced from 0.390 ± 0.225 μm, 0.346 ± 0.275 μm, and 0.228 ± 0.184 μm, respectively before the surgery, to 0.214 ± 0.108 μm, 0.218 ± 0.111 μm, and 0.101 ± 0.076 μm 3 months after the surgery. Trefoil and spherical aberrations were reduced from 0.167 ± 0.109 μm, 0.046 ± 0.155 μm to 0.107 ± 0.091 μm, 0.003 ± 0.055 μm respectively after the surgery, but not statistically significant (p = 0.059, p = 0.154). In the analysis of potential visual complaints, Blur/ Double vision, Glare/Halo and Starburst score were 1.35 ± 1.23, 0.72 ± 1.00 and 1.21 ± 1.18 preoperatively, and 0.41 ± 0.87, 0.17 ± 0.38, and 0.52 ± 0.74 postoperatively. They all showed statistically significant decreases (p < 0.001, p = 0.011, p = 0.024). At 3 months after surgery, intermediate visual acuity converted to logarithm of the minimal angle of resolution (LogMAR) was LogMAR 0.29 ± 0.31 and the median visual acuity was 0.19.
Conclusions:In patients with cataracts combined with retinal disease, a significant reduction in HOAs and visual discomfort were observed after cataract surgery using monofocal EDoF IOLs.
