Analysis of influencing factors of early activity in patients with mechanical ventilation based on theoretical domain framework

Abstract:
Objective:Based on the theoretical domain framework, the factors that affect early ambulation in patients with mechanical ventilation were analyzed to provide evidence for improving the rehabilitation effect of patients.Methods:The mixed method of systematic review and qualitative study were adopted. Studies about factors affecting early activity of patients with mechanical ventilation searched from Cochrane Library, Embase, PubMed, Web of Science, VIP database, Wanfang Database, CNKI and SinoMed published from inception to March 19, 2024. Following literature screening and data extraction, Nvivo11 qualitative research software, content analysis and rooted theory were used to encode the text data at three levels, and the logical relationship of influencing factors were linked.Results:A total of 24 articles were selected, 351 coding elements were encoded into 14 theoretical domains, 35 secondary nodes and 42 three-level nodes were concluded. The number of coding points involving "environmental context and resources" theory domain is the largest (115 coding points), followed by the "social influences" theory domain (42 coding points). Pareto diagram was used to order and analyze the frequency of influencing factors. Patients′ physical condition (45 times) and occupational role characteristics (22 times) were the most influential single factors on early activity.Conclusions:The influencing factors of early ambulation of patients with mechanical ventilation involve all theoretical domains, and are both pluralism, blending and complex.
