Preoperative ultrasonographic features for predicting the risk of lateral neck lymph node metastasis in thyroid cancer

Abstract:
Objective To explore the relationship between preoperative ultrasonographic features of cervical lymph nodes and lateral neck lymph node metastasis in thyroid cancer by analyzing these characteristics.Methods A total of 296 patients with thyroid cancer who visited the Second Affiliated Hospital of Anhui Medical University from January 1,2018 to August 1,2024 were selected.Patients with pathological positive results after cervical lymph node surgery were included in observation group(260 cases),while patients with pathological negative results were included in control group(36 cases).Analyze the ultrasound imaging characteristics of suspected lymph nodes before surgery and explore the influencing factors of lymph node metastasis in the cervical region of thyroid cancer.Results The results of univariate analysis showed that there was a correlation(P<0.05)between the size and frequency of suspicious lymph nodes,abnormal boundaries between the cortex and medulla,presence of microcalcifications,irregular shapes,and abnormal blood flow signals and lateral cervical lymph node metastasis.Multivariate Logistic regression analysis showed that ultrasound findings of suspicious lymph node microcalcifications,presence of hyperechoic masses,and presence of abnormal blood flow signals were independent risk factors for cervical lymph node metastasis(P<0.05).Conclusion The preoperative ultrasound imaging features of cervical lymph nodes in thyroid cancer have a good predictive effect on whether lymph nodes have metastasized.
