Abstract:
Thyroid storm is one of the most severe complications that patients with hyperthyroidism may face, often triggered by surgery and other physiological stressors. Preoperative optimization of thyroid function is considered crucial in preventing thyroid storm. However, a recent cohort study of 275 patients, led by Fazendin J, suggested that surgery may be safely performed without achieving a euthyroid state beforehand or administering Lugol′s indine solution or antithyroid drugs. This conclusion has sparked considerable debate within the medical community. In this article, we examine current strategies for preoperative preparation in hyperthyroid surgery, explore ongoing controversies, and review the mechanisms by which various pharmacological interventions help prevent thyroid storm.
