Clinical efficacy of hyperbaric oxygen combined with labetalol in the treatment of gestational hypertension and its effect on vascular endothelial function and pregnancy outcomes

Abstract:
Objective To explore the clinical efficacy of hyperbaric oxygen ( HBO ) combined with labetalol in the treatment of gestational hypertension and its effect on vascular endothelial function and pregnancy outcomes. Methods Ninety pregnant women with gestational hypertension admitted to the Department of Obstetrics and Gynecology in the General Hospital of Lanzhou from June 2016 to June 2018 were selected as study subjects, and were randomly divided into the control group (n=45) and the observation group (n=45). The control group was treated with labetalol, while the observation group received HBO in addition to the treatment received by the control group. The changes in blood pressure and endothelial function of the two groups were closely observed before and after treatment. The pregnancy outcomes including cesarean section, postpartum hemorrhage, premature rupture of membrane, fetal respiratory distress, fetal growth restriction and neonatal asphyxia were compared between the 2 groups. Results After treatment, the diastolic blood pressure (DBP) and systolic blood pressure (SBP) of the observation group [(84. 4 ± 6. 2) mmHg,(133. 3 ± 11. 2) mmHg] were significantly lower as compared with those of the control group [(90. 0 ± 6. 3) mmHg,(140. 7 ± 10. 4) mmHg] (all P<0. 05). Total efficacy of the observation group was 95. 6%, which was significantly higher than 82. 2% of the control group (P <0. 05). NO and ET-1 levels in the observation group were obviously better than those of the control group (all P <0. 05). The rates of cesarean section, postpartum hemorrhage, premature rupture of membrane, fetal respiratory distress, fetal growth restriction and neonatal asphyxia in the observation group were significantly lower than those of the control group, and statistical significance could be seen when comparisons were made between them (P <0. 05). Conclusion HBO combined with labetalol in the treatment of gestational hypertension could effectively reduce patient blood pressure, improve endothelial dysfunction, and reduce the rate of adverse pregnancy outcomes.
