Factors and pathways influencing health-related quality of life in patients undergoing bladder perfusion chemotherapy

Abstract:
Objective:To analyze the potential synergistic mechanisms among the influencing factors of health-related quality of life (HRQoL) in patients undergoing bladder perfusion chemotherapy by constructing the structural equation model, so as to provide a reference for the clinical development of multifaceted targeted intervention strategies.Methods:Convenience sampling was used to select 260 outpatient bladder perfusion chemotherapy patients from the Shanghai Jiao Tong University School of Medicine Affiliated Shanghai General Hospital and Jiangqiao Hospital, Shanghai Jiao Tong University School of Medicine Affiliated Shanghai General Hospital from January 2022 to October 2023 as survey subjects. General Information Questionnaire, Health Literacy Management Scale, European Organization for Research and Treatment of Cancer Core Quality of Life Questionnaire-30 (EORTC QLQ-C30), Fear of Progression Questionnaire-Short Form, Social Support Rating Scale, and Strategies Used by People to Promote Health were used to collect and analyze the data. Structural equation model was constructed by AMOS 22.0 software.Results:A total of 249 valid questionnaires were collected, with a valid recovery rate of 95.77% (249/260). The constructed structural equation model fitted well, parsimony goodness-of-fit index of 0.644, adjusted goodness-of-fit index of 0.917, root mean square of approximation error of 0.072, value-added fit index of 0.955, canonical fit index of 0.942, comparative fit index of 0.955. Path effects analyses showed that health literacy, social support, and self-efficacy directly and positively influenced HRQoL ( β=0.208, 0.128, and 0.187), and health literacy indirectly and positively influenced HRQoL through the mediation of social support and self-efficacy ( β=0.191). Self-efficacy directly and negatively influenced fear of progression ( β=-0.440), fear of progression directly and negatively affected HRQoL ( β=-0.364), and self-efficacy indirectly positively affected HRQoL through fear of progression ( β=0.166) . Conclusions:HRQoL in patients undergoing bladder perfusion chemotherapy is influenced by a number of factors, the main positive influencing factor is health literacy. Healthcare professionals should rationally match patients' coping needs after adequate assessment of patients' clinical characteristics, eliminate factors affecting HRQoL, and improve patients' quality of life in a multifaceted and progressive manner.
