Abstract:
Objective:To evaluate the efficacy and safety of glucagon-like peptide-1 receptor agonists (GLP-1RAs) in kidney transplant recipients. Methods:We conducted a retrospective analysis of 100 kidney transplant recipients at the Second People’s Hospital of Shanxi Province with long-term follow-up. 20 patients received GLP-1RA as the treatment group and 80 patients were selected as the control group using the 1∶4 frequency matching method. 13 clinical indicators from the two groups were collected during the same period, and statistical software R language was used to observe the differences between the two groups in body weight, transplant kidney function, blood drug concentration, glucose metabolism, lipid metabolism, bone metabolism, safety, and other aspects. Results:There were significant differences in body weight, body mass index (BMI), serum creatinine (Scr), fasting blood glucose (FBG), glycated hemoglobin (HbA1c), cholesterol (TC), and low-density lipoprotein (LDL) between the two groups ( P<0.05). Among the 20 kidney transplant recipients treated with GLP-1RA for 3 months, we observed a decrease in body weight of (4.8±3.03) kg, BMI by (1.61±1.02) kg/m 2, Scr by (4.7±9.21) μmol/L, FBG by (1.66±0.66) mmol/L, HbA1c by (1.57 ±0.91) %, TC by (0.44±0.82) mmol/L, and LDL by (0.36±0.66) mmol/L. Only one patient experienced significant appetite loss, while severe hypotension was reported in another case; however, all symptoms resolved following a dose reduction. Conclusions:GLP-1RA demonstrates good efficacy and safety in kidney transplant recipients, effectively controlling weight and improving glucose and lipid metabolism. Short-term observation of renal transplantation and stable blood concentration of tacrolimus does not increase the risk of renal transplantation rejection. Mild to moderate gastrointestinal adverse reactions are common, but most of them can be tolerated. Further evaluation of bone metabolism, cardiovascular events, proteinuria, and long-term survival of grafts and renal recipients remains to be seen.
