Classification and influencing factors of decision-making regret of surrogate decision makers in acute ischemic stroke patients

Abstract:
Objective:To explore the classification and influencing factors of decision-making regret of surrogate decision makers in acute ischemic stroke patients, to provide reference for improving the decision-making quality of surrogate decision makers.Methods:This was a cross-sectional study, surrogate decision makers of acute ischemic stroke patients admitted from October 2022 to October 2023 of Zhuji People′ s Hospital of Zhejiang Province were enrolled as the research objects by convenient sampling method. The general information questionnaire, Decision Regret Scale, Control Preferences Scale, Decision Conflict Scale, and Wake Forest Physician Trust Scale were used for investigation. Latent class analysis was used to classify surrogate decision makers according to decision-making regret, and multi-Logistic regression was used to analyze the influencing factors of the potential category.Results:A total of 154 surrogate decision makers of patients were included in the present study, 96 males and 58 females with 30-69(48.12 ± 10.09)years. The scores of Decision Regret Scale were (28.04 ± 4.49). The decision-making regret in surrogate decision makers of patients with acute ischemic stroke was divided into 3 latent classes, namely "low regret-wise decision making type" accounted for 27.9% (43/154), "decision regret-harmful type" accounted for 50.0% (77/154), "severe regret type" accounted for 22.1% (34/154). Taking severe regret type as reference, new rural cooperative medical scheme [ β=-2.757, OR=3.060(2.995-4.851), P<0.05], male surrogate decision maker [ β=2.330, OR=0.278(0.025-0.709), P<0.05], surrogate decision maker age [ β=0.207, OR=0.813(0.669-0.974), P<0.05], bachelor degree and above [ β=3.091, OR=0.045(0.003-0.710), P<0.05], share responsibility with the doctor [ β=1.349, OR=0.259(0.025-0.709), P<0.05], Wake Forest Physician Trust Scale scores [ β=0.184, OR=1.202(1.065-1.358), P<0.05] were the influencing factors for low regret-wise decision making type; new rural cooperative medical scheme [ β=-1.728, OR=1.754(1.630-2.074), P<0.05], male surrogate decision maker [ β=1.567, OR=1.791(1.652-3.895), P<0.05] were the influencing factors for decision regret-harmful type. Conclusions:There are 3 latent classes of decision-making regret in surrogate decision makers of acute ischemic stroke patients, medical staff can alleviate the decision making regret of surrogate decision makers through targeted intervention measures.
