Abstract:
Objective To explore the relationship between diffusion kurtosis imaging(DKI)based on MR and macrophage inflammatory protein 1 alpha(MIP-1α)and the prognosis of patient with Parkinson's disease, with constructing a prognostic model. Methods A total of 128 Parkinson's disease patients were selected and randomly divided into training set(n=102)and validation set(n=26)according to the ratio of 8∶2.After one year of treatment with oral levodopa drugs combined with deep brain stimula-tion of subthalamic nucleus, all patients were divided into good prognosis and poor prognosis groups. The serum MIP-1α and MR DKI parameters were compared between the two groups to screen for factors affecting poor prognosis of Parkinson's disease, and a risk prediction model was constructed and validated. Results Among 128 Parkinson's disease patients,53 had poor prognosis, including 44 in the training set and 9 in the validation set.The fractional anisotropy(FA)and mean kurtosis(MK)of the left and right globus pallidus,mini-mental state examination(MMSE)score in the poor prognosis group were significantly lower than those in the good prognosis group, while serum MIP-1α in the poor prognosis group was significantly higher than that in the good prognosis group(P<0.05).Left globus pallidus FA,left globus pallidus MK,right globus pallidus FA,right globus pallidus MK,MMSE score, and serum MIP-1α were the factors affecting poor prognosis in Parkinson's disease patients(P<0.05).The sensitivity, specificity, and area under the curve(AUG)of the nomogram model for predicting poor prognosis in Parkinson's disease patients in the training and validation sets were 0.823 and 0.749,0.751 and 0.764,0.821 and 0.802,respectively.Conclusion FA and MK of the left globus pallidus,FA and MK of the right globus pallidus, and serum MIP-1α are associated with poor prognosis in Parkinson's disease patients.Constructing a risk model can help identify the risk of poor prognosis in the early stage.
