Clinical Characteristics of Autoimmune Gastritis Patients Complicated With Pernicious Anemia

Abstract:
Background:Autoimmune gastritis(AIG),used to be called pernicious anemia(PA),is actually a disease with different characteristics at different stages leading to conceptual confusion.In fact,only a proportion of patients with AIG develop PA.In the last decade,there have been few studies on AIG and PA.Aims:To investigate the clinical characteristics of AIG complicated with PA,so as to improve the understanding and diagnosis of this disease.Methods:A total of 33 AIG patients complicated with PA admitted to Renji Hospital,Shanghai Jiao Tong University School of Medicine from June 2019 to July 2023 were retrospectively collected.The clinical data,serological indicators,endoscopic results and histopathological findings of all patients were analyzed.The data included age at diagnosis,sex,anti-parietal cell antibody(PCA),anti-intrinsic factor antibody(IFA),pepsinogen Ⅰ,Ⅱ and their ratio,gastrin 17,OLGA and OLGIM stages,red blood cell count,hemoglobin,mean corpuscular volume,vitamin B12 level,and the degree of anemia.Results:The average age of AIG patients with PA was 62.42 years old,including 11 males and 22 females.Twenty-four patients(72.7%)had mild anemia.PCA was found in 26 patients(78.8%).Iron deficiency anemia was found in 11 patients(33.3%).The mean vitamin B12 level was(92.7±42.0)pg/mL.The difference between the groups was not obvious and not statistically significant for patients over and under 65 years of age.There were significant differences in red blood cell count,mean corpusular volume,and OLGIM staging between patients with and without iron deficiency anemia(all P<0.05).Conclusions:The average age of AIG patients with PA was over 60 years,one third of the patients also had iron deficiency anemia,and the average vitamin B12 level was less than 100 pg/mL.Early diagnosis and intervention are essential for the prognosis of patients.
