Comparative study on the prognostic value of pericoronary adipose tissue CT attenuation and non-alcoholic fatty liver disease in patients with acute chest pain

Abstract:
Objective To evaluate the prognostic value of non-alcoholic fatty liver disease(NAFLD)and pericoronary adipose tissue(PCAT)CT attenuation for major adverse cardiovascular events(MACE)in patients with acute chest pain.Methods Patients with acute chest pain who underwent coronary computed tomography angiography(CCTA)were retrospectively selected.The time and type of MACE for all patients were recorded.MACE included unstable angina necessitating hospitalization,coronary revascularization,non-fatal myocardial infarction,and all-cause death.Patients' baseline,CCTA characteristics,right coronary artery(RCA)PCAT CT attenuation,and the presence of NAFLD were used to identifiy risk factors for MACE using multivariable Cox regression analysis.A comparative analysis was performed to assess the prognostic value of NAFLD versus RCA PCAT CT attenuation.Results A total of 514 patients were enrolled.During a median follow-up period of 31 months,60 patients(11.67%)experienced MACE.NAFLD[hazard ratio(HR)=2.599,95%confidence interval(CI)1.207-5.598,P=0.015)and RCA PCAT CT attenuation(HR=1.026,95%CI 1.001-1.051,P=0.038)were independent predictors of MACE.Global Chi-square analysis showed that NAFLD improved the predictive accuracy for MACE beyond clinical risk factors and CCTA metrics alone(global Chi-square:59.51 vs 54.44,P=0.024)and in combined with RCA PCAT CT attenuation(global Chi-square:63.75 vs 59.51,P=0.040).Conclusion NAFLD and RCA PCAT CT attenuation are predictors of MACE.NAFLD has an incremental prognostic value beyond that of RCA PCAT CT attenuation for predicting MACE in patients with acute chest pain.
