Abstract:
Background:Research has shown that there is a certain correlation between serum Ca2? levels and inflammatory markers in Crohn's disease(CD)patients,suggesting that serum Ca2? can be used for the assessment of CD inflammation levels.However,there is currently a lack of research in this regard.Aims:To explore the diagnostic value of serum Ca2?levels in the clinical and endoscopic different activities of CD patients and their correlation with some serological markers.Methods:This study is a single-center large-scale retrospective study.Hospitalized patients diagnosed with CD in Tongji Hospital,Tongji Medical College,Huazhong University of Science and Technology from January 2014 to December 2023 were continuously included.The Crohn's disease activity index(CDAI)was used to evaluate the clinical activity of CD patients,and the simple endoscopic score for Crohn's disease(SES-CD)was used to evaluate the endoscopic activity of CD patients.The differences in serum Ca2? levels among CD patients with different clinical or endoscopic activities were compared.Spearman rank-correlation analysis was used to evaluate the correlations between serum Ca2? levels and laboratory indicators,CDAI scores,and SES-CD scores.The receiver operating characteristic(ROC)curve was used to evaluate the value of serum Ca2? levels in judging the clinical activity of CD.Results:The serum Ca2? levels of CD patients in clinical remission,mild activity,and moderate-to-severe activity stages gradually decreased(P<0.001).There was no significant difference in serum Ca2? levels between the endoscopic remission stage and the mild activity stage(P=0.939),nor between the endoscopic remission stage and the moderate-to-severe activity stage(P=0.098),while there was a statistically significant difference between the endoscopic mild activity stage and the moderate-to-severe activity stage(P=0.021).The results of correlation analysis showed that serum Ca2? levels were significantly negatively correlated with ESR,hsCRP,hsCRP/ALB,CDAI scores,and SES-CD scores(P<0.05),and were significantly positively correlated with ALB and A/G(P<0.05).The results of ROC curve analysis showed that when the cut-off value was 2.19 mmol/L,the area under the curve(AUC)of serum Ca2? levels in judging clinical activity was 0.715(95%CI:0.652-0.777),the sensitivity was 83.1%,the specificity was 53.5%,and the Youden index was 0.366.Conclusions:Serum Ca2? levels can be used to evaluate the clinical activity of CD patients,but the effect of evaluating different endoscopic activities is not good.In addition,serum Ca2?may be an inflammation-related marker,which can be used together with serum markers such as hsCRP,ESR,and ALB for the assessment of CD inflammation levels.
