Aeromedical evaluation of cerebrovascular malformations in military aircrew:a clinical analysis of 20 cases

Abstract:
Objective:To explore the methods and standards of aeromedical evaluation for military aircrew with cerebrovascular malformations(CVMs).Methods:A retrospective analysis was conducted on the medical records of 20 aircrew with CVMs who were admitted to the PLA Air Force Medical Center from January 2000 to January 2020.Results:There were 11 cases with cerebral cavernous angioma(CCA)[CCA locations:occipital lobe(three cases),frontal lobe(two cases),temporal lobe(two cases),cerebellum(one case),posterior horn of left ventricle(one case),right basal ganglia region(one case),brainstem(one case)]. There were four cases with cerebral aneurysm(CA),three cases with cerebral venous aneurysms(VAs),and two cases with brain arteriovenous malformations(bAVMs). Among them,11 aviators were qualified for flight,three were temporarily disqualified from flight,and six were disqualified from flight.Conclusion:The aeromedical evaluation of CVMs in military aircrew should consider the type,size,location of the lesions,symptoms,signs,and complications of the malformations,as well as the types of aircraft,duties,flight experience,personal plan,and the opinions of working unit. It should be carried out in accordance with relevant standards and learn from the standard practices in developed countries and the 1% rule of civil aviation.
