Effect of hyperbaric oxygen combined with alteplase arterial thrombolysis in treating ischemic stroke

Abstract:
Objective:To investigate the effect of hyperbaric oxygen combined with alteplase arterial thrombolysis in the treatment of ischemic stroke and its effect on the levels of serum soluble intercellular adhesion molecule-1 (sICAM-1) and basic fibroblast growth factor (bFGF).Methods:A total of 68 patients with ischemic stroke admitted to the Affiliated Hospital of Putian University from June 2019 to June 2020 were selected and divided into control group and observation group with 34 cases in each. The control group was treated with alteplase arterial thrombolysis; and the observation group was treated with the combination of hyperbaric oxygen and alteplase arterial thrombolysis. The National Institutes of Health Stroke Scale (NIHSS) scores and clinical efficacy of the two groups before the treatment, 24 h, and 14 d after treatment were evaluated. The sICAM-1 and bFGF levels before the treatment, and 24 h, 7 d, and 14 d after the treatment were compared. After three months of follow-up, the recanalization rate of the affected vessels, the incidence of intracranial hemorrhage, the rates of favorable prognosis and the mortality of the two groups were analyzed.Results:The overall effective rate of the control group was 88.24%; and the overall effective rate of the observation group was 94.12%. The difference between the two groups was statistically significant ( χ2=0.732, P=0.393). After the treatment, the NIHSS score and the levels of sICAM-1 of the two groups were significantly lowered; and the bFGF level became higher. The differences were statistically significant ( P<0.05). As the treatment progressed, the NIHSS score and the sICAM-1 level gradually decreased; and the bFGF level gradually increased ( P<0.05). At any given time during the treatment, the NIHSS score and the sICAM-1 level of the observation group were all lower than those of the control group ( P<0.05); and its bFGF level was higher than that of the control group ( P<0.05). The follow-up showed that the favorable prognosis rate (85.29%) and the recanalization rate of the affected vessels (91.18%) of the observation group were higher than those of the control group (64.71% and 70.59%, respectively); and the differences between the two groups were statistically significant ( P<0.05). There was no significant difference in the incidence and mortality rate of intracranial hemorrhage between the two groups ( P>0.05). Conclusion:The application of hyperbaric oxygen combined with alteplase arterial thrombolysis can help with the neurological function of patients with ischemic stroke, reduce the expression of inflammatory factors, increase the content of bFGF, and improve the prognosis of patients.
