Analysis and intervention measures of PICC catheter-related infection in patients with prolonged disorders of consciousness

Abstract:
Objective:To analyze the risk factors of catheter-related infection (CRI) after peripherally inserted central venous catheters (PICC) in patients with prolonged disorders of consciousness (DOC) and put forward relevant nursing measures.Methods:The clinical data of 220 patients with DOC who received PICC catheterization in Department of Hyperbaric Oxygen of the Sixth Medical Center of Chinese PLA General Hospital from January 2017 to December 2019 were analyzed retrospectively. The patients were divided into infection group and non-infection group according to whether CRI occurred or not, and the risk factors were statistically analyzed.Results:Forty-two of the 220 patients had CRI after PICC catheterization with an incidence of 19.1%. There were statistically significant differences between the infection group and the non-infection group in age, degree of coma, underlying diseases, duration of bed rest, number of puncture during a single time of catheterization, and duration of catheter indwelling ( P<0.05). Conclusion:The risk factors related to the infection after PICC catheterization are as follows: 60 years-old or above, deep DOC, diabetes mellitus (underlying disease), duration of bed rest for more than 3 months, repeated puncture during a single time of catheterization, and catheter retention for more than 3 months. It is necessary to make nursing interventions against the above-mentioned risk factors in clinical work, thus helping reduce the risk of infection.
